System Came When the Doctor was Thirty

”Is splenectomy a surgery recommended by the guidelines? Can it be routinely performed in our hospital?”

Upon hearing this, Lu Cheng’s spine went cold!

Goodness, this is where the loophole is.

Spleen Preservation Surgery is different from Limb Replantation Surgery. In Limb Replantation Surgery, the standard is singular: replantation of the severed limb.

An important part of limb replantation is the suturing skill, so if you say you just did the suturing, making everything favorable for the patient temporarily, that’s acceptable.

Otherwise, would you just amputate when encountering such cases?

That certainly wouldn’t do.

But Spleen Preservation Surgery is different. The officially recommended surgical technique is splenectomy; why would you suture it up?

Lin Qianlong, clenching his voice, said, ”But we can’t just watch the spleen, if there’s a chance to save it, and let it be removed outright, can we?”

Tian Qi continued to question, ”Why is there still a chance of saving it? How did you come to this conclusion? What’s the professional standard?”

”According to which standard?”

”You say you assessed it independently? Director Lin, as far as I know, a Vice Director doesn’t have this autonomy, right?”

Lin Qianlong fell silent again.

This was aimed at him deliberately; they must have all kinds of evidence locked in. How could he get out of this?

Tian Qi continued, ”Director Lin, do you know the basic qualifications required to conduct a new surgical technique? Related to our hospital’s tier?”

”Firstly, there is one requirement: to pass a special training or further studies assessment for the new technology.”

”May I ask, where are the new technology training certificate and qualification assessment certificate of your team? Show them to me.”

Tian Qi continued reaching out, asking Lin Qianlong for the paperwork.

Lin Qianlong shook his head, closed his eyes, and replied, ”Director Tian, that’s not available either.”

”During the operation, who were the participants?” Tian Qi continued the investigation, as if going through the motions.

Lin Qianlong replied quickly, ”I was the primary surgeon, and everyone else were assistants.”

This was recorded in the surgical notes; Lin Qianlong couldn’t deny it. If he were to refute this, it would only create more trouble.

For instance, why would a Vice Director be operating while a resident surgeon isn’t? Based on what considerations?

Tian Qi immediately looked at Zhang Tiesheng: ”Vice Director Zhang, you were also present at that time. Are you aware of the formal procedures for implementing a new surgical technique?”

”If you were aware, why didn’t you stop it in time?”

Zhang Tiesheng, clueless, shook his head vigorously: ”I hadn’t thought of that; I didn’t check that clearly before.”

”Moreover, when the patient came, everyone was in a hurry, so we didn’t consider these details.”

”What about Dr. Lu Cheng?” Tian Qi only mentioned Lu Cheng in passing.

With two Vice Directors present, Lu Cheng, being a resident surgeon, was the least relevant, but he also needed to be examined.

Lu Cheng also shook his head: ”I’m not sure either.”

There are too many medical regulations, so thick that few people can remember every situation clearly.

Moreover, Tong Yuan’an had given Lu Cheng a set of rhetoric, simplifying the surgery into something straightforward.

However, Lu Cheng didn’t realize that this ”rhetoric” only applied to Limb Replantation and couldn’t be used to gloss over Spleen Preservation Surgery!

”So, Director Lin, you admit there were no violations in the unauthorized surgeries conducted in the operating room previously.”

”Everything recorded here is authentic, not altered?” Tian Qi continued to ask.

Lin Qianlong thought for a moment, but still didn’t nod: ”Director Tian, I didn’t violate the principle of non-maleficence; the patient’s postoperative outcome was excellent! Otherwise, the patient’s family would have already complained.”

Tian Qi said, ”Yes, the patient’s family didn’t complain, but the problem now is that these surgeries have been uncovered.”

”There are a few issues: First, the surgical costs for this patient were higher compared to similar conditions, despite no infections or complications.”

”The proportion of insurance payments was also higher than for other patients. What do you have to say about that?”

”Secondly, the hospitalization period for these patients was longer, with no explainable complications. When you report this data, doesn’t it indicate special circumstances?”

”Thirdly, if you wish to implement these techniques, you truly did not report them, nor could we see a comprehensive departmental discussion record in the medical history, let alone a risk report from the Medical Affairs Department.”

Hu Qingshan heard this, scratched his head vigorously, and added, ”Director Lin, the problematic point now is that the inspection team is focused on these medical records and has found logical gaps.”

”They have mandated the State Health and Health Commission and the medical insurance department to conduct a thorough and strict investigation, not only into the financial aspects but also regarding procedural justice issues.”

”Conducting a new surgical technique without reporting is mildly profiteering, but in severe terms, it’s human live experimentation!” Hu Qingshan emphasized.

Human experimentation is one of the taboos of medicine; live human experimentation is a major taboo among taboos.

Once it reaches this level, not to mention a Vice Director, even a top professor can be utterly destroyed in minutes.

Lin Qianlong was very weary: ”Director Tian, Dean Hu, my resignation is not to evade the responsibility of these surgical techniques, and you are welcome to investigate my financials.”

”Moreover, the patients had good outcomes, and both family and patients were satisfied with the treatment results.”

Tian Qi cursed outright: ”What qualifications and credentials do you, Lin Qianlong, have to conduct new surgical techniques? Especially without reporting them?”

”Where are your credentials? Show me! Use your head, will you?”
