System Came When the Doctor was Thirty

November 10th, Tuesday, weather clear.

Night shift in Zeng Huanqi’s emergency room combined with Tian Zhuang’s ward night shift, the surgery shift is under Director Du Qiang.

When Lu Cheng arrived, Director Du Qiang had already reached the department, standing arrogantly, fingers interlocked.

As Lu Cheng entered the doctor’s office, Du Qiang glanced at him casually and said, ”The Dr. Lu you’re looking for is here.”

Lu Cheng looked at the people seated around the table, and his heart tightened immediately.

Besides the familiar Zhang Tiesheng, Huang Haibo, and former emergency department Director Lin Qianlong, there were faces Lu Cheng did not recognize.

They were in uniform, accompanied by the hospital leadership.

Du Qiang looked at Lu Cheng and said rather officially, ”Lu Cheng, please change into work attire.”

”Alright, Director Du.” Lu Cheng quickly left the office and returned after donning a white coat.

Vice President Hu Qingshan said rather officially, ”Director Tian, now the people you were looking for have all arrived.”

”The previously sealed medical records have also been copied and are here...”

Tian Qi, sitting at the center, glanced at Lu Cheng unexpectedly and bizarrely said, ”It’s Dr. Lu Cheng again.”

Lu Cheng thought Tian Qi was calling his name, and replied, ”Hello, leader, I am Lu Cheng, a doctor in the emergency department, currently holding the title of attending physician.”

Vice President Hu Qingshan introduced in an official tone, ”Dr. Lu Cheng, this is Director Tian Qi from the Legal Affairs Section of the Health Bureau, and leaders Zhou Wenshu and Tan Ling also came to our hospital for inspection.”

”Later, be sure to cooperate truthfully.”

”Alright!” Lu Cheng nodded and sat down.

Zhang Tiesheng then got up, walked to the door, and temporarily closed the doctor’s office door.

Tian Qi then said, ”Director Lin, although you have resigned, we received information that mentioned your involvement in unauthorized surgeries while working in the emergency department at our county’s Renmin Hospital.”

”We have also previously discussed these issues carefully, and now that all related personnel who assisted during surgery are present, do you have anything to explain?”

Lin Qianlong’s expression was a bit troubled at this moment; after his eye corners and lips twitched for a while, he said, ”Director Tian, you have found me, I imagine you already hold very detailed information.”

”But I must explain a point, I have not violated the principle of non-maleficence. The surgeries we conducted were focused on preserving the patient’s organs and providing a better quality of life for the patient.”

Tian Qi pressed his hand down, ”Director Lin, whether the surgery had a significant impact will be discussed otherwise.”

”What we are currently discussing is the specific surgical technique.”

”According to the information we have, surgeries including but not limited to Spleen Preservation Surgery, which are not listed within the range of surgical techniques catalogued by the National Health Commission, are considered new techniques.”

”I would like to ask Director Lin if the surgeries you previously conducted were reported to the relevant health departments? Were new technologies strictly graded?”

”Were they discussed by a professional committee and received approval from the ethics council and other related work?”

Tian Qi’s questioning was very official; Lin Qianlong swallowed hard, pale-faced, unable to squeeze out a word.

Tian Qi’s tone turned more serious: ”According to the new technology, new business management system, new technologies are categorized as national, provincial, and hospital levels.”

”The national level of new technology is defined as having world-leading new achievements and projects not yet conducted or new medical and nursing services not yet used in the domestic medical field.”

”Provincial level is defined as having advanced national achievements, new projects not yet conducted, and new medical and nursing services not yet used within the province.”

”At the hospital level, it refers to advanced provincial achievements for new projects and new medical and nursing services not yet conducted within the city or hospital.”

”Director Lin Qianlong, did you evaluate the surgeries conducted in your department? Which category do they belong to?”

Lin Qianlong, guilty, replied, ”Hospital level.”

”Which units in our prefecture-level city are conducting them regularly?” Tian Qi further pursued, leaving no room for evasion.

Lin Qianlong was at a loss, his mouth opening and closing several times without a sound.

Lin Qianlong glanced at Huang Haibo, but Huang Haibo didn’t want to give him any attention, seemingly intending to embody the saying ”out of sight, out of mind” thoroughly.

Du Qiang, on the other hand, dared not touch these matters.

”Alright, then in our province, which units are conducting them?” Tian Qi continued to ask.

”Xiangya’s hospitals are routinely conducting them!” Lin Qianlong quickly said.

”Did you report it? Did you organize all physicians in the department for discussion, and where are the discussion records? Where is the ethics council’s approval?” Tian Qi extended his hand to ask Lin Qianlong for the documentation.

How could Lin Qianlong provide it?

”Is there a safety risk assessment form from the professional committee?” Tian Qi continued to extend his hand to ask Lin Qianlong for files.

This isn’t about forcing anyone; it’s primarily about the notion that if something is truly good, the procedures should be attainable.

If the process can’t uphold justice, then the medical technology is also unlikely to be positively oriented. Otherwise, why wouldn’t you go through the process?

Lin Qianlong thought for a moment and remembered a previous response he had prepared for Lu Cheng. He immediately said, ”Director Tian, I have something to say here. We never intended to perform Spleen Preservation... it was just that during the patient’s situation at that time... it was impossible to transfer.”

”So we conducted a tentative suture, striving to...”

Tian Qi interjected, ”Striving for what? Is splenic rupture not part of the recommended standard surgical procedure? Is there no standard treatment surgery?”
