Nile Builder: The Architect from the East

Chapter 59: Chapter 8: Medicine Supply Cut Off — Inventory Running Dry

The news of the supply disruption did not come from the merchants’ mouths. It seeped out from the silence of a boat captain.

That morning, the ferry crossing looked just as it always did — the sky not yet fully lightened, a thin layer of mist still clinging to the river’s surface, hugging the water so closely that it hid the roots of the reeds along the bank. Several boatmen crouched on the stone steps of the landing, chewing their flatbread in the river wind. There was no wind on the water, but a few cormorants flew in from upstream, their wings skimming the surface and breaking the mist into drifting fragments. The stone boat pulled in first, its hull scraping across the riverbank sand with a low, grinding rasp. Then came the timber boat. Then the lime boat. Meru’s building‑materials alliance had long since turned the ferry‑crossing unloading schedule into a semi‑automatic system: every cargo type had its fixed berth, its fixed unloading hour, its fixed transport crew responsible for the short‑haul relay from the landing to the worksites. Meru personally checked the shipping schedule twice a day, morning and evening — he had memorized the arrival and departure times of every vessel from the seventh hour to the eleventh hour, the sequence fixed and unalterable. Even the oarsmen knew which berth was theirs and which boat had right of way — because when a flash flood came up from downstream, jumping ahead in the queue might mean that by the time your own boat reached the landing, there would be no one left with enough strength to unload the cargo. That morning, the medicine boat did not come.

At first, no one thought much of it. Medicine shipments did not arrive every day. The Thebes merchants generally sent three batches a month — on the fifth, the fifteenth, and the twenty-fifth. But these past few days, with all three branch‑hall worksites running at full capacity, Imset had already submitted the branch‑hall pharmacy inventory requisition to the cargo masters, and everyone had silently assumed the boat would arrive on schedule. ”The requisition has been delivered to the landing — the boat will come before the end of the month.” That was the unspoken understanding that held the camp together.

The old boat captain on duty at the landing set his water pipe across his knees and squinted upstream. The river’s surface was empty. He crouched back down, picked up his flatbread, and chewed. The bread was hard — every bite pressed against his molars, the strain traveling all the way to his temples. It was the coarse flour he always ate, dry enough to crumble, each mouthful demanding a deliberate effort. But by the end of the morning hour, and well into the midday hour, the medicine boat still had not appeared. The old captain stood up and laid his water pipe on the stone steps. This time, he was not looking at the river — he was looking at the mountain ridge on the opposite bank. The ridge’s outline had already begun to blur, because the sun was climbing higher and the heat was lifting the dust from the river flats, veiling the distant contours in a pale brown haze. His right knee — the one he had injured years ago when he fell from a mast, the kneecap cracked and never quite healed right — was starting to swell from the long crouch; when he straightened up, he felt the joint pull sideways. He pressed his palm against the knee, squinted hard, and stared for a long time.

”The boat didn’t come.” The words were squeezed out between his teeth — no surprise in them, just the flat acknowledgment of a fact he was delivering to himself. A man who had spent his entire life on the river had a sense for this kind of ”not coming” that arrived earlier than any ordinary man’s — before the absence of the boat’s silhouette had even registered in his eyes, he had already run through every possible consequence in his mind, rehearsed each one like a play staged in the dark. The Thebes medicine merchants would not halt their shipments without cause. This route had been running for nearly seven years — the loading schedule, the water passages, the shallows where extra oarsmen were needed to drag the hull across — all of it was fixed and unalterable. If the boat had stopped, there could be only one reason. Someone had blocked it.

At that moment, He Lu came out of a treatment room on the upwind side of the camp. He was on his way to the drying shed for a freshly dried mint leaf. The drying shed sat close to the riverbank, and from here he could see the stone steps of the landing directly. He Lu glanced in that direction, said nothing, and neatly arranged the mint leaves he had just taken off the drying line into a wooden tray, pressing them down gently with his fingers. The leaves were dried just right — they still carried a faint warmth from the shed, and when he rubbed them between his fingertips, the cool, sharp scent immediately rose into the air. But now, that scent made his brow tighten even deeper: the smell told him, in a language his nose had learned long ago, that the mint remaining in the pharmacy could not withstand another late shipment. Then he walked to the water basin and washed his hands. The first rinse — clear water. The second — abrasive sand. The third — clear water again. He dried his hands on his apron, walked to the north wall, and silently read the thirteenth standard from beginning to end — the one on tiered dispensing during medicine shortages. His lips moved with the charcoal strokes, now slightly worn and blurred from the damp, tracing each word again and again. That thirteenth standard had been written on some midnight long past. Back then, the yarrow stock had dipped below the minimum alert threshold, and Imset had come out of the treatment ward, sat down beneath that same wall, and spent nearly a full watch writing with a freshly sharpened charcoal stick — so long that the black dust had worked its way into the creases of his fingers, staining the skin. He Lu stood before the wall now and read that standard one more time: critical cases first, children next, mild cases reduced. Slow‑consuming herbs substituted for fast‑consuming ones. Chewable preparations over decoctions; topical applications over internal doses. Topical applications consumed the least, but their range was extremely narrow. That was the rule for making the most of what little remained. ”Mild cases reduced” — when he saw those four characters, he paused. Before today, those words had been nothing more than a technical guideline. Now, he would have to stand at the pharmacy door and explain their meaning to the patients queuing up. He knew some would not understand — a child with a low fever but no dehydration: was that mild or critical? It would depend on what he saw when they came in, on the fall of his judgment.

After midday, Meru returned from the ferry crossing. With him came an old boatman he had found on the riverbank — a man who had been working the water for forty years. The old boatman’s palms were covered in thick calluses, and where the calluses had cracked, the tender pink flesh underneath showed through. He stood beside Meru, and before he spoke, he tilted his head slightly southward — south was the direction of Thebes. His eyes moved slowly from the river’s surface to the shore, and then from the shore back to the river’s surface. That was not an ordinary boatman’s habit — it was the way a man who had spent half his life on the water read the currents. The color of the water, the speed of the flow, the sediment that the wind could not shift — he read all of these.

”Three boats from upstream are all tied up at the Thebes landing,” he said. ”The seizure order came from a priest under Nekhem. The seal was pressed so hard that the ink soaked through the papyrus. The order says: any boat shipping ’temple‑controlled medicinals’ to Akhetaten must carry a transit permit issued by the priests themselves. Without the permit, no boat is allowed to load. Any cargo already loaded must be unloaded back into the warehouses.” He bit down on the words ”unloaded back into the warehouses” — ”not at the landing itself. Offloaded at the Thebes warehouses. After that, whether the cargo will be released — that requires further notice.”

Meru broke his flatbread in half and handed one piece to the old boatman. Then he turned and walked toward the workshop. As he passed the stone steps of the landing, the old boatman watched his back in silence — the old boatman knew that Thebes’s seizure order had not appeared out of nowhere. It must have been planned for some time, waiting for a specific moment to strike. That moment had not been chosen when the stockrooms were full — if the medicine was still plentiful, a seizure could be compensated for, and the blow would not hurt. They had waited for the moment when Akhetaten was at its most vulnerable. They had been waiting for the tipping point. This tipping point — the old boatman had been reading the water for decades, and his mind had moved one step further than Meru’s: behind this, there might not be just one priest.

”Thebes — every medicine merchant has been ordered not to ship to Akhetaten — unless Nekhem himself issues a transit permit.” Meru pushed aside the reed‑mat curtain of the workshop, bringing in the river wind that carried the smell of coal smoke and sand. ”This is not any single merchant’s decision — it’s a coordinated directive from the temple of Amun. No High Priest before has ever cut off a new city’s medical supplies so completely.” He picked up a charcoal stick and drew a horizontal line across the blank space of his wooden board — above the line, he had already written the branch‑hall pharmacy allocation schedule; below it, the space was still empty. In that empty space, he began to write: ”Thebes — blockade — effective date.”

Lu Zhu sat before the workbench, the water‑channel plans for the southern district spread out in front of him. He listened to everything Meru said, then placed his charcoal into its groove and fell silent for a long moment. Then he picked up a sharpened wooden wedge, pushed aside a curl of wood shavings that lay across the pipe plan, and revealed an older survey map pressed underneath.

”The route to the south,” he said, his tone no different from saying ”the water‑channel reroute.” ”Nubia — that’s where Kes’s family is from. You mentioned before that you’d seen medicinal plantations on the southern border, didn’t you?” He did not pause. His charcoal continued moving across the southern route map, tracing several feasible paths — and beside each path, he marked the risk level: the water route — fast, but required passing through a shallow stretch occupied by hippopotamus pods; during the rainy season, the water level rose, making it easier to strike submerged rocks. The overland route — safe, but far, a one‑way journey of at least twelve days, more than three times the window between two expected shipments. The wood shavings he had brushed aside had kicked up a thin film of dust; he stopped, swept the shavings away with his palm, and then continued drawing. His carpenter’s eyes, fixed on the route map, did not even flicker. He did not have a southbound caravan ready, nor a camel merchant waiting to depart. The only thing he could do at the table right now was to sift through every possible route, one by one.

Shafi heard about the seizure order at the pharmacy door. She had just finished registering a small batch of Nubian licorice root that had come in that morning — she had just written the licorice’s registration number and shelf position on the ledger, her fingers still holding the charcoal. Meru’s youngest son came running to tell her, breathless, one of his reed sandals having fallen off somewhere along the way, his bare foot striking the gravel road. The gravel road was scattered with fresh stone chips, sharp-edged and pointed; they cut into his sole, but he did not stop. He gasped to Shafi: ”Thebes won’t send us medicine anymore. Father says all the boats have been seized. Everyone at the landing is saying the temple of Amun has already put its seal on the order.”

Shafi did not cry out. She did not ask ”why.” She listened, and then turned over the ledger sheet she had been working on, and on the blank back she drew a timeline. Current stock — the remaining quantity of each herb, how many days it had been in use, how many days it was projected to last. Requisitions — what the branch halls needed, what had already been sent. Restrictions — which herbs could be substituted with fresh reed‑root, the problem of insufficient volatile oils in wild mint, and the loss of lung‑moistening and hemostatic effects when using sweet beet root as a substitute for dried dates. Her charcoal tip moved across the back of the sheet without stopping — she did not look up at anyone, and she did not double‑check numbers she had already calculated. Every time she filled a column, she pressed a tiny notch into the corner of that column with her thumbnail.

The scratching of the charcoal against the leather was fast, but steady. She stopped once — not because the numbers had tripped her up, but because a gust of dry sand from the riverbank stung her face, breaking her concentration for a single breath. The sand hit her skin with a fine, dense impact, like someone pressing a handful of crushed dried mint against her cheek. She blinked, brushed a grain of sand from her lashes, and did not let herself be distracted again. Sweat began to bead on her forehead. Pharmacy stock — here: how many days could it last? Branch‑hall initial allocations — here: which herbs were still needed? The dried‑mint substitution He Lu had mentioned — using fresh mint in place of dried, but the wild mint she had scouted near the drying yard these past few days was not yet mature enough, its volatile oils still insufficient, not ready to harvest. Fresh reed‑root in place of dried — that was feasible, the riverbank was full of it, but it had to be sliced thin and spread out flat, never stacked; every evening she had to re‑check each batch for mold. Sweet beet root in place of dried dates — it would lose some lung‑moistening effect, but right now sweet beet root was all she could get her hands on.

She did not complain. She did not curse the temple priests. She calculated every substitution, every possible extension of the stock, every delivery time from the riverbank to the southern branch — all of it. For every calculation she completed, she drew a tiny checkmark on the leather. The marks were so small they were barely larger than a fly’s head.

The young priest‑apprentice crouched in the dim shadow of the reed‑mat shelter, his back against the main hall’s north wall. Spread across his knees were the old papyrus pages Imset had left him — pages now so worn from handling that their edges were nearly translucent. He had been reading them all day, his eyes bloodshot, his fingertip moving down the lines one by one, counting every alternative formula he could find — and every time he identified a substitute, he drew a horizontal line on his own papyrus comparison chart. Sweet beet root in place of dried dates — not acceptable, because sweet beet root lacked the hemostatic effect that dried dates provided for uterine bleeding. Wild mint in place of dried mint — only a partial substitute; its fever‑reducing power was insufficient, requiring an extra dose of dried yarrow to compensate. Dried arborvitae leaf in place of dried reed‑root — possible, but its lung‑moistening effect was only sixty percent; to make up the difference, the decoction would have to be simmered twice as long to extract the less soluble compounds. ”Fresh reed‑root — can be gathered daily, but cannot be stored. It is vulnerable to damp and mold — if stored for more than a day, it spoils.” He looked at his comparison chart, then checked it against the existing stock inventory, item by item, calculating how long the remaining herbs could sustain each branch at the current rate of consumption.

Shafi took the comparison chart from his hands and did not say a word. First, she sat on the pharmacy’s stone step and checked every herb’s name against the stock. Then she stood up, leaned against the pharmacy’s mud‑brick wall, and recalculated every entry marked ”days it can sustain,” pressing a small mark with her thumbnail beside each maximum‑day figure. After she had marked them all, she ran the timeline backward from each mark, checking the numbers in reverse.

The usable stock would last less than eight or nine days. After that, only wild herbs and chewable fresh plants would remain — and chewable preparations delivered their active compounds much more slowly than decoctions, making them useless for high fevers and severe dysentery. At that point, the branch‑hall pharmacies would have to rely on people rather than herbs.

It was on that same afternoon that Meru decided to go south to Punt himself to procure medicine. He walked from the landing onto the gravel path along the riverbank, where some quarry workers were squatting by the lime pits, tossing waste stone into the water. Meru walked unhurriedly, his head down, muttering to himself: ”Punt. Nubia. I’ve done business with the medicine merchants in both places.” He was not wearing a robe — only a wide leather belt cinched around his waist, with a water‑skin and a pouch of dried dates hanging from it. He did not need to explain to anyone where he was going, nor did he need anyone’s permission. Lu Zhu did not give him a formal commission — he simply handed him a leather sheet with a southern route marked on it, and at the point where the river curved south of the southern branch hall, he had drawn a cross‑mark: during the dry season, this point could be crossed on foot. Bypassing the hippopotamus shoals that lay on the original route, this crossing would save at least seven or eight days. He did not say ”the journey is dangerous.” He did not say ”take care of yourself.” What he said was: ”When you reach the first fork, do not take the eastern channel. That stretch has little silt, the water looks shallow, but the boatmen will lie to you — there are submerged rocks, the bottom is slippery, and the hull will be scraped through. Take the western detour — it adds a day, but it’s safe.”

Meru had run these trade routes more than thirty times in his life. He had heard this kind of warning from so many mouths that he no longer showed any expression. He tightened the date pouch a little more, folded the leather sheet, and tucked it into his sash.

Then he set out.

Three days later, Meru made his first contact at a spice‑trading depot south of the Nubian border. It had taken him nearly three days to get there from Akhetaten. Halfway through the overland route, his water source had run dry — the marker he had left by a shallow pool in the river valley had been washed away by a sudden rain, and the sand had been redeposited over the spot where he had dug his cache. He relied on his sense of smell to locate damp ground, then scraped through the sand until he reached water. He had performed this same routine dozens of times on the trade road. He crouched, and after the hole he had dug began to fill with water, he scooped up the first handful and tasted it — faintly sweet, faintly salty — drinkable. Then he found a riverbank stone to cover the newly dug water hole as a marker. There were no boats here, no carts — every step was pushing deeper into unfamiliar southern territory. The farther south he went, the more humid the air became — the river was visible, the surface glinting in the sun, but the underwater currents churned up the silt, and the water had to be settled for half a day before drinking. He had been to Nubia before, but every time he entered this dry tropical zone, he could feel the skin on his calves growing tighter and drier from the lack of moisture. The trading depot belonged to an acquaintance he had made on his last run — the son of an old woman, a man who had once traded dried ginger and dried garlic in this region. He said he could get Nubian‑grown dried plantain and dried licorice root — not from Thebes channels, but from surplus tail‑end stock smuggled out of the Punt plantations further south. The Punt plantations lay further south, where the rains were abundant and the soil was mixed with volcanic ash, producing licorice roots with deeper taproots — but because the goods had to be smuggled around a long detour, the price was much higher than the Thebes market. Meru did not haggle over the price. He did not have the time. He only asked: ”Where is the stock?”

”Piled at the Punt docks,” the man said. ”Tail‑end surplus from the last merchant caravan. Not much left.”

”Find me a fast boat. I need the shortest route — water route, sailing day and night.” He smoothed a patch of sand flat with his palm and laid out a few small stones on the surface — the Nubian landing, the Punt docks, the Akhetaten landing — three points in a line. Exactly three stones. He pushed the stone between Punt and Akhetaten a little closer to the Nubian landing. That meant that on the return journey, he would have to stop at the Nubian landing to pick up a familiar barge captain — a man who had run the shallow rapids several times before and knew how to avoid the whirlpools, knew how to angle the rudder against the current to protect the hull.

That afternoon, he pushed further south from the depot toward Punt, traveling another full day. He did not even have a camel — he rode a donkey borrowed from the Nubian trading post, one of its ears half‑bitten off by some animal, but its endurance was remarkable. The donkey’s hooves landed on the cracked red clay with a dull thud each time, like a finger tapping against a wall crevice. The donkey’s back was narrow and uncomfortable to sit on. He did not complain. He just kept recalculating the time in his mind — no matter how the eight or nine days the main hall had predicted might stretch, this round trip would take at least eight days even in the best case. By the time he returned to Akhetaten, the storehouse would likely be nearly empty. He tightened his leather belt and let the wind blow through.

Another two days of travel — and he reached the Punt plantations. The owner was a Nubian merchant with blackened teeth and a faded blue headwrap tied around his head. He pointed to several piles of dried licorice root and dried mint in the corner of the plantation and said: ”These tail‑end goods have been sitting here for nearly three months — they were meant for that old Memphite priest. He died last month, no one paid the balance. You can have them. I’ll give you a good price. But you’ll need to re‑dry them and check them thoroughly — some of the sacks have been sitting too long, the bottom layers are starting to compact. The outer color still looks fine, but there may be spots and discoloration inside. If you want to inspect them, open the seams carefully — don’t just yank the stitching, you’ll break the core threads and the whole sack will come apart.”

After Meru had taken receipt of the goods, he did something that made the seller frown — he spread out a brush and a thin wooden plank, and on the back of the plank he drew an itemized list: the varieties, quantities, batch origins, unit prices, delivery deadlines, and penalty clauses for breach. This was no longer a verbal negotiation — this was a formal contract. The seller stared at it for a long time, surely wondering what kind of trader this was — no priestly seal, no copper‑stamp endorsement, yet he put every single detail down on paper like a government official.

”You’re not buying for the temple,” the seller finally said.

”No. I’m buying for the Akhetaten healing hall.” Meru said this with the charcoal stick clamped between his teeth. The tail end of the charcoal had been bitten so many times that it was worn smooth as glass. He was biting the tail, not the tip — because a good charcoal stick, in his view, was like a good cutting tool; the seller could haggle over the price, but he would never break his own writing tip.

At dusk, Meru loaded the first batch of dried licorice root and dried mint onto a boat at the Punt docks. Before the cargo went on board, the weather was dry — the boat captain ran his hand over the surface of the sacks and said they were fine; the herbs had been exposed to the sea breeze for several days while sitting at the dock, and most of the surface moisture had already evaporated. After boarding, Meru did not sit in the cabin — he stood on the deck, holding a goatskin map he had borrowed from the Punt plantation owner. The map showed only the coastal water route from Punt to the Nubian landing. Every now and then, the warmth of his fingers would melt the grease in the goatskin, releasing a faint musty smell, but he never noticed it — in moments like these, his nose was only used for one thing: reading the wind. The southeast wind was against them — headwind, slow going. The northwest wind came from behind — faster. He wiped the salt spray from his eyes with the hem of his robe — the salt crystals, fine and sharp, stung like tiny needles. He bit the hem of his robe to keep it out of the way, held the goatskin map taut against the wind, and crossed out two shallow stretches near the reef zone with red marks.

”Cut through here — stay outside the sandbar, keep close to its outer edge — don’t make landfall,” he said to the boat captain.

On the return journey, with the wind and the spray and the sandbars, he barely slept at all. The skin around his eyes was red and swollen from the wind and the salt and the lack of sleep, stinging with every gust; he pressed a wet cloth soaked in well water against them — for a few seconds at a time — before the boat captain called him back to the bow to guide the next turn. He had left the donkey at the trading post; now all his attention was on the water. The sacks of dried licorice root in the hold were Akhetaten’s last chance.

That same afternoon, on the southern branch hall’s worksite, Kamon discovered something. A few quarry workers, when backfilling the crushed‑stone bedding layer, had mixed in several blocks of freshly quarried coarse sandstone from the upstream pit — the rock was veined with iron, its surface weathered to reveal rust‑colored streaks underneath. An old stonemason squatted by the stone pile, lifted one of the rust‑streaked blocks, gauged its weight in his hands, and held it up to the light to examine the fracture surface — the crystal grains were fine, but the stone was too heavy for the bedding layer, and if too much of it was used, it would crack the waterproof barrier underneath. He said to Kamon: ”We can’t use all of this batch — the iron‑streaked sandstone is more than twice as heavy as the larger river cobbles. If we put too much of it in the bedding layer, it will crush the waterproof wall underneath — unless we split it into thin slabs and lay them on top of the waterproof barrier as a pressure‑distribution layer. That way, when the rainy season raises the water table, the foundation wall won’t crack.” Kamon listened, then selected several blocks from the pile and split them into thin slabs, tapping them lightly with his stone hammer. The hammer’s edge left a shallow mark on the sandstone — so delicate that only a man who had spent his whole life splitting stone could keep the mark no larger than a grain of rice. He laid the split slabs on top of the waterproof barrier at the bottom of the pile pits, then covered them with a thin layer of small cobbles. When he finished, he stood up and ran the back of his fingers along the contact line between the slabs and the cobbles, checking that no slab was lifting at the edge, no line of the interface deviating from the charcoal mark he had made the day before. Then he unhooked his water‑skin from his belt and poured half of it over the seam — the water seeped down through the cobble gaps, not fast, but evenly, pooling nowhere. He crouched and watched until the water had fully drained, then said: ”It passes water. That means it won’t swell.”

Another scene unfolded near the northern branch hall’s worksite, in an old storage shed behind the weaving workshop. The shed had been unused for a long time — half its roof had collapsed, and the floor was littered with dried grass and timber scraps left over from earlier construction. A woman from the main hall’s kitchen — the one whose cooking pot had cracked — passed by during her noon break and noticed a pile of old vine stalks in the corner of the shed, soaked by rain and then sun‑dried, tangled around a discarded wooden beam. They had been washed down from upstream and caught against the beam. She crouched down, scraped the dried mud from the outer bark, and found the bast fibers underneath still tough and resilient — the vine’s nodes had been softened by the water, but the white inner fibers were still tight. She pulled several vines loose, pressed them against a rusty shovel blade in the corner to strip the outer bark, and laid the bark strips on a dry stone by the wall to cure. Her hands turned purple from the vine sap as she stripped them, but she paid it no mind — she just wiped them on a coarse cloth and kept going. She said to the carpenter’s apprentice passing by: ”Don’t let these old vines rot here — strip the outer bark off that row against the wall. These bast fibers, after soaking for two days, can be twisted into rope. The new water jar in the kitchen needs a lift rope for its lid — this will work, and it won’t get brittle by the fire.” The carpenter’s apprentice crouched down and mentally rehearsed the whole sequence — collecting, stripping, curing — each step broken down the same way he broke down a timber job on his workbench. At the end of the day, he delivered a bundle of stripped and cured vine bast to the kitchen. The woman took it, bent it back and forth between her fingers — good flex, not brittle, strong enough to hold the lid of the water jar.

That same night, someone knocked at the pharmacy door.

Not the kind of knocking that came from the treatment ward — this was different. In the night, after the wooden bolt had already been slid home, someone outside pushed gently against the door. He did not call out. He did not knock. He just stood outside, pressing the tip of one finger against the bolt. The bolt did not move. He did not push again. He crouched at the base of the pharmacy wall, his back against the north wall, his knees drawn up to his chest. Shafi found him during her midnight rounds.

He was a boatman from across the river, carrying a small child in his arms. The child did not cry, did not move, his lips dry as ash. The boatman said his son had been suffering from diarrhoea for four days. His wife had died first — conscripted the year before to work on a temple, worked to death in the construction shed, no one to retrieve her body; the other women in her shed had buried her on the riverbank. Now this child was all the family he had left. He did not know how to comfort the child. He could only hold him, palm pressed again and again to the child’s forehead, searching for that faint, flickering, intermittent thread of warmth. He had laid his carrying pole across the embankment, let the child lean against his shin, and sat on the riverbank with the pole laid across his knees like a guardrail, facing the water, letting the night wind blow for a long time. During the day he ferried cargo; at night he kept the child beside him and fed him rice gruel. Today, after he tied up his boat, he did not go home. He carried the child straight to Akhetaten. He had not come for treatment — he knew the medicine was running low. He had come to beg for a single packet of medicine. ”I know there isn’t much left. But I have nothing else.” His voice was soft, but when he spoke, his chin trembled involuntarily — an old habit, worn into him by fear long ago. He was not afraid of dying of illness himself. He was afraid of losing the last person in the world who was still tied to him.

Shafi did not reach for the inventory ledger — because the numbers were already burned into her memory. The amount she could dispense was not large, but she still stepped to the side of the pharmacy door, reached up to the shelf, and took down a fresh ball of reed‑root still wrapped in wet mud. She crouched outside the pharmacy door, using a broken brick shard to scrape the black mud from the root nodes, then peeled off the softened outer bark of the old root. The grooves she scratched in the mud were straight and even. With the leftover mud, she drew a damp line at the base of the wall — she did not know why she drew it. Then she pinched the root into small segments and handed them to the boatman. ”Soak them in cooled boiled water twice — discard the first wash, it’s just mud; soak again in the second wash until the water turns a cloudy white. For a three‑year‑old, two small spoonfuls each time. Don’t force it — let him swallow one mouthful at a time. The pharmacy will have a new shipment in twelve days — bring the child back then.”

After the man left, Shafi returned to the pharmacy and sat down. She retied the spare bolt core that had been loosened by the wind — threading the cord through the groove in the center of the core, then tying a double figure‑eight knot so the tension would distribute evenly and not snap. The core had been stored in the shade for two days, and fine cracks had begun to show on the surface of the wood; she pulled the knot tighter. Her fingers rested on the knot for a long time. Not just because the fine cracks needed to be secured, but because of the image of that child lying against his father’s chest, his hand hanging limp — that small dark hand curled against the rough wool of the boatman’s coat, unable even to cry. What that sick child reflected was not merely a disease, but a cry from the very bottom — a cry from those who had been forgotten by the ordinary machinery of social provision, treated as surplus to be discarded. She heard the man’s breathing, the soft rustle of the leather sheets on the outer wall stirred by the night wind, the thin, dry scent of herbs fermenting in the pharmacy now that they were no longer at full freshness — all these sensations surged into her fingertips at the same instant, embedding themselves in the grain of the knot. The registration numbers on the pharmacy ledger were only numbers, but in this moment, what she held in her heart was an entire land being drained by indifference and poverty.

During the days of the makeshift medical stall, the critical cases, the dying, the old people who kept night vigils, the mothers carrying children — they had never been registered in the government’s ledgers as ”persons in need of protection.” But tonight, this boatman’s presence made her think beyond the moment: all those people in the hovels across the river, who could not afford the medicine, who could not even reach the landing, who relied on luck for whatever charity came their way — one day, there would be more of them. Imset had taught her — the healing hall’s door is open to every person. But today she realized once again that behind those words ”open to every person,” someone had to stand behind that door preparing stock that would never be enough, leather sheets that would never be enough, water and flatbread that would never be enough. All of these things — the women sifting sand during the day, the women returning at night with used bandages to reweave — every pass of the shuttle, every knot, every bowl of hot broth pressed from the kitchen’s meager stores — all of it was already being forged. And just as Imset, in the makeshift medical stall, had never charged a patient a fee and never asked where they came from — a practice she had followed since her apprenticeship — so now she had to turn that practice into written rules, and enforce them on every registration leather sheet and every length of rope.

In the deep of the night, she sat on the wooden crate and reopened the inventory records. She recalculated the daily allocation limits for every substitutable herb, changing them from a two‑tier system to a three‑tier one: critical cases, children, pregnant and postpartum women. The third tier — ordinary adults with mild conditions — would be temporarily switched entirely to fresh‑herb substitutes. She also went through Nefer’s used‑bandage recycling logs from start to finish, assigning a quota to each kitchen that had received surplus grain, then tied a tiny knot with a thin strand of dried grass at the edge of the leather sheet as a marker. After tying the knot, she set the sheet down, knelt beside the crate, and brushed the riverbank sand grain by grain from between her fingers — fine sand mixed with tiny bits of broken mussel shell, rough and irritating against her skin. She picked them out one by one and dropped them into the waste basket. Then she washed her hands and rechecked every single donation entry she had recorded that day.

In the dead silence, she remembered the boatman’s words, the steam rising from the pots where Nefer and her workers had been boiling used bandages in the small shed behind the pharmacy these past few days, the patched bowls Nefer had distributed to the kitchen women — she set her hands on her knees and, with her charcoal, added a line at the end of the original branch‑hall medicine transfer schedule: ”This system has operated without interruption since its inception. If medicine arrives at the landing within twelve days, full supply will resume immediately — no interruption, no additional consumption.”

After she placed the ledger sheet back on the shelf, she stood outside the pharmacy door for a while. The moon had risen to the height of the old ventilation slot in the corner of the pharmacy wall; the moonlight streamed in through the slot and silvered the thin layer of dried mint dust that had accumulated on the sill beneath it.

A while later, footsteps approached — light, but crisper than ordinary steps. The young priest‑apprentice came carrying half a bucket of cooled boiled water, which he set on the stone platform beside the door. He said to Shafi that this water came from the first branch of the supply channel connected to the upstream landing — he had walked the full length of the channel twice, checking every section from the diversion point to the main hall; there were no leaks. The water now flowing into the pharmacy could be used first for soaking fresh medicinal roots — he had made this trip specifically to tell her that the water path was clean. Shafi looked at him and did not answer about the water. ”There is still some dried date crumb left in the kitchen. Go eat it. If you don’t eat, you won’t last the night.” She said this with her fingers still resting on the edge of the ledger — the numbers, though already at rock bottom, had not stopped her from finding at least one hot meal for every person who was keeping her afloat.

On the seventh night, Meru’s boat pulled in at the Akhetaten landing.

He had returned from Punt nearly a full day earlier than predicted. He had taken the detour through the coastal shallows — when he had the boat captain cut through the shallow water outside the sandbar, the hull scraped against a submerged rock, leaving a long white scar along the bottom planking. The captain said it would leak. Meru said not to patch it — keep going. The river water beat against the hull for half the night, the wind blowing sand into the cabin; Meru and the captain bailed water left and right until they reached the landing. When Meru jumped onto the stone steps of the landing, his knees nearly gave way — he had been standing too long on the boat, and his knee joints had locked into a straight, unbending position. He carried two large sacks of dried licorice root and dried mint off the boat — the sacks still carrying the mingled smell of sea salt and river water. Shafi and He Lu stood waiting at the top of the stone steps. She was already holding the registration ledger, but at this moment she did not write. She crouched down and pressed her palm against the outside of the licorice root sacks — the salt crust on the surface was the trace of sea wind, indicating that the cargo had been exposed to ocean spray before Meru had covered them with cloth at some point.

Then she looked up and saw the scrape mark on the boat’s hull, and then she looked at the cracked skin on Meru’s hands — the result of gripping ropes too tightly at sea. She did not ask how the boat had made it back.

Meru did not mention the fatigue. He just pulled her to the pharmacy door, opened the mouth of the sack, and showed her. Most of the licorice root had some mold spots on the outer layer, but once she dug down to the inner core, it was dry — about three tenths of the batch could still be used. ”I need to go back to Nubia for a second batch — there’s another shipment waiting on that side. This time I’ll take the overland route, bypassing Thebes. No boat this time — carts.” After he said this, he lowered his head, pinched out the last date from his pouch, and chewed it slowly. He swallowed after several chews.

After the medicine was stored, Shafi recalculated the inventory.

She wrote the new stock‑on‑hand figures on the north wall — the minimum reserve for each branch hall, the daily consumption rate, the disaster‑minimum stock, the estimated sustainability before the next shipment arrived — every figure recalculated. After she finished writing, she paused and drew a horizontal line at the bottom of the estimate table, and beneath the line, in small characters: ”Around the twelfth day, medicine supply can be fully restored.”

But she also knew that this shipment had only temporarily bridged the gap. The source of the medicine still lay in Thebes. If Thebes did not lift the blockade, the disruption would come again. After completing her calculations, she wrote another line at the very bottom of the ledger sheet: ”Dependence on a single source is unsustainable.”

This line — written at the bottom of the estimate sheet — had been meant for Imset’s eyes. But Imset had not been at the main hall these past few days — for several days, he had been making rounds at the branch‑hall worksites, mornings at the southern branch, afternoons at the northern, evenings at the riverbank. His old knee injury had flared up from all the walking — every now and then his left knee would buckle outward as he walked. Every time he crouched to feel a patient’s carotid pulse, that slight misalignment was visible in the bend of his knee — the pain was there, sharp and constant. But every time he crouched, his movements were just as precise as ever — forehead, behind the ears, the side of the neck, the pulse at the wrist. He never shortened his examination, even when the person he was examining was only a passer‑by ”asking for a drink of water.” Once, he had said to Sebek on the reed mat of the southern work shed: ”After the medicine supply is cut off, you’ll find that what patients need is not a particular herb. What they need is a place they can come back to, a place they can trust. They need to know that someone is here, and that someone is not going to leave.”

He said something even heavier: ”What we are guarding against is not a shortage of medicine. It’s a collapse of trust. Once people feel that even this place has nothing left for them, they will stop coming. And if they stop coming — it’s not just their loss. The entire healing hall’s credibility will collapse with them. That credibility is something we have built — day by day — with standardized prescriptions, with the daily habit of recording case histories, with medicine prices posted on the beam, with the pharmacy of every branch hall cross‑backed against the main hall’s pharmacy. We cannot let our patients return to that old state — waiting for charity, waiting for alms. Waiting once, waiting three years, dying, forgotten — that cannot happen again.”

Sebek carried that principle to its most complete expression during the afternoon follow‑up consultations. On his consultation table, less than two palm‑widths of dried mint remained. Each time, he cut only a single small stem and cracked it gently with a wooden mallet. He no longer shook the herbs by instinct — instead, he laid each mint leaf on the wooden tray and, with his fingernail, picked off the damp spots at the base of each stem one by one. Under normal circumstances, those damp spots would not have been a major problem — a quick shake and they could be chopped — but now every leaf was worth a human life, and he dared not waste any. As he reached the fourth leaf, a mother with a three‑year‑old child stood at the door of the consultation room, afraid to come in. The child in her arms had been weakened by diarrhoea to the point where he no longer had the strength to cry. Sebek still remembered the last prescription he had written for her: mint and licorice root. The licorice root — a newly arrived batch from Punt, slightly damp, with faint black veins on the bark — was still usable once the outer skin had been scraped off. When he wrote this prescription, he reduced the licorice root by one third and substituted fresh reed‑root for the missing portion — dried mint, fresh reed‑root, and a small amount of sweet beet root. Every dose fell within the ”maximum allowable substitution zone” marked on the wooden board beside the pharmacy door — never exceeding the limit, never duplicating. His hand on the mallet was steadier than before — because the force of every blow determined whether this piece of medicine would be used for a specific person, not for a statistical abstraction.

”Next time, don’t wait until the illness has progressed to moderate dehydration,” he said, placing the medicine packet in the mother’s palm. With his other hand, he gently closed his fingers around the child’s small, dehydrated fist — now a gray‑blue from the loss of fluids — and gave it a light squeeze. The fist was so small that it fit only across two of his fingers — cold as a stone just lifted from cold water. He could feel the child’s knuckles protruding too sharply, the skin too loose, the only remaining warmth concentrated along the vein on the back of the wrist. He had felt this sensation many times before — it was always the same with severely dehydrated children. This feeling did not make him sadder. It made him, for the first time in days, bite down on He Lu’s words from the treatment standards one more time: ”Mild cases reduced” — these four characters were not rules written on the wall. They were the few mint leaves that the person standing outside the door was waiting for you to calculate for them. When he withdrew his hand, he did not look up immediately — because he was recalculating in his mind the number of mint leaves he still had, and every number on the allocation sheet he would send to the southern branch tomorrow. He turned every stock limit over in his mind once more, then raised his head and let his voice settle back to its baseline of calm. That baseline — steady, unhurried — had the same quality as the words he had written in the sand on his first day in the hall: slower now, but thicker, harder.

In the hours that followed, he did not reduce the dosage in any patient’s prescription. He simply used every single herb to its finest, most precise unit — reducing the allowable tolerance to within a quarter‑unit of the standard specified on the leather sheet.

Under the reed‑mat shelter of the waiting area, an old stonemason who had come from the southern worksite sat at the very end of the row. He was not ill — he had come to visit his young nephew, who was suffering from chronic diarrhoea. The old stonemason bore an old injury on his hand — the flesh at the base of his thumb had been crushed by a stone hammer years ago, and after it healed, his grip strength had been permanently halved. Before coming to Akhetaten, he had worked on construction sites in the old city of Thebes, where he had seen with his own eyes the conscription orders stripping men away — his fellow workers in the shed had been taken to build temples, some gone without word for three years, others later found at the bottom of quarry pits, still gripping their drills when they died. The last time he had gone to a Thebes medical hall to ask for a packet of fever‑reducing yarrow for his wife, the priest‑apprentice standing at the door of the waiting room had told him — first, offer copper. He had no copper. He stood there for a long time. Later, a servant who was clearing rubbish took pity on him and pressed two broken fragments of dried mint into his palm. ”Don’t let anyone see you. Go,” the servant said. He clutched those two fragments of dried mint all the way home, only to find that his wife’s fever had already broken on its own. Not cured by those two fragments of mint — her fever had simply run its course and broken on its own.

Today, he sat under the waiting shelter of the Akhetaten healing hall, with no copper in his hand. No one had asked him to offer anything at the door. He only watched Sebek, while cracking mint leaves with a wooden mallet, say to a mother waiting nearby with her child: ”Two small spoonfuls each time” — his voice perfectly calm, as steady as the rhythm of the mallet. The old stonemason reached into his pouch and pulled out a small cloth bag — inside was a handful of dried licorice‑moss he had chipped off the rocky slopes after work each day at the southern worksite. ”You can eat this. You can boil it in water — it soothes sore throats. I’ve tried it.” He placed the bag on the wooden bench in the corner of the waiting shelter, did not wait for anyone to record his name, and walked away.

That evening, the bag of dried licorice‑moss was delivered to the pharmacy door. When Shafi saw the bag, she was in the middle of sorting fresh reed‑root into small bundles and tying labels on them with dried grass stems — her hands paused. She opened the bag with her thumbnail, took out a few fragments of the moss, and held them in her palm against the setting sun — it was licorice‑moss, yes, but some fine sand had been mixed in during harvesting. She did not dismiss it as ”worthless wild grass,” nor did she relax her standards just because it was free. She assigned it a number according to her own pharmacy protocol — S‑031, temporary supply reserve — with a note: ”Wild‑harvested from southern rock slopes. Must be rinsed twice before dispensing. Efficacy pending verification; may be included in substitute registry upon confirmation.” Then she placed the bag gently on the edge of the old stone platform reserved for ”unidentified donations” — the platform where, the day before, an old woman from the southern district had left a bundle of dried plantain, also unnumbered, which had since been distributed to the riverbank branch’s paediatric emergency ward and used up. She covered the platform with a piece of worn reed mat, pressed a small stone on top, and wrote two characters on the reed mat with charcoal: ”Pending inspection.”

That evening, Sebek returned to the main hall and saw the stone platform and the two characters on the reed mat. He crouched down and retied the mouth of the cloth bag — the knot had been poorly tied, a loose square knot that would soon slip. He retied it with the waist‑cross knot he had used for decades to secure emergency medicine packets in Thebes — a knot that would not slip loose on its own. After retying it, he placed the bag back on the platform and did not look at what else was there — he knew that in this place, where every person brought the last and most precious thing they had, the only thing his hands should do was to hold steady, without trembling.

As the night deepened further, Shafi walked once more to the pharmacy door. She looked at the damp line she had drawn at the base of the wall — most of it had already dried in the night wind, leaving only the last short stretch still holding its moisture. She crouched and touched the remaining dampness with her fingertip, then stood and looked toward the ferry crossing. The distant river showed nothing — only the faint rustle of wind passing over the water’s surface. But she knew Meru was already on the road again — maybe already past Nubia, maybe somewhere in the reef zone, skirting the outer edge of a sandbar. She went back inside the pharmacy, read through the ledger sheet filled with substitution protocols and alert thresholds one more time, and added one final line at the very bottom: ”Tomorrow — send someone to the landing to ask: has any boat from the south come in?”
