Nile Builder: The Architect from the East

Chapter 96: Chapter 4: The Medicines Are Sifted — The Rules of Storage Take Shape

Shafi’s old war ledger ran to its last page on the sixteenth day after the siege was lifted.

For those sixteen days she had walked the same road every morning: out of the field hospital first, then south to the medical stall at the surrender point, to look at the wounds of the men who had come over the day before; then doubling back north to the medical point in the Haven of Settled Lives, to ask after the children’s coughs and the birthing mother’s movements; and at last home to the field hospital, where the numbers from all three places were gathered into one. Three points, one line — and her medicine ledger hung from that line like a thing on a rope. The morning road was her own invention; no one had ordered it. She had simply found, in the first days after the siege, that the numbers would not stay straight unless she saw the people behind them with her own eyes — at the surrender point, the morning queue already forming, the men standing with their bowls and their wounds in the same patient order; in the Haven, the children running out to meet her because her coming meant the medicine shelf was open. The rounds had taught her the city’s geography of pain: where it gathered at dawn, where it hid at noon, where it came out after dark.

”This city,” Nefer said once, meeting her on the way, ”you walk more rounds than the night patrol.”

”Accounts wait for no one.” Shafi said. ”A wound at the stall, seen a day late, rots a day more. A sickness in the shelter grounds, seen a day late, spreads to one more household. The faster I walk, the sooner the medicine follows.” She said it without slowing, and Nefer, who knew better than to try to slow her, stood and watched her go.

The last page was short. It recorded the night of the decisive battle: one hundred and seventeen packs of staunching cloth, two and a half chests of wound medicine, and every boiled cloth strip in the place, used to the last. That single night had been the most expensive night of the four months, and the page carried it in five short lines. Below that came the new accounts of the days after the war — the medical stall at the surrender point had seen more than three hundred men; the medical point in the Haven opened its door every day without fail. She drew a hook at the end of the page, closed the ledger, and pressed her fingers against its cover. The cover was worn soft at the corners, the way a thing is worn by being carried, opened, and closed, day after day.

”Teacher,” she said to Imset, ”the old account needs to be gathered up.”

”Gather it.” Imset said. ”In these next few days. Before the next battle, the four months of this ledger have to become something that can be handed on.” Handed on, he meant, to the next war — and to every medical house that would ever stand in Egypt.

”Something that can be handed on.” The words stirred something in Shafi. She went to the bottom shelf of the medicine rack and brought out every book of the four months — old ones, new ones, broken ones, mended ones — and stacked them, pile by pile, on the long table in the hospital’s open yard. Some were bound in leather, some in cloth; a few were no more than folded sheepskins tied with cord — the hospital had used whatever it could find in the siege. The piles stood tall and low, crooked and straight, like a small wall built of four months, held up by nothing but the care of the hands that had stacked it.

”Where do we begin?” She asked.

”At the beginning.” Imset said. ”The first day of the siege.” He said it without looking up; he was already reaching for the oldest book, the one whose corners had gone softest.

The gathering began with counting the people. It began with the people because the people were what the whole account was for; the medicines came after, the buildings after them, and the rules at the end — each layer resting on the one beneath.

The first entry was the treated. In the four months of the siege, the field hospital had taken in one thousand one hundred and thirty-seven cases of the wounded and the sick, of whom three hundred and forty-two were serious. That number had long stood in the old account, counted out by lamplight, in the intervals between the carrying in of stretchers. Shafi worked on from it: the two months of the tug-of-war at the walls had brought the most wounded — the wall broken five times and mended five times, and every break sent a fresh batch of men down — the menders and the wounded were often the same men, a week apart; the fire-arrow raids at night had brought the burned, wave after wave, their dressings needing changing twice a day while the salve jars emptied faster than the potters could fire new ones. In the days when the water was cut off, the blade wounds had grown fewer and the diarrhea and the weakness had grown more — most of it from drinking the muddy water; the children had suffered worst of all. After winter set in, the coughing and the frostbite had joined the rolls, the frostbite coming first to the men who stood the night watches on the wall.

”Look at this number.” Shafi drew a line with her charcoal stick, and then a second line under it, the way she marked the sentences that mattered. ”The wounded do not come evenly. They come in waves. The way the war is fought is the way the men are hurt.”

”War has its temper.” Imset said. ”Men have their temper. Medicine must follow the temper too.”

But the ledgers recorded numbers; what pressed down beneath the numbers were people. The gathering had begun as arithmetic. It did not stay arithmetic for long. Page by page, the people of the siege came back — not as entries in a register, but as faces, and wounds, and the particular way each one had walked through the hospital door.

Shafi turned to a page, and her brush slowed. The page carried only five words: ”Arrow wound, shoulder, seven days.” She said: ”This is A-Ka. He was hit by an arrow while mending the wall; the shoulder wound went untended half a month before it was treated. If he had not reached the hospital in time, that arm would have been lost. Now he is healed, and he is back on the wall.” The five words had covered a month of pain that he had never once mentioned to anyone. She paused. ”Teacher, in gathering these accounts I have come to understand — every number is a person. When the person mends, the number is alive. When the person is gone, the number lies here, and cannot breathe.”

”That is why the account must be kept.” Imset said. ”Keep it, and the people are done justice.” He said it without looking at her, the way he said the things that cost him something.

There were other pages, from the days the water was cut. The children with diarrhea were many, most from the muddy water — the water that came into the quarter’s jars when the rebels dammed the clean intake, and that the whole quarter drank for a tenday before the new wells were dug deep enough. One child had been drained until he was a husk of himself; when his mother carried him in, he was burning with fever and talking nonsense. She had carried him the whole way from the far quarter, and her arms were still shaking when she set him down on the table. The hospital gave antidiarrheal medicine, paired with thick boiled rice-gruel. The child lived. The account said: ”Antidiarrheal, two days, recovered.” Shafi said: ”Those three words were kneeled for at the hospital gate by the child’s mother.”

”Kneeled or not,” Imset said, ”on the account it is one word: recovered. A healer records numbers, not thanks. But every line in this account has a person standing outside it.”

The hands of the wall-builders filled many pages too — the shortest entries in the whole account, and the most repeated. The hands that rammed earth and shaped mud-bricks — grazed by arrows, crushed by falling bricks, split open by the frozen ground. Hand wounds were seldom serious; but when a hand was hurt, the wall lost a pair of hands, and the wall could not spare a single pair. The account noted: hand wounds, more than one hundred and ten in all, most of them light — wound salve and cloth strips, and in three to five days the man was back at work. The entry did not record which hands had built which stretch of wall; the wall itself remembered that.

”Teacher,” Shafi said, ”look at this line. ’Hand wound, light, back to work in three days.’ Behind those six words stands the wall that never fell in four months.”

”Hands on the wall,” Imset said, ”and the wall in men’s hearts. Both are accounts.”

The second entry was the medicine. If the people were the first entry, the medicines were the second — and the medicines, unlike the people, could be laid out on a table and counted.

Six shipments of medicine — what each one had brought in, what each one had left — Shafi had recorded them all. The records had come from three places: Meru’s boat lists, the transfer depot’s ledgers, and her own unloading count at the dock, and when the three sets of numbers disagreed, she went back and counted the chests again. She sorted them by kind: the styptics, the wound medicines, the antidiarrheals, the fever medicines, the cough medicines, the salves for dressing wounds. When the sorting was done, she drew the tenday consumption of each kind as a series of jagged lines, running side by side down a long strip of sheepskin that had once been a cargo list. The lines rose and fell together like the courses of a wall, and the shape of the war was written in their jumps.

The lines, once drawn, told their story plainly: in the tendays thick with night raids, the wound medicines and the staunching cloth shot upward; in the days after the rebels threw dead sheep into the channel above the city, the antidiarrheals ran to the bottom of the jar; when the weather turned cold, the cough medicines and the fever medicines climbed. The steadiest line of all was the salves — no matter how the war was fought, the salves were consumed evenly, neither fast nor slow. Of all the lines, that one was the one she trusted most — the medicines that jumped could be watched and guessed at, but the one that never jumped had to be planned for.

”Teacher,” Shafi said, pointing at the steadiest line, ”I used to think the most precious medicine was the one used up fastest. Now I see — the most precious is this steady one. The war can wait; a wound cannot. Salves must never be allowed to run out for a single day.”

”What cannot be allowed to run out,” Imset said, ”is no longer mere medicine. It is destiny. Set it apart in its own line — from now on, when the medical houses of every region lay in their supplies, this one is secured first.”

The wounds also followed the seasons. It was a thing the old doctors had known without being able to say it — and Shafi, with the four months laid out before her in numbers, could at last say it in lines.

While she was gathering the accounts, Shafi divided the four months into three stretches. The first stretch was the opening of the siege, when the weather was still hot. Then the great fear for a wound was rot — sweat and dust got into a cut, and in two days it festered. What was used most then was barley beer for washing wounds and pounded herbs for dressing. The second stretch was the two months of the tug-of-war, when autumn had come and the wounds rotted more slowly — but the rebels’ fire arrows came, and the burned were carried into the hospital wave after wave, needing cold water, oil-salve, and soft cloth, not one of which could be spared. The third stretch was the present — winter — when the arrow wounds and blade wounds had grown fewer, and the coughing, the frostbite, and the chills took more than half the beds, and the fever medicines and cough medicines were climbing before her eyes.

”The same hospital,” Shafi said, ”three seasons, three kinds of sickness. From now on, every region must stock its medicines by the season. When the weather is about to turn cold, the cough medicines have to enter the granary first.”

”The seasons.” Imset said. ”Write those two words into the book as well. A medical house is not a trade that opens for a day; it is a livelihood that turns with the four seasons.” He spoke of the seasons the way the farmers of the river spoke of the flood — a thing that would surely come again, and could be survived only by those who had made ready.

The third entry was the spoilage. It was the kind of lesson that is written in no book until it has been paid for once, in spoiled medicine. The summer lines sat at the top of the page like a warning, and the winter lines below them like a relief.

Spoiled medicine accounted for nearly half of all the losses — and of all the losses, it was the one that stung most, because it could have been prevented. The first two shipments had come in summer: some had mildewed on the road, some had dampened in the storehouse, and Shafi had recorded each one: which pack had come wet, which had gone bad before it was found, which had only smelled wrong when it was brewed. She drew these into lines too — the two summer shipments spoiled most; after autumn, the spoiling fell away.

”Damp rises from the ground.” Shafi said. ”Medicine laid on the earth goes soft in three days. Raised up, it keeps longest. This account says it plainly — raise it half a foot, and the spoiling is cut by more than half.”

”That is why,” Imset said, ”when this hospital was first built, the medicine stores were raised half a foot. The granary’s logic is the medicine’s logic.”

”There is more.” Shafi opened another page. ”The granary taught me another thing: keep the dry separate from the wet, and keep what taints its neighbors by itself. In the first two months I kept the mint and the garlic on one shelf, and the two tainted each other. After that I separated them, each to its own. It is all noted here.”

The two of them worked — one turning pages, one writing — in the same silence they had kept through four months of night shifts, the silence of people who do not need to talk because the work itself is talking. The stacks of books in the yard sank lower, pile by pile; the new-written sheepskins grew thicker, page by page.

On the third day of the gathering, the apprentices came to help. The word had gone round the hospital that the four months were being gathered into one book, and they came of their own will, bringing their own charcoal sticks and their own lamps.

The copying was divided among those who could read well. One young medicine-boy copied a row of figures wrong, and broke into a sweat. He had been at the copying since noon, and the error sat in the middle of a long column; he waited for the scolding with his head down. Shafi came over and looked — she did not scold. She only drew a single line through the error with her charcoal stick and said: ”When an account is wrong, strike one line and write it again. The thing a medical account fears most is not an error; it is an error hidden. A hidden error, by the time it reaches a patient, is a life.” The medicine-boy’s face changed as he heard it — the fear went out of it slowly, replaced by something steadier.

The medicine-boy nodded and copied the row again, stroke by stroke. An apprentice beside him said: ”In the old days, mixing medicines went entirely by the hands’ feel of the masters. How much was much, how little was little — there was no number for it. Now that this book is coming out, we mix by the numbers, and the hands do not tremble anymore.”

”The hands not trembling,” Shafi said, ”is only the half. The heart must not tremble either. When the numbers are right, the people can be right.”

Imset listened from the side and said nothing — he only nodded. He had watched her teach before, and he never interrupted her when she was teaching.

The copying was mostly done at night. By day the yard belonged to the wounded — to the dressings, the porridge, the endless coming and going — and the books had to wait for the dark, when the table came free and the only sound left was the wind. The hospital’s door could not close by day; only when the wounded had all lain down did the long table come free. The apprentices lit three lamps and sat in a ring around them, one charcoal stick apiece, copying from the old books. The flames leaned toward one another, and the heads above them leaned together with the flames. The slowest copier was the smallest medicine-boy — he knew few characters, and for every line he copied he had to ask three times. But he copied the most carefully of all, stroke by stroke, slowly, as if he were carving. The others had long since learned to read the old pages at a glance; he read them letter by letter, and for that reason he never missed a stroke. His lips moved as he copied, sounding out the characters he knew and memorizing the ones he did not.

”A-He,” Shafi said one evening, calling in the boy from the kitchens, ”if you have time at night, come and help dry the pages. If the ink is left damp, it runs, and the account is spoiled.”

A-He agreed. He crouched in the lamplight and spread the finished sheepskins one by one, pressing the four corners down with small stones. He spread one, and looked at one — he could not read much of it, but he knew that what was written there was the thing that had saved his life. He had come to the hospital from the kitchens without being asked, and after that first night he came every night, saying little.

”Sister Shafi,” he said in a low voice, ”is there something on these pages for the cough that my mother has?”

”There is.” Shafi said. ”The cough medicines have their own column — the treatment and the dose are both written there. When the war is over and your village’s medical point is raised, your mother will have nothing to fear.”

A-He said nothing more. He bent his head and went on spreading the sheepskins, laying them even and straight, as if he were spreading a bed for the words of the whole yard to sleep on.

One apprentice dozed off near midnight; his charcoal stick dragged a long black line across the sheepskin. He woke with a start, his face gone white. Shafi came over and looked, and said: ”That line is drawn straight. Leave it — let this book record that you once dozed off.” The whole room of apprentices laughed. The one who had dozed scratched his head, then worked the black line into the shape of a small sun — a seal stamped on that page’s account. The little sun sat in the margin from that day on, and the apprentices took to calling it the account’s watchman.

”Wake yourselves up.” Shafi said. ”An account is the one thing that cannot bear sleep. When a man is drowsy, his hand goes crooked; when the hand goes crooked, the number goes wrong; when the number goes wrong, the medicine goes wrong. We would rather lose a night to slowness than lose one stroke to error.”

”Lose a night, then.” Imset came out from the inner room and said. ”This book is for the people who come after us. They will not look at how many nights we stayed awake. They will only look at whether our strokes are wrong.”

In the lamplight, the sound of copying went on, soft and dry, like the wind of four months blowing once more across the pages. The lamps burned low by then, and outside the yard the night had gone the colour of old ink; inside, the writing went on, page by page, the four months being set down a second time.

The rules for grading the use of medicine were also re-ground, in these same days, out of the account — re-ground as a blade is re-ground, for the rules had been worn down by four months of use, and the gathering of the account put an edge back on them.

Four months had brought several times when medicine ran short. In the tightest time, wound medicine had held out only twelve days. The three rules laid down then — the critical wounded first, the light cases in reduced doses, and food in place of medicine where it would do — were now stepping out of the account, one by one, each with its numbers behind it. They had been made in haste, in the dark days, spoken once and remembered by everyone because lives depended on them; now they were being written down, and writing made them law.

”Critical first — there are numbers.” Shafi said. ”On the night of the decisive battle, all the lightly wounded were turned to the little dressing shop at the lane mouth, and the hospital took in only the critical. That night, the hundred-odd critical cases — not one of them was delayed.” The lane-mouth shop had been Imset’s idea, and it had worked so well that the hospital had kept it open ever since.

”Light cases in reduced doses — there are numbers too.” She turned to another page. ”In the days of the fire-arrow raids, the doses for the dozen-odd burned were cut to seven-tenths, paired with cold-water dressings. Three-tenths of medicine saved — and the wounds healed no slower; they only took a few more days.”

”Food in place of medicine,” she said, pointing at a column of dates and honey, ”that is Sister Wei’s doing. The western line’s country remedies — honey on wounds, wild garlic pounded to paste, wounds washed with barley beer. How much dressing medicine these remedies saved, the account has it all.”

”A remedy may be a country remedy,” Imset said, ”but an account is a proper account. When a country remedy enters a proper account, it can be handed on. Write these down, every one of them, into the book.”

Shafi agreed. That same day she sent for Sister Wei and asked after the western remedies one by one, in full detail, and wrote them down — how the honey was laid on, how the garlic was pounded, how the barley-beer washing was done, how many times a day, which wound took which treatment. Sister Wei talked and gestured — the way country women speak of their remedies, proudly and a little shyly, as if offering a dish to a guest — and at the end she said, suddenly: ”Girl Shafi, these remedies were passed down by the old women of the western line. I always thought they were just for the villagers. Now they go into your book...”

”Into the book,” Shafi said, ”they become the remedies of all Egypt.”

Sister Wei was still a moment — and then she laughed, and wiped her eyes with the back of her hand. ”Then that is a fine thing. My aunt taught me the honey dressing on the western line, where the desert wind dries a wound before it can heal. When she learns her remedy has traveled this far, she will be beside herself with joy.”

The boiling of the cloth strips went into the book too.

For four months the hospital’s cloth had never been enough, and everything depended on boiling the strips and using them again. The boiling had its own rhythm in the yard: the great pot going from dawn, the strips hanging in the sun in long grey rows, the women’s hands passing through the steam. More than a thousand old strips had been boiled, round after round. Sister Wei was in charge of the work, and she explained it exactly: the strips had to be boiled through, and hung in the sun after boiling; when dry, they were sorted into three piles — the clean pile, the urgent pile, and the pile fit only for padding the feet. Well water, thirty loads a day, all of it went to this work.

”This work,” Sister Wei said, ”is the least noticed and the one that cannot stop. Cloth strips are the hospital’s clothes. When the clothes are in rags, the strongest man catches cold.”

”So in this book,” Shafi said, ”the boiling of cloth gets its own entry: how much water, how long the fire, how many boilings, where to hang it. Wherever a medical house is raised from now on, it can do the same by following it.”

”The boiling, too?” Sister Wei’s eyes went wide. ”What is there to record in that?”

”You think it ordinary,” Shafi said, ”because you have boiled cloth half your life. But a newly raised medical house has no one to teach it how to boil cloth. A book is a teacher. This work of your hands, from now on, is the work of every medical house in the country.”

Sister Wei clicked her tongue and said: ”So this entry of mine counts as being in the book?”

”It counts.” Shafi said. ”It stands in the same row as the honey dressing and the garlic paste.”

Sister Wei smiled. In the smile there was a little pride and a little embarrassment. ”In all my life, this is the first time I feel that the work I do is worth the word ’recorded.’”

Neht’s experience was handed over the same day.

The old man had kept his pharmacy for half his life, and his triage ran on nothing but a pair of eyes. No one had ever taught him the craft; he had learned it the way a man learns the moods of a river — by standing beside it long enough. When a man came in, he said, look first at the face, then at the walk, and last listen to the breath — with those three looked at, the weight of the case was divided, eight or nine parts out of ten. What was saved was not only time; it was medicine.

”These eyes of mine,” Neht said, ”have no numbers in them. If you ask me to write it down, I cannot write it. Ask, and I will answer.”

And so the questioning went on for a whole afternoon. What color of face counts as grave, what walk counts as weak, what breath must keep a man in the hospital, which wound must not be called light because it bleeds much, which wound must not be called heavy because its mouth is small — Neht spoke a sentence, and Shafi wrote it down. By the fourth hour the old man had forgotten that he was being recorded and was simply teaching, the way he had taught apprentices for forty years.

The old man warmed to the telling. He pulled a wounded man resting in the colonnade over as an example: ”Look at him. His face is greenish, but his walk is steady and his breath even — this one has lost a little too much blood; let him rest two days and he is well; he does not need to take up the examination table. Now look at that one —” he nodded toward the far end of the yard, ”— he walks leaning to one side, his eyes half closed, and there is a whistle in his breath. That one is hurt inside, where it does not show from outside. A case like that must not be delayed one step; carry him straight in.”

”And the ones that bleed much?” Shafi asked.

”Nine times out of ten, the ones that bleed much are not the frightening ones.” Neht said. ”A split head, a torn scalp, a face streaming blood — most of the time the inside is untouched. Clean it, bind it, and in three days it scabs. It is the ones that do not bleed and do not cry out — press the belly and it is hard, push on it and the man cannot shout with pain — those are the ones that kill.” He paused. ”In my half life at the pharmacy, what I feared most was not blood. It was stillness. While blood flows out, the man is still alive. When the blood will not flow at all — the man is hanging by a thread.”

”Recorded.” Shafi said. ”’Fear stillness, not blood’ — I will write it word for word into the triage section.”

When the recording was done, the old man took the pages himself and read them for a long time. Then he said: ”With this writing of yours, these eyes of mine have become a craft that anyone can learn.”

”Craft or not,” Imset said, ”the triage gate is the first gate of the whole hospital. When the gate is held true, everyone behind it can breathe easier.”

”The first gate.” Neht clicked his tongue. ”All my life I only knew I was looking at people. Now I know I was holding a gate.”

The account of the medical stall at the surrender point was gathered in too.

That was the new sickness of the time after the war: the diseases of hunger, and the wounds of neglect. Shafi opened a separate column for them — ”wounds of the aftermath.” Half the people in this column were surrendered rebels; half were refugees from the east bank. Their wounds were not the wounds of the siege: those had been fresh wounds, come suddenly and gone quickly. They were built on months of hunger — a whole body of sickness held down and pressed under, seeming light, and then, at one chill or one overwork, all of it breaking out at once. These were the wounds the war left behind it, and they would take longer to heal than the war had taken to fight.

The deserter with the festering arm came back on the fifth day.

Imset remembered him — the first time at the stall, his arm rotten half a month, kneeling on the ground and saying the medicine had to go to the officers first. This time his arm had closed with new scar, and he had spirit in him again. He stood before the stall and, before showing his wound, bowed once, and said: ”Priest, the flesh on my arm was grown by this city. From today I am enrolled in the work-teams. From now on, half of what I do belongs to this city.”

”It belongs to yourself.” Imset said. ”The arm healed for you; the work done well, the food is earned by you.” He unwound the old cloth and looked. ”Three more dressings, and this arm will be whole and sound.”

The deserter nodded — then said, suddenly: ”Over there, there is still a lame old veteran and a boy soldier. They asked me to carry a message — they say they drink thin porridge every day over there, and when they truly cannot hold out, they will come over too. This stall of yours... will it still be open?”

”Open.” Imset said. ”While the porridge pot stays, the stall stays. Tell them — the sooner they come, the less they suffer.”

”I will send word back when I can.” The deserter said. ”Only — the men who can carry word are few over there now.”

The words fell quiet over the stall for a moment. The wind came from the east, rolling a few dry leaves in circles before the table.

Old Sun brought the porridge that day, saw the stall’s accounts, and came over to look. He set his carrying-pole down by the table and wiped his hands on his apron before he touched the pages. He knew few characters, and could only make out the word ”medicine” here and there. He squinted at the pages, turning his head sideways, the way a man looks at writing he respects but cannot follow. He said: ”Your stall here and my porridge pot over there — an arrow-shot apart. But we do the same work.”

”How so?” Shafi asked.

”Porridge is the medicine from outside.” Old Sun said. ”A man felled by hunger — his first mouthful has to be porridge. Medicine is the porridge from inside.” He gestured with his ladle. ”My pot treats hunger; your stall treats sickness. Hunger and sickness are brothers. So I set my pot downwind, and the smell of it drifts to your stall — a man who smells porridge drinks it first, and then sees the doctor. Warm the belly, and the sickness loses three parts of its force before it is even treated.”

”Uncle Sun,” Shafi said, ”I will record those words too. Add one line to the book for war-field medical houses: the porridge pot and the medical stall are to be set side by side.”

”Then that is a fine thing.” Old Sun beamed. ”My ladle of porridge has been cooked into your book as well.”

”Teacher,” Shafi said, ”this column may grow longer than any other. When the war is over, the people’s sicknesses are only beginning.”

”So,” Imset said, ”this book is not called a war account. It is called a medical account. What belongs to the war goes in; what belongs to the years after the war goes in too. As long as people live, they will have sicknesses to see to. As long as a medical house is open, the account must not stop.”

”I remember.” Shafi said. ”Not a day has passed without a line since the first day of the siege. From now on, not a day will pass either.”

The building of the field hospital was dictated by Imset.

He began with the choosing of the site: the Old Market Square, enclosed on three sides, sheltered from the wind; half the city’s breadth from the wall — close enough for carrying the wounded, far enough for safety. The walls, waist-high mud-brick, with beams laid across and ship-sails spread on top — the sails were old ones A-Tu had spared from his own boat — he had refused any payment for them — holding off rain and letting air pass. The three areas were clear: treatment in front, wards behind, the medicines in the middle. Triage by red and yellow markers; two lanes marked out with grass rope — the patients walked one lane, the medicine-boys the other, and the two never crossed. Filth went out the back door; dirty water went into the seep trench.

”This hospital,” Imset said, ”was raised in three days, four months ago. Taken apart now, every beam and every trench has its reason. Draw all these reasons down. Wherever a war breaks out from now on, build by this drawing — three days will be three days.”

”Drawing it is easy.” Shafi said. ”But the changes made through the four months must be recorded too. The first version had no wind-curtain, and men were harmed; the second version added it. The first version did not raise the medicine store, and two packs of medicine spoiled; the second version raised it. These detours are the things that most need writing.”

”Write them.” Imset said. ”A detour is worth more than a straight road. If the man who walked it does not record it, the men behind will walk it again.”

Shafi drew the drawings very finely. One hospital was taken apart into dozens of small pieces: how high the walls, how wide the doors, where the windows opened, how many shelves in the medicine rack, where the stretchers rested, where the water entered, where the filth went out. Beside every piece stood its measure, and the number it had been changed to over the four months.

When she drew the last page, she set down her brush and said: ”Teacher, this drawing is like the granary drawings.”

”It is the granary drawing.” Imset said. ”The way a granary stores grain is the way a hospital stores lives. Everything you learned from the granary, you have now paid back to the hospital.”

Lu Zhu came on the sixth day.

He looked through the finished drawings once, and turned the pages of the gathered account, and said: ”Good. Grain has its net — and now medicine has its net too.” He pointed at the consumption lines. ”Order your supplies along these lines, and the day of the wound medicine running dry will not come again.” He pointed at the building drawing. ”I will have a copy of this made and put in the Chief Construction Bureau. It will stand on the same shelf as the optics and acoustics records of the temple.”

He looked closely. He had built granaries before he had built anything else in this country, and his eyes went to the damp and the air the way a waterman’s eyes go to the current. When he came to the medicine store, he crouched down and tapped his fingertip on the place of the two narrow windows in the drawing: ”Windows in the south wall, high up, set slanting across from each other. That is right — cross-flow, not direct draft. Medicine fears two winds: no wind, and it mildews; a direct wind, and it dries out. Set slanting, the wind passes under the eaves — just right.” He pointed at the rack. ”The bottom shelf half a foot off the floor — that is how the drawing has it. But when it is actually built, it must be raised another half finger’s breadth. Tamped earth looks level, but when the rains come back, the damp swells up through it. That extra half finger is one more measure of dryness.”

”Half a finger.” Shafi noted it with her charcoal stick. ”I will have the foreman change it tomorrow.”

”One more thing.” Lu Zhu said. ”Your drawing, laid beside Mery’s granary drawings, looks like they were born of one mother. The granary’s door opens on the shaded side; your medicine house’s door opens on the shaded side too. The granary’s windows are narrow and high; yours are narrow and high as well.” He straightened up. ”Years ago I taught you the granary’s methods. You have now given them back to the hospital. That round trip is the most valuable account of all our years.”

”This is not my account alone.” Shafi said. ”The raising by half a foot — you taught it. Keeping the dry from the wet — that came from Master Mery’s granary. The boiling and reusing of cloth — that is Sister Wei’s. The triage by the face — that is old Neht’s. I am only the one who keeps the numbers.”

”The one who keeps the numbers,” Lu Zhu said, ”is the most precious of all. Crafts die because no one records them. Once recorded, they belong to all Egypt.”

”On the same shelf?” Shafi was a little surprised.

”On the same shelf.” Lu Zhu said. ”Those records are the light we calculated in the past. These are the beams to be raised in the future. Both are things this country grew out of the war.”

”One more thing,” he said. ”Meru’s boats — from now on the cargo lists carry one more column: medicine. Medicine travels like grain — checked against the list, sealed by mark, handed from station to station. Only when medicine is inside the net can it truly never run dry.”

”Then I will write the procurement book tomorrow.” Shafi said. ”Which kind of medicine, how often it is taken in, how much at a time, where the warning line is drawn. When it is finished, I will send a copy to Merchant Meru.”

”Write it.” Lu Zhu said. ”What you write will be more accurate than what he reckons up in his pipe.”

Mery came too — not to check accounts, but to bring dates.

The official granary’s stored dates, by the old rule, were allotted to the hospital for food-in-place-of-medicine. He set down the basket, saw the tables full of books, and said: ”Your medical account and my grain account are walking side by side now.” He paused. ”Grain must be kept from damp; medicine must be kept from damp. Grain is stored in separated sections; medicine is stored in separated sections. Grain has its warning lines; medicine has its warning lines. Shafi — from now on, this medicine store of yours is the official granary’s sister — a sister granary that stores lives instead of grain.”

”The sister granary.” Shafi said the words over, and smiled. ”Then I must keep my account as finely as you keep yours.”

”More finely than mine.” Mery said. ”When grain goes bad, the worst is empty bellies. When medicine goes bad — lives are lost.”

The procurement book was carried by Shafi herself to the beach.

Meru’s boat had just put in. He squatted on the gunwale and turned the pages of the book. When he came to the warning-line column, he tapped the head of his pipe on the words ”wound medicine” and said: ”Wound medicine below the amount for twenty days — and the boats are to be urged. Is that the meaning?”

”It is.” Shafi said. ”Twenty days is the bottom line. The boats from the West Bank to the beach take three days at best and five at worst; counting the northern line’s borrowed route, the supply can always be made up within ten days. But in war, stores are emptied in a moment. So the warning line is drawn at twenty days, not at ten.”

”Twenty days.” Meru clicked his tongue. ”You are harder than us traders. A trader’s bottom line is keeping three days of goods in hand. You, now — you cry out at twenty.”

”Medicine is not goods.” Shafi said. ”When goods sell out, the worst is lost money. When medicine runs out, the hospital must close its door. For every day the door is closed, more people outside will not live through it.”

Meru said nothing. He knocked his pipe against the gunwale, and said: ”Done. From now on, the guild’s boats carry a medicine column on the lists. You draw the line, I haul the goods; wherever your line is drawn, my boats run to it.” He paused, and grinned. ”Only do not draw your line up in the sky. There is no river up there.”

”There is no river in the sky,” Shafi said, ”but there are clouds. One rainy day, I will ask the clouds to deliver your goods.”

”Then that is a fine thing.” Meru laughed out loud. ”The guild’s boats will be able to sail to heaven.”

The account of the medical point in the Haven was gathered in too, carried up from the north of the city along the same road Shafi walked every morning.

Neht’s books held two months of fine detail from the shelter grounds, kept in his plain hand on whatever paper the pharmacy could spare: how many children coughed, how many old people’s legs ached with cold, how many caught chills, how many had eaten something that disagreed with them. The fullest column was the children’s — more than half from kicking off their blankets in the night, a smaller part from drinking unboiled water. The column ran almost to the bottom of the page, and Neht had drawn a thick line under it, the way he marked the things that would come again with every winter.

”These sicknesses,” Shafi said to Nefer, ”half of them are cured in medicine, and half in the shelters. Press the blankets down, boil the water, and the medical point’s work is cut in half.”

”That is what I say too.” Nefer said. ”But children do not listen to such talk.” She changed the subject. ”That birthing mother — have Sister Wei look in on her more often. These days the child moves often; it may be near its time. A child born in the middle of the war — its first cry has to have someone there to catch it.”

”Sister Wei goes every day.” Shafi said. ”Hot water, clean cloth, reed matting — all of it is waiting at the medical point.”

”Then all is well.” Nefer said. ”This city is in great need of one cry — not a cry of fright. A cry of birth.” She said it looking north, toward the birthing room, and for a moment the two women stood in the doorway of the medical point without speaking.

The letter from the north came on the seventh day, carried down with the river boats; the sheepskin was cold when Shafi took it, as if the north wind had ridden the whole way with it.

Old Camel’s letter was written for him as always, in the even, plain hand of the station scribe; the news inside it was anything but plain. It said the sky of the northern border had already let fall its first snow-grains. The soldiers on the border wall had more frostbite than blade wounds now. The old army doctor was not enough, and the medicine was not enough. Seb asked one question, in the careful hand he had learned from his teacher: did the city’s hospital have a remedy for frostbite?

”Yes.” Shafi said. She turned to the cough-and-frostbite column and copied out this city’s frostbite treatments of the past months, remedies and doses together. The column had been written, line by line, out of the frostbitten hands that had come through the hospital door that winter, and she copied it with a brush that never hurried. ”This book has to be sent north too. The war in the north has not begun.”

”Send it.” Imset said. ”When Merchant Meru’s boat next goes north, carry it aboard. The road is long, so copy it thin — the essential remedies will fit on one page.”

Shafi copied a thin book that same night. The writing was smaller than the main book’s, squeezed together, and into one page she packed half a winter’s remedies.

The copying was done as dawn was breaking. She had numbered the entries, so that a man reading by firelight in a snowstorm could not lose his place. The writing was so small that she had stopped twice to rest her hand; but every character was clear, because a character that was not clear was a dose that could be mistaken. She wrapped the thin book in three layers of oiled cloth, and bound the bundle in a piece of old felt, and carried it to the beach. Meru’s boat for the north was to sail in two days — a long time to carry a frostbite remedy that might be needed tomorrow, but the river ran at its own pace, and the north had learned to wait for it. The boatman took the bundle and weighed it in his hand. ”Light,” he said.

”Light.” Shafi said. ”But when it reaches the north, it will be heavy.” The boatman looked at the small bundle again and nodded; he had carried heavier things that were lighter.

”Heavy or light,” the boatman said, ”set your mind at ease, Miss Shafi. On our northern run, where the goods are, there we are.”

”Where you are, the goods must be too.” Shafi said. ”This book is laid in for the frostbite of the border wall. For every day you arrive early, a few less men lose their hands and feet to the cold.”

The boatman tucked the bundle inside his coat, against his skin — the place where, before, he had carried his own dried rations for the winter crossing. He patted the place once, the way a man pats a pocket he means to keep closed.

”I have written one more line of reply to Seb.” Shafi said. ”Just one: the city’s remedies stand together with the wall.” She had thought about the sentence a long time before writing it — a message to a boy who was raising a wall in the snow, from a city that had learned what walls and remedies cost.

”A good message.” The boatman said. ”The craftsmen will feel the warmth of it.”

The boat left the shore. Shafi stood on the beach and watched it go north until the sail was swallowed in the mist. The sky of the north was heavy grey, like a pot that had not yet had its lid lifted.

News of the rebel camp came in with the deserters — as most news of the enemy had come all along, not by scouts, but by the men who had walked out of the camp and were willing to talk.

A newly surrendered soldier came to the stall to have his wound dressed, and spoke of the medicine over there: the whole camp of more than twenty thousand men had one old army doctor. The medicine had been gone long since. The wounded queued, and what they waited for was a handful of plant ash. ”Plant ash stops the bleeding,” the deserter said, ”and it also starts the rot. Our tent of the wounded — listen to it at night and it sounds like wind scraping through reeds. All groaning.”

”Plant ash is not unusable.” Imset said. ”Wash the wound clean, press the ash on it — it is a remedy for emergencies. But with only ash, and no medicine, no clean cloth, no hot porridge, a man can only grit his teeth and endure.” He paused. ”So write this in the book: medicine is not only medicine. It is cloth. It is water. It is food. It is warmth.” He said it slowly, as if writing the words into the air, one by one.

The deserter listened, and said suddenly: ”If we had come over sooner...”

”Coming over now,” Imset said, ”is not too late either. That wound on your arm — come tomorrow and have it dressed again.”

At the river bend, that tenday was quieter than ever. The river bend had been the quietest place of the war all along — the quiet of a held breath — and in this tenday the breath had begun to run out.

The man in the white robe came to report that the camp’s old army doctor had himself collapsed three days before — he had gone down from hunger, and from exhaustion. More than twenty thousand men, and he the only one who could treat a wound; he had worked day and night until the day he squatted down and could not get up again. ”In the tent of the wounded now,” the white robe said, ”no one is left to see to them. The badly hurt are carried in, and they simply lie there waiting.”

Nekhem heard it all, and was silent for a long time. He was eating his evening meal by one lamp — half a bowl of thin porridge, exactly the same as any soldier in the camp. The bowl was plain clay, chipped at the rim, and he ate slowly, the way a man eats when he is thinking about what is left.

”Take my portion,” he said at last, ”and spare another mouthful for the old doctor. He can treat wounds; he must get back on his feet first.”

”High Priest,” the white robe said, ”you have grown thin too. These last nights I have heard you cough.”

”Coughing does not kill a man.” Nekhem said. ”Hunger does.” He set down the bowl; its bottom was so clean it mirrored the lamp-flame. ”They, in the city, have medicine and doctors and hot porridge. We, on this side — what do we have to keep men? The god.” He paused. ”Now even the god must eat.”

The white robe did not dare to answer. Outside the tent, the wind from the north puffed the tent cloth in and out, like someone in the night, patting this tent, again and again.

The talk between teacher and student came on the seventh evening of the gathering — the evening before the book was to be bound, and both of them knew it.

That night the apprentices had all gone. Imset and Shafi sat in the open yard, keeping watch over the lamp that never went out. The flame was small; when the wind passed, it leaned aside. The night had gone quiet around them, the way nights go quiet in a hospital when, for once, no one is calling out.

”Shafi,” Imset said, suddenly, ”do you remember where you first followed me?”

”The ruined shrine on the West Bank.” Shafi said. ”I knocked over half a basket of dried mint, and sneezed for half an afternoon.” She had been thin then, and quick, and afraid of everything except the work; she remembered the cold floor of the shrine, and the mint scattered over it, and how the sneezing would not stop — and how, when it finally stopped, the man before her had laughed, and the laugh had sounded like a door opening.

”Back then,” Imset said, ”my head was full of my family’s old case. I thought of the day I would return to Thebes and find the men who had destroyed my family, one by one.” He looked into the distance, his voice level — the voice of a man who had carried an old grief so long that it had worn a place in him, and the place had healed over. ”Later — how I set it down, I cannot say myself. Perhaps it was from the day the first medical house stood up. Once a wall is raised, a man wants to live inside it; he no longer wants to live inside his old griefs.”

”Teacher,” Shafi said, ”do you still think of those things now?”

”No.” Imset said. ”The people of the old case — dead, or scattered. And I have long since stopped being the priest I was back then.” He turned to look at her. ”But you. When we first met, you could not even remember the names of the medicines. Now the medical houses of the whole country — how to lay in their medicines, how to store them, how to use them — will follow the book you have written.”

”I...” Shafi lowered her head. ”I only kept the account.”

”Being able to keep the account,” Imset said, ”is the gift itself. There are not many in this world who can heal the sick. There are fewer still who can turn the work of healing into rules anyone can learn.” He paused. ”The people I have saved in my life — I can count them. The people this book of yours will save from now on — they cannot be counted. When a student has come this far, a teacher has no other wish.”

The night wind came gently. The lamp swayed once, and straightened. The two of them sat on in the dark, the finished account between them on the table, and neither felt the need to speak.

”Teacher,” Shafi said, ”the book is not finished yet.”

”Not finished,” Imset said, ”then keep writing. The road is long, and the lamp is still lit.”

The book was finished on the ninth day of the gathering. Nine days of counting, nine days of asking, nine days of writing in the intervals between the wounded — and now it lay on the table, and the yard felt almost empty without the work. Through that day the apprentices kept wandering to the table to look at it, as if it might disappear.

A thick stack, in four parts. The first, wounds and medicine-consumption, held the numbers of the four months. The second, the storing of medicines, held the granary’s methods. The third, the building of the hospital, held the drawing that had been taken apart and drawn again. The fourth, grading and food-in-place-of-medicine, held the three rules of the lean days.

The chief officiant came at noon of the ninth day, carrying a roll of old sheepskin in his arms; its corners had gone brittle. The roll was bound with a cord that had yellowed, and he undid it carefully, the way a man undoes a thing that may break.

”This is the account of the old temple’s pharmacy.” He spread the sheepskin on the table. ”Thirty years old. Look at it — thirty years ago, seven parts in ten of the city’s good medicines went into the temple’s pharmacy. The account records it plainly: what came in. But of who it was used on — not one word.”

Shafi bent over it. The strokes of the old account were even and upright, every one; but turn the whole roll, and not a single page recorded a ”patient.” The medicines named in it were beautiful and strange — unguents, salves, tinctures — and the pages said nothing at all about the hands they were meant to heal. She raised her head. ”Chief Officiant, this old account was kept for those above.”

”And so it is dead.” The chief officiant said. ”Your account is kept for people. While people live, the account lives.” He pointed at the new book. ”This book, one day, will be placed in the old temple’s library, beside the old scriptures that no one turns anymore. So that the people who come after will know — there was once, in this city, someone who reckoned the two words ’saving lives’ into numbers.”

”And the old account?” Shafi asked.

”The old account is left to you.” The chief officiant said. ”Put it beside the new one. Old and new side by side — let the people who come after judge for themselves: which account can save a life, and which account is only fit to gather dust.”

After noon, a few of the wounded came into the hospital to look at the book. The word had gone round the wards that the book held them all, and they came on crutches, and leaning on one another.

The first was the old warden’s younger son — the young man whose leg had been hurt, the one who now carried stretchers beside his elder brother. He leaned on a wooden stick, walked to the table, and said: ”I heard from my elder brother that your book records my leg. I wanted to see how it is written.”

Shafi turned to the page — she found it without searching, for she knew it the way she knew the faces of the hospital. It said: ”Leg wound, arrow cut, neglected one night, salved seven days, bone unhurt, recovered.” She read it aloud to him. He listened, and was still a moment, and then said: ”Just those words? That night I hurt so badly I could have died — and in the end it is one word: ’recovered.’”

”That one word is for you to look at.” Shafi said. ”Behind that one word stand the numbers of four months in this hospital, the cloth Sister Wei boiled, the medicine Merchant Meru carried. Now that your leg is well, that one word has earned its place.”

The young man said nothing. He reached out and touched the page lightly, the way a man touches a leg that has healed. The page bore the print of his finger after he left, and Shafi did not smooth it away.

”My elder brother said,” he said after a while, ”that when this leg of mine is fully sound, I am to carry a stretcher too. You can record me a second time then.”

”The second time,” Shafi said, ”I will record the word ’strong.’”

The young man smiled, and walked out leaning on his stick, haltingly. At the door he turned back and said: ”Sister Shafi — from now on, let this book record more words like ’recovered’ and ’strong.’ Fewer of those pages... that cannot be filled.”

The whole room went quiet. Shafi stood holding her brush for a moment, and said, low: ”I wish that too.”

Imset read the book through from beginning to end, the way he read everything — slowly, and twice, and with his finger following the lines. When he reached the last page, he took up his brush and wrote eight words on the cover: ”The account of wartime; the remedy of the age.” The brush moved eight times, unhurried, and the room held its breath without knowing it.

That evening, everyone in the hospital gathered around. It was the first time the whole hospital had come together for something that was not a crisis.

The apprentices, the medicine-boys, Sister Wei, Neht, and a few of the wounded who had come to have their dressings changed — they ringed the long table — the apprentices standing, the old ones sitting, the wounded leaning on the wall — and Shafi stood behind it and read out the table of contents of the four parts, item by item. The yard listened the way a yard listens when its own history is being read aloud. When she came to ”wounds and medicine-consumption,” she gave the numbers; at ”the storing of medicines,” she told of the half-foot raising; at ”the building of the hospital,” she unrolled the great drawing made over nine days; at ”grading and food-in-place-of-medicine,” she spoke of Sister Wei’s remedies and Old Sun’s porridge pot.

”This book,” she said when the reading was done, ”was put together by all of us, over four months, one life at a time. When I copied the numbers I saw the nights behind them; when I read the remedies I smelled the salve-boiling yard. From now on, when anyone asks — how a war should be fought, how a sickness should be treated, how grain should be stored, how medicine should be managed — they will turn to it.”

”Turn to it.” Neht said. ”I will be the first. At my age, to be able to study again by a book — that is worth it.”

”I will turn to it too.” A-He squeezed in at the back of the crowd and said, low. ”When I know more characters, I will learn every line of it by heart.”

”Learning it by heart is good.” Imset said. ”Skill on paper is dead; skill in a man’s heart is alive. Every one of you in this room is the book’s second life.”

No one in the room said anything. In the lamplight, the book lay on the table, and the eight words on its cover shone in the firelight.

”Teacher,” Shafi said, ”this name is too great for it.”

”It is not too great.” Imset said. ”These four months are numbers reckoned out in human lives. What is reckoned in human lives — however great a name it bears, it can bear it.” He paused, and the firelight moved over his face. ”When I was young, I only wanted to save the people before my eyes. Later I built a medical house, and wanted to save a whole city. Now this book is for countless people yet to come — men who never fought this war can still learn, from it, how to snatch lives back from the jaws of death in a war like this one.”

He closed the book and laid it gently on the table’s end. The night wind came in from the yard’s mouth and turned the pages’ corners, lightly — as if the book, too, were turning itself over, page by page, wondering at what it had become.

”The four months of this war,” Imset said, ”were not fought for nothing.”

Shafi said nothing. She picked up the book again and wrapped it, with great care, in a piece of oilcloth — oilcloth saved from the wrapping of the last medicine shipment, for nothing in the hospital was ever wasted. The oilcloth was new; its four corners were folded even and square. When it was wrapped, she went into the inner room of the hospital, and from the top shelf of the medicine rack took down an old wooden box — the one Imset had brought from the West Bank, which had once held his most precious medicines. She took the medicines out, put the book in, and settled it straight at the bottom of the box.

”Here?” Imset asked.

”Here.” Shafi said. ”This is the medical house’s archive box. From now on, the foundations of every medical house in the country come out of this box.”

She closed the lid and locked it. The lock was old, and its key hung at her waist, ringing lightly against the keys of the medicine cabinet — the sound of two duties that had been joined into one.

”Teacher,” she said, ”tomorrow I will open the new account, to the numbers of the decisive battle. The day of the general assault — it will be fixed soon, by General Horenheb’s word.”

”It will be fixed soon.” Imset said. ”When the next battle is over, the thing in this box will see the light of day.”

Several days passed. Then, one night, Horenheb’s letter arrived — by the night courier, along the same road the war reports had ridden through all the months of the siege.

The letter carried one line: ”The date of the general assault will be fixed within three days. The city’s medical account may be opened anew, to the numbers of the decisive battle.”

Shafi read the letter through — once for the words, and once for what stood behind the words — brightened the lamp, and opened the first page of the new ledger.
