Chapter 248: Chapter 200: No Longer What It Used to Be, Wu Ting’s Confidence

The debut of the 5.0 model once again left Shen Xiaofei and Fu Jinfen in awe. Clearly, this artificial intelligence had evolved yet again!

Backed by the vast database of Xuanshang University Affiliated Tumor Hospital, it could now locate the tumor’s position and borders with even greater precision. It could also simultaneously display the tumor’s proximity to intrahepatic blood vessels and bile ducts, showing their pathways and relationships...

Furthermore, it could calculate the probability of major hemorrhage after an incision along the corresponding liver resection line...

It could also provide real-time updates after the lesion’s removal, showing whether any tumor remained, the blood supply to the remaining liver, and the patency of the hepatic veins...

Looking at this brand-new 3D imaging and navigation platform, Director Fu and Shen Xiaofei were utterly amazed.

”This is incredible!”

”What hepatobiliary director wouldn’t want a system like this as standard equipment!”

”This generational leap is truly no less significant than the difference between traditional open surgery and minimally invasive laparoscopic surgery.”

The more they used this perfect trio of auxiliary technologies, the more Director Fu and Shen Xiaofei wished they could have such an assistive team for their daily operations.

Even if Wu Ting never performed another surgery for the rest of his life, the patent licensing for this system alone would be enough to let several generations of his family live in luxury!

This was especially true in the next step of the procedure: occluding the hepatic portal blood flow.

The patient’s lesion was extensive, so it was necessary to dissect the second hepatic portal.

Without Wu Ting’s auxiliary system, manually dissecting the second hepatic portal would be extremely difficult. Isolating and occluding the hepatic vein laparoscopically is incredibly risky. Any accidental damage to the hepatic vein or inferior vena cava could lead to the patient’s death within a short time.

However, with Wu Ting’s assistive technology, Director Fu handled it with ease and confidence.

”Ah, what a smooth operation!”

Director Fu truly admired Wu Ting. It was just a shame he wasn’t his student or in his department.

’Once Wu Ting’s perfect trio of auxiliary technologies is officially released, I have to get a set for the department, no matter what.’

After occluding the hepatic portal blood flow, the next step was the radical resection of the tumor.

The guiding principles were: a sufficient resection margin of two centimeters from the tumor’s edge and adherence to the no-touch isolation technique.

With the help of the auxiliary system and Director Fu’s surgical skill, the resection was naturally performed with precision. The specimen retrieval also went smoothly.

Of course, to be safe, the removed specimen still had to be examined to confirm if the tumor was completely resected and if the scope of resection met the standard for a radical cure.

After repeated verification, the results were satisfactory:

”Intraoperative tumor resection complete...”

”Resection margin achieved...”

Even though Director Fu and Director Shen were veterans, they waited anxiously for the confirmation. Once it came, they visibly breathed a sigh of relief, their moods lightening considerably.

Next was the management of the raw liver surface. The most critical tasks were hemostasis and preventing bile leakage!

”...Monopolar electrocautery knife... dissecting forceps... disposable clip applier... titanium clips...”

”...Fibrin glue... hemostatic gauze...”

At this stage, Director Fu dared not be the least bit careless, skillfully performing electrocoagulation to seal the small blood vessels and bile ducts...

Then, guided by the 3D imaging and navigation system, he precisely located the bleeding points and clamped them with titanium clips to achieve hemostasis...

After the raw surface was treated, they began the irrigation, confirming there was no further bleeding or bile leakage...

Meanwhile, Wu Ting’s immune negative regulation therapy began.

Flow cytometry... Mononuclear cells were isolated from whole blood treated with EDTA anticoagulant...

...Using fluorescently-labeled anti-CD3 and anti-CD4 antibodies...

...Negative regulation of TNF-α and NKG2A on the surface of NK cells...

As time went on, various indicators changed in real-time...

SIGIRR expression decreased...

Negative feedback led to TLR4 activation...

Levels of NKG2A on NK cell surfaces and pro-inflammatory cytokines such as IL-6 and TNF-α increased...

Anti-inflammatory cytokines like IFN-γ, IL-10, and IL-18 decreased...

Under Wu Ting’s direction, the regulation was completed in sync with Director Fu’s procedure, successfully inducing immunosuppression.

Even though they had cooperated with Wu Ting once before, Director Fu and Director Shen were still filled with admiration.

Wu Ting was a true genius. He had not only discovered the negative immune regulation mechanism in theory but could also design a concrete surgical plan for its clinical application and then execute it with practiced skill. This was the mark of a true clinical prodigy!

So far, the prognosis for Li Ai, the previous patient who underwent this therapy, was good. The surgery had been nearly flawless, and now it was just a matter of waiting for a follow-up examination.

After Wu Ting’s immune negative regulation therapy, the pathogenesis of the hepatitis B virus was radically addressed, transforming the patient from having chronic hepatitis B to being merely a ”hepatitis B carrier.” This was truly a stroke of genius!

While Wu Ting was operating, Cheng Feng, who was at the side, perfectly documented the procedure. Of course, the recording was selectively edited to feature Wu Ting’s perspective, his immune negative regulation therapy, and the perfect trio of auxiliary laparoscopic technologies as the main highlights!

[Ding! Congratulations to the Host for completing another classic demonstration-case surgery with Director Fu and Director Shen Xiaofei. You have obtained a trait from Director Fu: Hepatic Encephalopathy Diagnosis and Treatment Technique (Purple). Hepatic encephalopathy is a neuropsychiatric syndrome caused by metabolic disorders arising from severe liver dysfunction or impairment, leading to central nervous system dysfunction. This represents decades of Director Fu’s expertise, making him a top domestic expert in this field.]

[Note: The main clinical manifestations of hepatic encephalopathy include personality changes, intellectual impairment, psychiatric and behavioral abnormalities, and asterixis (flapping tremor). In severe cases, it can lead to disturbance of consciousness, coma, or even death. It is the most severe complication for patients with cirrhosis and is easily confused with psychiatric and neurological disorders.]
