System Came When the Doctor was Thirty

Tian Zhiliang, after transferring the patient to Vascular Surgery and Orthopedics with Lu Cheng, returned to the office to start documenting the patient’s basic information.

”Director Tian, in such surgeries, isn’t our Emergency Department’s task only to stop the bleeding, and all follow-up is handed over to specialists?” Lu Cheng asked as he watched Tian Zhiliang transcribe the medical record.

The Emergency Department at Xiangzhou People’s Hospital is quite different from that of Long County People’s Hospital!

By comparison, this place is more like a large specialized referral center.

Especially in surgical emergencies, aside from debridement and suturing, there are almost no surgeries to be performed independently.

Tian Zhiliang smiled amicably, ”Does your hospital have different practices?”

Lu Cheng nodded, ”We do perform some surgeries currently.”

Even though Tian Zhiliang was a Deputy Chief Physician in the Emergency Department at State People’s Hospital, he had heard about the county’s emergency department affairs: ”In that case, you have more guts than us.”

”Our hospital currently follows the practice of strictly adhering to departmental business divisions. Our Emergency Department, we just handle emergency procedures.”

”We provide simple treatment for the patient’s condition, addressing only some critical points.”

”For instance, debridement and suturing, intubation, and tracheotomy. Other operations, like closed thoracic drainage, are performed by the Thoracic Surgery personnel themselves.”

”It’s to avoid complications, after all, the Emergency Department already has plenty of troubles.” Tian Zhiliang replied truthfully.

Lu Cheng replied, ”Director Tian, that seems all right too.”

As they talked, Lu Cheng saw his own interface.

[Through effective hemostasis technique, you promptly addressed the patient’s active arterial injury, reducing the risk of shock and death to a certain extent, and gained 5.3 skill points.]

The most crucial treatment for active arterial damage is not vascular suturing, but timely hemostasis to save lives.

Although Lu Cheng only performed the simplest hemostasis technique, the skill points gained were comparable to a Level 3 Surgery, which wasn’t a small gain!

Tian Zhiliang was meticulously recording the patient’s general information and condition upon admission. The patient had already been taken to the operating room, and the specialist doctors had arrived.

After the consultation, the patient was automatically transferred to the respective specialist department, and the current task for the Emergency Department was merely to complete the basic medical record.

Lu Cheng watched quietly without offering additional suggestions.

After finishing writing the illness course, Tian Zhiliang turned and said, ”Dr. Lu, in our hospital’s Emergency Department, the two operations we emphasize are incision and hemostasis.”

”These two procedures might seem unrelated, but in reality, they both aim to preserve the patient’s life as much as possible.”

”The topics of your operational exam will revolve around these too.”

”I’ve finished documenting the course of the illness in this medical record. Since you are here for assessment, write the admission conversation record and critical illness notice.” Tian Zhiliang still assigned tasks to Lu Cheng.

Lu Cheng nodded, ”Okay, Director Tian. Should I talk to the patient’s family?”

Tian Zhiliang said, ”There’s no need for that, just print it out and hand it over to the emergency operating room later. The specialist doctors in the emergency operating room will handle the discussions and signatures with the patient’s family.”

”If the specialist doctors don’t arrive in time and the patient’s condition is extremely critical, then we will need to talk to the patient’s family.”

As Tian Zhiliang spoke, a call came from the duty doctor in the Emergency Surgery Clinic: ”Director Tian, there’s a case of tendon rupture here.”

”There’s bleeding, Director Tian, can you ask Liang Jianhong to help with a temporary dressing? I still have a few patients here.”

Tian Zhiliang turned and called out, ”Liang Jianhong, please go to the procedure room and give the patient a temporary dressing.”

Liang Jianhong, responsible for the daily work of the observation ward patients, was quickly up, ”Okay, Director Tian, I’m on it.”

Tian Zhiliang went on to explain to Lu Cheng, ”Xiao Lu, here, we generally have a Level 3 duty system. Both internal and surgical departments have either a Senior Associate Chief or a seasoned Attending to work together with the Emergency Clinic and Ward Shifts.”

Lu Cheng said, ”Director Tian, then aren’t you and Director Zhu on duty every day?”

The Emergency Surgery Department has three senior titles in total, Xiao Zhaoxi is the Chief Physician, while Tian Zhiliang and Zhu Yuanzhou are the other two Deputy Chief Physicians.

Tian Zhiliang said, ”No, the secondary shifts for the senior associates are rotated among me, Zhu Yuanzhou, Director Xiao, and Cao Gui.”

”Of course, Director Xiao only takes one single shift every two weeks. Generally, it’s us three who rotate.”

Lu Cheng worked quickly, as Director Tian said, the two documents, once the templates were set up and adjusted, were manageable.

State People’s Hospital’s Emergency Department has been in operation for many years, and the templates are very mature, not something Lu Cheng could easily find errors in for correction.

After checking that there were no issues with the two templates, Lu Cheng printed them out.

”Director Tian, after printing these, is there anything else?” Lu Cheng asked.

”Supplement the doctor’s orders. I’ll handle it; you wouldn’t be familiar with our department’s order sets, and it’s too tedious to search for them.” Tian Zhiliang had already completed the previous orders on another computer.

The main additions were routine orders like oxygen supplementation, blood pressure measurement, as well as debridement kits and billing for clinical emergencies like debridement and hemostasis.

After verifying the doctor’s orders, Tian Zhiliang turned and asked, ”Dr. Lu, at Long County People’s Hospital, were you frequently involved in emergency rescue work like this?”
