Chapter 211: Chapter 188: A True Genius

Once the video call connected, Dean Chen Bofeng appeared before them all. An assistant stood beside him, holding a stack of the patient’s files that Director Fu had sent over.

Due to the unique nature of Li Ai’s surgery, Dean Song was also participating in the consultation.

Dean Song personally initiated the call with Dean Chen, which not only demonstrated how seriously they were taking the patient’s case but also showed his respect for Dean Chen.

Dean Chen was an extremely prestigious figure in the Central Region, a true top-tier authority in the field of oncology.

The Xianglin City Xuanshang University Affiliated Tumor Hospital was one of the most renowned specialized oncology hospitals in the Jin Country. Dean Chen’s team was considered to be at the forefront of the nation, especially in the diagnosis and treatment of primary liver cancer secondary to hepatitis B. Even the top teams in Qinghe City wouldn’t dare claim to be superior to Dean Chen’s.

After some official pleasantries, Dean Song, representing Jinggang University Affiliated First Hospital, thanked Dean Chen and his team.

Dean Song then proceeded to introduce the patient’s case, his tone filled with respect and admiration:

”Dean Chen, this patient’s identity is somewhat sensitive, so we are not at liberty to disclose the specific details. The patient is a national scientific researcher, and we’ve taken this on as a special assignment. That’s why we’ve temporarily assembled this surgical treatment team for Li Ai.”

”We are grateful that you could take time out of your busy schedule for this remote consultation, Dean Chen. The goal of this surgery is very clear: to extend the patient’s survival time as much as possible. To that end, we’ve assembled a team of experts from different fields to design an entirely new treatment plan.”

”The lead surgeon is Fu Jinfen, our hospital’s Director of Hepatic Tumor Surgery. The team also includes Director Shen Xiaofei from Hepatobiliary Surgery and Director Shen Siyuan from Cardiac Surgery. Director Shen Siyuan is particularly skilled in the application of immunomodulatory therapy during major surgeries, which is why we invited her to join us. We hope she can provide support from an immunological standpoint.”

”And this young doctor, Dr. Wu Ting, is another key member of our team. He and his staff will use a combination of auxiliary technologies—ultrasound, indocyanine green fluorescence imaging, and a 3D visualization system—to help the lead surgical team perform precise localization of the tumor.”

After finishing his introduction, Dean Song gave the floor to Director Fu.

As the one in charge of the operation, Director Fu began to present the patient’s case to Dean Chen.

”Dean Chen, I’ve already sent the general case files to your team. Now, I’ll go over the specifics again, as well as our team’s preliminary surgical plan...”

”The patient presents with intermediate-stage primary liver cancer complicated by cirrhosis from hepatitis B... The patient’s liver function is mildly impaired...”

”The tumor is located deep within the liver parenchyma. Fortunately, the tumor itself is relatively small, the lesion affects less than half of the liver, and it has not spread to the porta hepatis...”

”Therefore, our current plan is to perform a left trisegmentectomy. However, the patient is in poor physical condition with weak immune function. As such, we need to implement some form of immunological regulation to inhibit the postoperative replication of the hepatitis B virus and improve the prognosis...”

”Of course, we have not yet formulated a systematic immunomodulatory therapy. We invited you, Dean Chen, hoping to hear your opinion on any viable approaches.”

”Our Director Shen Siyuan has successful experience with mature therapies in the field of cardiac surgery, but the immunomodulatory mechanisms for primary liver cancer secondary to hepatitis B are different. That’s why we need your help in designing the plan, Dean Chen.”

After he finished speaking, Director Fu looked toward Shen Siyuan.

Shen Siyuan’s own ideas weren’t fully developed yet, so she held back and said humbly:

”Dean Chen, I haven’t formulated a complete plan myself. I only have a few fragmented ideas at the moment. I’d like to hear your thoughts first and see if they can provide some inspiration for our discussion.”

Shen Xiaofei, Fu Jinfen, and the others found Shen Siyuan’s humility completely normal. In fact, Shen Siyuan was from a younger generation of doctors; in front of an authority like Dean Chen, she was very much still a student.

Dean Chen nodded in understanding. He did, in fact, have some thoughts on the patient’s case.

”First, regarding the resection itself, I see no major issues with Director Fu’s surgical plan. The only thing is your team’s so-called ’triple-modality auxiliary technique’—I’m not familiar with it, so I won’t comment on it.”

”Moving on, the core of this treatment plan is how to manage postoperative replication of the hepatitis B virus, as well as issues concerning the patient’s immune system and potential complications.”

”I have given this some thought. I have a few ideas, but they aren’t fully developed yet either.”

”One option is an immunotherapy drug that my hospital currently has in the experimental phase. I’m sure Director Shen has heard of it—it’s PD-1 antibody therapy. This drug can activate immunocompetent T-cells to inhibit the growth of cancer cells... However, this antibody works for conventional viruses, but given the unique mechanisms of the hepatitis B virus...”

”Its therapeutic dosage and treatment cycle follow a parabolic curve. Within a certain range of dosage and duration, it can control the disease. But if it crosses a critical threshold, the patient’s condition can take a sharp downturn, as it will instead promote white blood cell-mediated damage to hepatocytes... The dosage and cycle control for this is extremely strict and also depends on the individual patient’s immune function...”
