Doctor: I Can Synthesize Infinite Entries

Chapter 203: Chapter 182: You Can Do This Too? Zhang Heng’s Mentality Collapses

Wu Ting had also been called over by Qin Donglai to participate in the consultation.

Under the entry that combined the EST (Purple) of many experts and Director Shen Xiaofei’s Gallstone Removal Technique (Purple), Wu Ting was also very curious to see what kind of treatment plan he would come up with for this patient.

However, he had just acquired a top-tier entry from the research institute at Jinggang University’s School of Life Sciences and was planning to take his time absorbing it. Therefore, he didn’t plan on being too high-profile.

Since Associate Chief Zhang Heng was preparing to be the chief surgeon, with Director Fu and Director Ma observing and supervising from the side, he believed it wouldn’t be his turn to step in.

He would just quietly participate in the consultation when the time came.

[Recommended Entry Synthesis: EST (Purple) + Gallstone Removal Technique (Purple) = ESPBD (Endoscopic Sphincterotomy Plus Balloon Dilation) (Black), a brand-new gallstone removal technique that combines the strengths of various experts in the field of hepatobiliary surgery in Jin Province.]

[This is a brand-new, original surgical plan based on EST, which uses balloon dilation to increase the operating space. Compared to the traditional EST technique, this method separates the bile duct and pancreatic duct openings with a small incision, preventing further trauma. It also guides the balloon to dilate toward the biliary sphincter. This provides a significant advantage when dealing with large stones, while also markedly reducing pressure on the pancreatic sphincter, which helps to mitigate the stress response.]

[Note: A Surgical Technique worthy of publication in a top-tier journal, with significant promotional value in the field of hepatobiliary surgery.]

Wu Ting was completely stunned. ’Is this system trying to stir up trouble?’

Director Zhang had gone to great lengths to invite so many experts, gathering all the elite physicians from the emergency department and hepatobiliary surgery. The stage was set, and he was all ready for his grand debut... ’Am I supposed to steal his thunder?’

Wu Ting thought about it and decided against it. He would let Director Zhang perform a conventional EST this time. He believed it would be sufficient to treat the patient.

Wu Ting waited quietly in the conference room.

Beside him, Yasui, Luo Ming, and the others were a little excited. Even Qin Donglai and Cai Qi looked pleased.

Only Director Huo sat ramrod straight, his expression unreadable.

After a short while, Zhang Heng arrived at the emergency department, bringing Director Ma and Director Fu with him. Gu Xiaodong, Liu Xingye, and Guo Guoguo followed behind.

Huo Yun and Cai Qi led a large group to welcome them.

After the two groups met, everyone exchanged enthusiastic greetings.

Zhang Heng, in particular, was beaming.

Wu Ting, on the other hand, was hiding modestly behind the crowd, but his attempt to keep a low profile was not to be.

”Dr. Wu, we meet again.”

”So you’re Dr. Wu. I’ve heard so much about you.”

The two directors, Director Ma and Director Fu, greeted him proactively, so Wu Ting had no choice but to step out from the crowd.

”Director Fu, Director Ma, good to see you both.”

Wu Ting sighed inwardly as he shook their hands, then greeted Liu Xingye as well.

The other doctors present weren’t surprised that the two directors greeted Wu Ting separately. After all, given Wu Ting’s many past achievements, it was normal for him to be recognized by these heavy-hitters.

However, the star of today’s show was still Associate Chief Zhang Heng.

”Wu Ting, you can help entertain the two directors on the side.”

Huo Yun just assumed that Wu Ting had made his face known in Haijin Port, and that Director Ma was simply being polite.

Huo Yun led everyone to the conference room. The core doctors gathered around the main table, while the rest sat in an outer circle.

”Dr. Wu, come sit over here.”

Director Fu gestured for Wu Ting to sit next to him.

Qin Donglai was a little surprised that Director Fu held Wu Ting in such high regard and appreciation.

”Then let’s begin.”

Qin Donglai took the lead, displaying the patient’s information.

On the way over, Director Fu, Director Ma, and Zhang Heng had already reviewed the patient’s various physical examination and imaging records, so they were already very familiar with the case.

With the supplementary information provided by Qin Donglai this time, they gained an even more detailed and comprehensive understanding of the patient’s condition.

After the introduction, Director Qin handed the microphone to Associate Chief Zhang Heng.

Zhang Heng smiled suavely. It was time for his performance.

Zhang Heng was quite experienced in treating cases like this patient’s:

”Fellow directors, I have a great deal of experience treating a condition like this patient’s... Because of the severe liver damage and the cirrhosis caused by chronic hepatitis B... I recommend using the new, minimally invasive EST!”

Listening to Zhang Heng’s analysis, the young doctors from City First Hospital were filled with anticipation. This was the first time an EST would be performed at City First Hospital, and it would be under the observation and supervision of Director Ma and Director Fu.

Director Zhang really had some clout.

Zhang Heng began to introduce his plan in detail:

”We’ll insert a cannula into the bile duct via guidewire assistance through the duodenoscope... Then perform a sphincterotomy with a needle-knife... According to the imaging data, the stone in the patient’s bile duct is quite large in diameter, so we’ll need to break it with a lithotripter and then remove it with a retrieval basket... After the stone is removed, we’ll flush the common bile duct with a balloon catheter...”

As expected of a senior associate chief physician from Jin Province Tumor Hospital, Zhang Heng’s explanation of the plan was detailed and thorough, having considered various possibilities.

After listening, Wu Ting picked up the patient’s imaging files and frowned slightly. The patient’s liver damage was quite severe, and the stone’s diameter was very large. If they used a conventional EST, there would indeed be some shortcomings.

Sure enough, just as Wu Ting was thinking this, Director Fu spoke up.

”I generally agree with Director Zhang’s surgical plan, but there are a few small issues.”

”First, the patient’s gallstone is very large. During an EST, the orientation of the papilla and the lower end of the common bile duct can easily change, making the direction and extent of the incision difficult to control. To remove such a large stone, the incision will have to be quite large, approaching the maximum limit. If not controlled properly and the incision is too long, post-traumatic recovery will be an issue.”

”Of course, if the incision isn’t long enough and we can’t get the stone out, a second incision would be an even bigger problem.”

”Furthermore, using mechanical lithotripsy will add more risks. That’s the first point.”

”The second issue is the patient’s poor liver function. A stress response may occur during the EST procedure, leading to immunosuppression. If the first issue isn’t handled well, it could delay the patient’s recovery and increase the risk of complications...”

”So, I think we need to carefully simulate and review the procedure beforehand.”

”This surgery places very high demands on the Technician’s operating skills.”

Director Fu was, after all, an expert in this field. His assessment of the problems was sharp and experienced.

Hearing Director Fu’s concerns, Director Ma also expressed his agreement.

”Director Fu’s concerns are very practical. However, as of now, compared to a traditional open surgery, EST is already the best option. As far as I know, there isn’t a better, more minimally invasive surgical plan with a faster recovery.”

”In fact, in terms of surgical design, I believe Zhang Heng’s plan is already the best possible one. The only question is the actual surgical execution.”

”In other words, the choice of chief surgeon.”

”However, I believe that with Associate Chief Zhang Heng’s skill and experience, there will be absolutely no problem. Plus, with Director Fu and myself supervising, I can’t imagine a better treatment plan.”

Director Ma was very supportive of Zhang Heng. Of course, in his view, this really was the best course of action available at present.

Hearing Director Ma’s response, Director Fu thought for a moment and had to agree.

Although he could see these potential pitfalls and risks, they were caused by the patient’s unique condition.

The gallstone was too large, the liver damage was too severe, and the patient’s constitution was poor.

It was already quite remarkable that Zhang Heng could propose such a detailed plan.

”Director Ma is absolutely right. I just have a habit of raising all potential problems and risks during the consultation phase so we can all brainstorm.”

”However, every surgery has its own risks and shortcomings. Given this patient’s physical condition and illness, the plan Director Zhang has presented is indeed the best one we have.”

”Unless there’s a more advanced, modified Surgical Technique that can solve these two problems.”

In the end, Director Fu also agreed to this plan, whose merits outweighed its flaws.

Watching the brilliant discussion between Director Ma, Director Fu, and Zhang Heng, the doctors from City First Hospital had their horizons broadened.

They were also very impressed with Zhang Heng for designing a plan that even Director Ma and Director Fu couldn’t suggest any improvements for.

After Director Fu also gave his approval, a smile appeared on the corner of Zhang Heng’s lips.

’It seems the surgery will begin next,’ he thought. ’In the operating room, I’ll continue to display my solid fundamental skills.’

Although he knew that it was impossible for there to be any further suggestions for improvement to a plan that both Director Ma and Director Fu had agreed upon, he still had to maintain appearances. Zhang Heng’s goal was to make every doctor present wholeheartedly convinced.

Zhang Heng then proceeded to ask Director Huo, Director Cai, and Director Qin for their opinions in turn.

Obviously, no one had any objections. The doctors from City First Hospital were not familiar with EST to begin with, so how would they dare to speak rashly?

Besides, Zhang Heng had even convinced Director Ma and Director Fu.

The three directors all gave Zhang Heng’s plan high praise and compliments, expressing their approval and admiration!

After seeking the opinions of the three directors, Zhang Heng had completed the formalities.

After all, there was no point in asking the other junior doctors.

Just as he was about to announce the start of pre-operative preparations, he saw Wu Ting sitting next to Director Fu.

Zhang Heng was currently full of swagger. Seeing Wu Ting, his interest was piqued. He decided to ask Wu Ting, wanting to hear his praise for the plan, to hear him admit his wholehearted admiration.

”Dr. Wu, do you have any opinions?”

”Today at the Jin Province Tumor Hospital, Director Fu and Dr. Liu gave you very high praise.”

”Last time, during my hepatic artery chemoembolization surgery, I also received a lot of pertinent advice from you. It allowed my skills to take a quantum leap. So, this time, I was wondering if Dr. Wu has any different ideas.”

”Dr. Wu has also been studying at the hepatobiliary surgery and Hepatic Tumor Surgery departments in Haijin Port lately and has read many cutting-edge journal theories. Do you have any good ideas that might solve the problems Director Fu raised?”

Zhang Heng had a deep understanding of the art of language. First, he would praise Wu Ting generously, putting him on a pedestal, while adopting a humble posture himself. In a moment, Wu Ting would have no choice but to return the praise, and even more extravagantly.

Zhang Heng looked at Wu Ting with a sincere expression, asking for his guidance.

Seeing Zhang Heng’s earnest face, Wu Ting felt a little embarrassed.

’I was just wondering how to find a suitable moment to present my own view,’ Wu Ting thought. ’This Zhang Heng is really so considerate.’

Wu Ting had wanted to keep a low profile, but for the sake of the patient’s safety, he had no choice but to steal Director Zhang’s spotlight.

His abilities simply wouldn’t allow it!

”You’re too kind, Director Zhang.”

”However, regarding the two risks of this surgery, I actually do have some ideas—a brand-new, improved plan.”

”I call it: ESPBD—Endoscopic Sphincterotomy Plus Balloon Dilation!”

Zhang Heng: o(一︿一+)o!
