Nile Builder: The Architect from the East

Chapter 52: Chapter 1: Outbreak The Makeshift Medical Stall Overwhelmed

Sickness never announces its arrival with a formal knock. It comes the way desert wind slips through a reed-mat screen — unseen, unremarked, until the coughing starts and does not stop.

The illness did not appear at any single moment that could be marked or named. It seeped in.

Akhetaten was a city that had not yet learned to stand on its own feet — a sprawling labor camp of ten thousand souls, migrants hauled in from every corner of the kingdom, still sleeping under makeshift shelters and eating from communal pots. Stone from the quarries arrived in barges that groaned against the river’s current; brick from the kilns rolled in on donkey-drawn sledges, the runners leaving parallel grooves in the hard-crusted sand. The new capital’s construction encampment stirred before dawn every day, fresh work crews streaming from the high ground down to the riverbank flats, man after man after man, and where there were men, there was filth — and where there was filth, sickness needed only time.

Lu Zhu stood at the edge of the adobe-brick drying yard, looking down at the sprawl of shelters below the encampment’s northern rise. Reed-mat lean-tos, palm-timber poles, roofs of coarse linen that had started out the color of undyed flax and were already darkening with grime. People had begun moving in. They dotted the hard-crusted ground in sparse clusters, their presence against the desert’s edge looking painfully thin — the way a handful of scattered pebbles looks thin when cast across an empty courtyard. There was no proper shelter, not yet. The latrines were temporary, dug in haste; drinking water was hauled from the riverbank shallows, the water level barely above the sand in the reed-lined pits. A crowd of ten thousand, crammed into a place that had not been built to hold them. How could sickness not come?

He had been standing at the brick yard since first light. The kiln-fired bricks from yesterday’s batch had developed a hard, glassy crust on the surface — the kiln had run too hot. He crouched, one knee pressing into the sand, and picked up a brick. He turned it over in his hands, over and over, checking: the firing temperature had been uneven, the clay mix maybe off. Half the kiln’s fuel had fallen short of quota, the bricks were stacking up in the drying yard slower than the walls were rising, and when a kiln ran too hot, the brick’s surface glazed — a vitreous shell that looked hard but was brittle underneath. When a bricklayer struck it with a trowel, it would crack. He stood there studying that single brick for a long time, then set it back down on the pile. He tapped its edge with the toe of his sandal.

The laborers in the drying yard had been coughing all morning. One of them coughed and could not stop — his breath caught, as if something had seized his throat and would not let go. Lu Zhu, standing where he was, did not turn around. He heard that cough and kept it stored in his hearing. The brick in front of him — he picked it up, then put it back on the reject pile.

Fever and chills. Limestone dust coating every laborer’s shoulders. The brick kilns pouring smoke. The riverbank sand-sifting women with sweat-soaked headcloths clinging to their foreheads. The camp medic, Old He, hadn’t paid it much mind — desert nights ran cold, and catching a chill from bad water was common. A cup of hot water, a night’s rest, and it passed. It always passed.

Until the one time it did not.

On the third day, the sickness broke out in three work zones simultaneously. The symptoms were identical — chills, fever, diarrhea, could not stand. The first to go down was a stonemason from the quarry gang, a man in his forties, thick-shouldered, his head striking the ground with a sound that was wrong — too heavy, too loose. In the morning he had said he felt a little off. His work-mates helped him to the shade and brought him a bowl of hot water. By the time the water had cooled enough to drink, he was already unconscious, his body entirely limp, the water dribbling from the corner of his mouth, soaking into the sand beneath his cheek. Before nightfall, three others from the same work-shelter had followed him down, one after the next, the symptoms identical in every detail.

Old He was the camp’s only physician — a man who had spent more than a decade practicing in the river-trade towns along the Nile. He was the first to come to the stonemason’s side, and the first to feel genuine fear. He took one look at the man’s face — the color of his cheeks, the cracked lips, the corner of the mouth still carrying a thread of vomit that had not been wiped clean — and his knees went weak beneath him. He lifted the man’s leg. The limb dropped. The skin was pale. The pulse was thready. Dehydration. Severe.

”Get him out.” Old He’s voice came out flat, but the words — those five syllables — carried something that did not need to be spoken aloud. The men standing around the shelter fell back. The boys who had been peering in from the doorway suddenly scattered, like sparrows startled by a stone, a rustle of feet retreating in every direction. They did not run far. They ran home — to their own water bowls, to their own families, to tell them not to drink from this well or that ditch. Old He crouched there on the hard ground, one hand bracing himself against the reed mat, the other gripping his own knee, staring at the laborers still drifting near the doorway. He did not call them back. He already knew. He had seen enough to know: these symptoms did not spread by touch. They spread through water. He looked down at the man on the ground once more, then turned and walked back toward his medicine chest.

By that afternoon, the brick kiln yard, the riverbank sand-sifting station, and the quarry’s intake point had all reported new cases. At the kiln yard, a brick-loader — a young man who had told his wife that morning he felt ”a little off” — collapsed mid-load and did not get up. His work-mates carried him to the shade and tried to pour water down his throat; the water had barely touched his lips when the convulsions began. At the riverbank, the sand-sifting women went down together — two of them, mother and daughter, both shaking uncontrollably, their bodies wracked with the same dry heaving. They had shared a single water ladle. At the quarry intake, the water-hauling boy — no more than twelve — made it to the door with his buckets and then the buckets simply slid from his hands, the water spreading in a dark stain across the ground, the boy crumpling onto his side, his eyes rolling back, his mouth working as if chewing on a word he could not get out. ”Something’s... wrong,” he managed, and then he was gone.

This was not a sickness that moved from person to person by proximity. These people did not know each other, had never shared a meal or a shelter. For them all to break out at once, the source was either common to everyone or located at a place every single one of them had visited.

The news reached the encampment’s command shelter at dusk. Lu Zhu had just come in from the brick yard; his sleeves were stiff with the dried slurry of lime and river water that had coated them that morning. He stood in the doorway and knocked the hardened lime dust from his cuffs. It fell into the sand at his feet — white-gray motes drifting down through the amber dusk light, settling into the dark grit like something that belonged to neither world. Kamon stopped at the doorway and did not wait to step inside before he started talking — his voice was low, hurried: eleven men down at the quarry, eight at the riverbank sand station, and now the water-hauling boy too. Old He was saying it was an epidemic.

Lu Zhu did not answer right away. He walked to his tool case, pulled out a charcoal stick, and drew directly on the rammed-earth floor. He marked four outbreak clusters — the quarry laborers’ shelter, the riverbank sand-sifting station, the kiln-yard drying ground, the quarry intake workers’ shelter. Then he drew circles around the laundry-washing area and the cooking-pot station. After the first round of marks, he ran the charcoal through each cluster a second time, and beside each cluster location he added a small circle — marking out the water-drawing point for that cluster. Every single cluster drew from the same water source. The riverbank — that shallow, sand-floored seepage pit, the one the camp had been using from the start. Its water was only a few dozen cubits from the nearest latrine, and the day before, the sand barrier between them had given way. The seepage had flowed straight into the drinking-water pit. Right there — Lu Zhu’s charcoal pressed down, darkening the circle into a solid black dot.

A shallow well. A single shared water source. One mouth.

When sickness entered that mouth, it did not need to knock.

He stood up and went out. His thumb found the worn groove on the hammer handle and rubbed along its length — that handle had been with him for more than twenty years, the wood worn into shallow hollows by the press of his fingers, and every time he needed to think, his thumb found its way into those grooves on its own. He walked to the door and turned back. ”Kamon,” he said. Lu Zhu had not said the man’s name — but Kamon understood. He ran through the headcount one more time. Not a single cluster sat adjacent to another. They had all broken out at the same time but in different places, which meant the contamination was in the water, not in the air, and the water source was the drinking supply — no one had told these people to boil their water. They did not know. They had come from villages where well water ran clear and safe; they did not know that in a construction camp, a pit dug in sand a hundred paces from a latrine was a death sentence waiting to be handed out. Lu Zhu came back in, and the man who met his eyes at the door was Kamon, who was holding a stone from the quarry’s reject pile. That stone was meant for the kiln. The riverbank laundry-washing women — the ones who washed the workers’ kilts — had been going down all day, the same symptoms every time.

”The quarry intake well is no good either,” Kamon said. The stone in his hand turned once in his grip. ”The laundry-washing women — the sickness got them too. Every single one.”

Kamon’s thumb on the hammer handle paused. The laundry-washing women too. At the quarry intake there was an old water-lifting device — a bucket-and-pulley — that the laborers had been drinking straight from. Kamon turned toward the quarry intake, but the workers there were already hauling the water buckets toward the stonemasons’ shelter, and Kamon himself stood outside a long moment and did not move. In the gray dust of dusk, he could feel each breath scraping the back of his throat.

Imset and Shafi reached Akhetaten that same afternoon.

They had come on a cargo boat hauling quicklime from Thebes, and they disembarked at the ferry crossing. Shafi carried the medicine chest strapped to her back, walking ahead; Imset followed behind. The chest was heavier than it looked — their old teacher had commissioned it from cedar planks, the cedar’s own grain still visible under the leather straps, the iron corner-brackets already rust-streaked from years of river damp. Every time Imset set foot in an unfamiliar place, his fingers moved on their own, skimming the ground, reading the terrain the way a blind man reads stairs in the dark, mapping each surface before committing his weight to it.

He stood on the ferry-crossing stone quay and looked out across Akhetaten’s sprawl — a wilderness drowned in the color of dusk. The unfinished construction walls stood like broken teeth against the fading light, stained a pale ash-gray. The wind blew across them, carrying the river’s damp, and from somewhere in the distance rose the smell of the encampment — unwashed bodies, old cooking-fire ash, the sour sweetness of grain fermenting in open-air stores. Back at the Theban docks, waiting for the boat to cast off, people had told him Akhetaten was ”already built.” He had seen now with his own eyes. ”Already built” meant a construction site that had not stopped digging. Walls with no roofs. Roads with no paving. Water channels with no lining. People living in rooms that had no doors, in buildings that had no floors. A city that could not yet call itself a city — a place that could barely call itself a camp.

”Let’s go,” Imset said. His hand rested on the worn cloth bundle of dried medicinal herbs, his fingers finding the familiar ridges of bundled stems — licorice root, the thickness of his finger; dried pomegranate rind, still carrying the faintest trace of its fruit’s scent beneath the age. The bundle was tied with the same strip of old linen he had used when they left Thebes. His cheekbones stood out sharper than they had when they departed — the journey had been long, the rations thin — but his back was still straight. He stood at the ferry crossing and did not move immediately. He smelled the air first — caught the smoke rising from the encampment’s distant cooking fires, and beneath it, something else. The smell of sun-dried reed from the riverbank. He had spent decades in Thebes. He knew that smell. It was the smell that clung to every place where too many people lived in structures that were not yet finished.

The plan had been to settle their belongings first, then go see Kamon in the morning. But at the ferry crossing, they heard about the quarry intake. And then the camp — the encampment was filling with sick. The medical stall was already overwhelmed. Old He, the camp medic, had not slept in two nights, one man running between a hundred patients, and the two helpers he’d managed to gather — one an apprentice barely old enough to grind herbs, the other a bone-setter — were stretched past breaking, running between the patients who could still be saved and leaving the serious cases to lie where they had fallen. No one was managing the ones already in fever. No one had time.

”Old He says he’s out of medicine.” The voice came from ahead — one of the laborers, a young man, his throat catching on the words. He said it once, then rubbed his nose with the back of his hand, hesitated, as if deciding whether the next sentence was worth speaking aloud. Then he glanced at the medicine chest on Shafi’s back, then at Imset’s leg — the way it favored one side — and did not say what he had been about to say. Instead he turned toward the shelter where Old He was working, and his voice, when it came, was rougher than before. ”The old doctor came down from Thebes. He’ll see you. Can you take a look — just a look.”

Imset listened. He did not reach for the man’s arm. He stood on the ferry-crossing stone quay and tightened the strap of the medicine chest across his shoulder one notch. He turned his head toward Shafi. ”Don’t unload the medicine chest. Go straight there.” His voice was calm — so calm that the ferrymen standing a few paces away, coiling their mooring ropes, lifted their heads and looked at him. One of them, an old river hand with a face like cracked leather, held the gaze half a beat too long, then dropped his head back to his work. His fingers, winding the rope, made half a circle in the air — a gesture that in Theban dockside slang meant this is trouble. The old river hand was past fifty. He had not needed to explain trouble to anyone in a very long time.

The makeshift medical stall was set up on a patch of open ground beside the adobe-brick drying yard. Calling it a ”stall” was generous. A few palm-timber poles, some old reed mats thrown over them, a roof that let the wind straight through — when the wind gusted, the walls bowed inward, pressing against the four reed screens that served as partitions. The reed screens had been salvaged from the construction site at the high-ground palace district — they still carried dried splatters of the mortar that had been smeared across them in their previous life. The palm-timber poles stood crooked in the sand, leaning at angles that had no business standing upright; one of them had already cracked. A corner of the roof had collapsed inward. Earlier that day, when stretcher-bearers had carried in a woman burning with fever, her foot had caught the cracked pole. She had not had the strength to pull it back, and the roof corner had torn free, trailing a few strips of shredded reed mat behind it. In the dusk light, those pale strips fluttered against the darkening sky, the color of water that had been scared out of its depth.

Inside, Old He was running on empty. The shelter was crammed with more than forty patients. The mild cases lay on the ground, waiting. The severe cases lay on reed mats against the walls. There was no isolation — the feverish and the vomiting jostled shoulder to shoulder, the coughing and the belly-holding and the ones too weak to cry out. The heat of their bodies pressed against the reed screens and could not escape. The air inside was thick with the smell of sickness — a smell that clung to the roof mat, the walls, the folds of your clothing and the skin beneath. An old man with white hair sat just inside the entrance, knees drawn up, head buried between his knees. He had not spoken a single word from beginning to end. He had brought his grandson in that morning, and the boy had died by afternoon. The old man was still waiting — waiting for someone to tell him what to do. There was no one left to tell him.

Beside him, a young mother was burning with fever too. Fever and chills, one chasing the other. Her child lay on the reed mat before her, head scorching hot, lips cracked like drought-parched earth. She kept running her hand across the child’s forehead — each small stroke punctuated by a pause, as if her hand, pulled back, could somehow pull the fever back with it. She had been doing this so long her arm had gone numb. She knew — she must have known — that the child was already beyond her reach. But she kept stroking, one stroke and then a pause, one stroke and then a pause, because if she let herself believe that her hand on his forehead was still doing something, then he was not yet beyond saving.

And in that moment, a woman — a patient who had been lying on the ground among the others — suddenly toppled sideways and did not get up. She had not cried out. She had not made a sound as she fell — she simply slid along the reed mat toward the ground, the way sand slides down a dune’s face with no wind to push it. The man beside her jolted back, and then he screamed. ”She’s dead! She — she’s dead!” He scrambled backward, shoving through the crowd. And then the entire shelter exploded — the screaming, the shoving, the shouting over shouting, all of it happening at once. A timber rack was knocked over; the last few bundles of dried medicinal herbs scattered across the floor. People trampled them underfoot, the dried leaves crunching to powder, their dark-green paste smeared into the dirt, releasing a sharp, bitter smell that cut through the stench of sickness. Bodies surged toward the door.

Imset did not chase after the ones who ran. He walked straight into the shelter, bent down beside the woman who had collapsed — his fingers found the pulse at her throat and then moved lower, probing the abdomen, pressing, pausing at each position and pressing again. He had already found it. When his thumb stopped — right there — he knew. She had been dead before she fell. Diarrhea too long, fluid loss too severe, and in a shelter with nothing but dried herbs and river water, there was no saline to push back through the veins. He stood, and before he straightened his back fully, the reed-mat lean-to on the eastern side of the stall gave way — the pole that had been cracked finally splintering through. Imset caught the crossbeam before it fell, wedged it between the palm-timber poles, and secured it with a twist of reed rope. Then he lifted that fallen mat back up and tied it into place. The apprentice who had been dispensing herbs was so frightened he had bolted — but halfway across the yard, he turned around, came back, and knelt on the ground, picking up the scattered dried-lizard cuttings one fragment at a time, cradling them in the fold of his own tunic. They were already dirty — smeared with mud, mixed with sand — but they were still medicine. He kept picking. There was a grown man standing nearby, frozen in the doorway, not knowing where to put his hands.

But Imset did not stop. The people who still lay on the ground — the ones who had been too sick to run — he did not say anything to them, nothing like Don’t be afraid. He simply pulled down the reed mat that had just been knocked from its frame, set it back between the timber poles, and hesitated for the span of a single breath — just long enough to make sure the pole would hold.

And then he spoke. His voice was not loud — loud enough, no more. Every syllable landed the way a prayer lands that has been prayed for decades: no drifting notes, no swallowed endings. He was not angry. He was not pleading. He was simply telling them: there is a road, there is water, there is clean, and the way to walk it — the only way — is to not let the dirt cross from one side to the other.

The shelter went still. In one instant. Everyone — the ones who had been running, the ones who could no longer run — stopped. A young mother holding her belly looked up at him and opened her mouth to speak; nothing came. Maybe she wanted to say something. Maybe she was just too tired to say it. Then someone — a laborer, hands still trembling from the panic — let out a long breath, and the sound of it in the stillness was louder than any cry. He did not know what to do in a moment like this. He had just watched a woman die, and now here was a man who spoke as if death could be told to wait its turn. That was a kind of certainty he had never encountered — the certainty of someone who had walked so many roads that the one underfoot was just another road, not a miracle, not a curse.

Imset stood in the doorway a moment longer. He did not step inside first. He took in the layout.

First act: he had the stretcher-bearers move the fallen timber rack to the center of the shelter and lashed four reed mats to it, dividing one chaotic space into four chambers. The section nearest the door became the triage intake. The center-left became the observation ward for moderate cases. The center-right became intensive care. The far back corner became the medicine storage and preparation zone. Between every two sections, he left a passage no wider than one person’s shoulders, so narrow that no two people could walk it at the same time — you entered from the front and exited from the back, and no one ever crossed paths.

Second act: he had the mild cases — the ones who could still walk, the ones merely feverish — moved outside to reed mats laid on the sun-warmed ground near the riverbank grass, separate from the severe cases lying inside. The recovering and the declining were now cut apart from each other. Cross-infection had a wall it could not see but could not pass.

He did not explain any of this to anyone while he was doing it.

Old He watched from beside the medicine rack. He watched for a long while, and then he slapped his own thigh — a sound of revelation so sudden it made the apprentice jump. ”Why didn’t I think of that!” His voice cracked with the force of the realization. ”The grain-storehouse layout — you divide the stores by variety and grade, grain from grain, so spoilage in one silo doesn’t spread to the next. A medical stall — you can divide a medical stall the same way!” He began moving the medicine rack on his own, one hand bracing and the other lifting, and nearly toppled a timber support onto the man beside him. Old He scrambled to catch it, one hand holding the timber and the other waving in explanation: ”You know this, right? The grain storehouse — people enter from that side, exit from this side. No crossing in the middle. A medical stall is the same — the sick enter from this side, exit from that side. They can’t meet. The ones who can’t walk, you carry them the long way around. That way you don’t carry the sickness from one end to the other.” His voice, by the end, was almost laughing — the wheezing laugh of a man who had been too exhausted to do anything but keep moving, and who had just seen someone else hand him the answer that had been sitting under his nose.

Only Imset did not laugh. He was crouched beside the reed-mat bed of a boy burning with fever — a boy whose leg kept kicking free of the reed mat, convulsing in short, violent jerks, his teeth sunk so deep into his lip that the skin had already broken. The fever, running on its own momentum, had started a cramp that would not release. Imset looked at the boy for a moment, then pressed the leg down with a little more force.

Shafi stood with the medicine chest still strapped to her back. The straps had not been loosened. She had never seen so many sick people crammed into a single place — the smell was so thick it felt like a wall you had to push through to enter. And then her eyes adjusted to the dimness, and she saw: in the corner of the shelter, a woman was holding a child and weeping. It was the same woman she had seen at the ferry crossing — the one wearing the same Theban-style coarse-linen dress. The woman was holding a small bundle wrapped in undyed cloth. The child inside it — all of her, her whole life — was burning. The bundle lay on the ground. The child’s head was hot to the touch, lips cracked, breathing shallow and fast. The woman’s hand kept stroking the child’s forehead — one hand, always the same hand. Shafi noticed that the other hand was already numb. When she lifted it, her fingers trembled — not from fear, but from the way muscles give out when they have been holding the same position too long. In the half-dark of the shelter, Shafi saw the woman’s fingers curl and uncurl, testing whether they still worked. They did not. Her master needed that medicine chest. But this — this was something she could not leave unseen.

”Don’t worry.” Imset did not turn around. He was speaking to Shafi, but his words were for the room. ”Check how much dried pomegranate rind we still have — the anti-diarrheal. Whatever variety we have, bring it all out. Then go find Old He and ask what he’s running short of.”

Shafi unslung the leather strap and opened the medicine chest. Her lips moved — she was silently counting. Every time her eyes landed on a dried herb packet, her lips pressed together once. Dried pomegranate rind, one bundle. Dried myrtle leaf, even less. Licorice root — she laid it out piece by piece, counted it, and her lips moved again. She went through them all. Powdered willow bark — two pouches. Powdered chamomile — barely enough to fill a palm. She ran through them all, then stopped. Her fingers paused over the empty slots — the spaces where the missing herbs should have been. Then she stilled her breathing, and from the start, she counted them all again, running her fingers from the first row to the last, making sure she had not missed a single packet.

Then she went to find Old He. Old He was leaning against the medicine rack, his finger still resting on a dried-herb bundle. He had just finished his own inventory — counting through what little he had left — and when Shafi opened the medicine chest and pointed to the positions of the dried pomegranate rind and the dried chamomile, she said, ”Sir, this is all you’ll have.”

Old He looked at the chest for a long moment. He did not ask, who is your master. He had already figured it out. He turned to his own medicine rack and pulled out two bundles — one dried chamomile, one dried pomegranate rind. ”These too. That’s everything I have.” His finger pressed against the dried pomegranate bundle as if pressing a seal. ”When it’s gone, I’ve got nothing left.”

Shafi took the herbs, cradled them in her arms, and paused where she stood. ”Uncle He — when it’s gone, what then?”

Old He did not answer. He opened the bottom compartment of the medicine rack and scooped out the last loose fragments — a palmful of dried-mint leaf, crushed and dusty. He brushed his fingers clean. A fine river of sand, fine as milled grain, sifted through the gap between his fingers — the desert dust that seeped into everything, no matter how many layers you wrapped around it. ”It’s fine,” he said. ”They’ll find more upriver. They always do.” He had been saying this to himself for a very long time.

Shafi carried the herbs back to the medicine storage zone and laid them on the timber rack. Old He had said it was fine — that there would be more. But Shafi’s hand paused on the bundle, and she did not say what she was thinking. The camp had more than ten thousand people. These herbs — these few dried bundles — would not last three days.

Imset nodded once and crouched beside the boy whose leg was cramping. His fingers found the pressure points above and below the cramp — pressed, held, pressed — with a rhythm so fine and even that the boy’s convulsing leg gradually stilled, like a panicked animal slowly ceasing to struggle. He stayed crouched there a long while, and when he stood, his knee gave a crack — he had been crouching too long. He straightened himself with the heel of his hand pressed against his own thigh, and looked at Lu Zhu, who was standing in the doorway watching. Lu Zhu had been there since the moment Imset divided the shelter into four chambers with reed mats, and he had not stepped inside. He was still holding the brick-mold from the kiln yard, the charcoal-black on his fingers long since dried to powder.

”This makeshift stall was never meant to handle this many patients,” Imset said. ”We need a proper medical hall. A building — a real building. Medicine storage, treatment wards, isolation, quarantine, clean-water supply — the entire flow line from clean to contaminated, intake to discharge. Without a building, you can’t call it a medical hall. You can’t stop the spread. You can’t even start.”

Lu Zhu did not answer — he was still looking at the triage passages Imset had lashed together from reed mats. That flow-line layout — the one-way passage design — was something he had been turning over in his mind for a very long time: patients enter from one side, flow through, exit from the other, never crossing paths. His right index finger, crooked from years of pressing against stone, traced a line down the surface of the brick mold in his hands. Clean intake to contaminated discharge, and then one more line — from the quarantine zone back to the pharmacy. Only once the lines were all drawn did he raise his head and look at Imset.

”The drawings are already on the drafting board,” Lu Zhu said, pointing toward those reed-mat screens.

Imset nodded and looked down at his own hands. Every nail-bed was packed with dried-medicine powder — the residue of a ritual he had practiced his entire working life: after every time he handled herbs, he would wipe his fingers clean with a dry cloth, once, twice, three times. But today he had not had time. The powder was embedded in the creases of his nail-beds and the lines of his knuckles, impossible to wash out with water alone, impossible to remove without scrubbing until the skin beneath was raw.

”Treat the sick first. The drawings come tonight.”

That night, the old man with white hair did not make it through. Imset sat beside him for the length of one oil-lamp burn. The old man had no fever, no vomiting — his body had simply stopped wanting to take in water. The dried herbs steeped into a decoction could not pass his throat. In his last moments, all the healers stopped what they were doing, and the shelter — that shelter that had been roaring with chaos all day — fell into a silence so complete it was terrifying. The apprentice who had been hauling stretchers opened his mouth as if to ask something, but the only word that came out was one that had already been asked too many times. ”Water...” he whispered, and then, louder: ”He needs water...” And then he stopped. Because in the darkness at the edge of the shelter, where the lamp’s glow could not reach, no one was answering him. Old He wiped a hand down his face in the corner, said nothing, and turned back toward his remaining half-bundle of dried chamomile — he was already calculating how far it would stretch. He laid the half-bundle on the dispensing bench by the door. It was a physician’s instinct, as automatic as breathing: the medicine you have left goes nearest the door, so you can grab it first when the next stretcher comes through.

By the time the last lamp in the encampment burned out, more than ninety people had fallen sick. But the news that Imset had walked into that shelter — that he had not flinched — had already spread faster than any sickness. No one in the camp had panicked. Not one person had scattered. The panic that had been poised to erupt in a single instant — it did not erupt. Instead it was the families who came, crowding before the medical stall. The ones who got medicine — whose hands closed around the tiny packets of dried herbs — clutched them tight and would not let go; the ones who did not get medicine pressed right up to the doorway and would not leave. The quarry stonemasons stood at the entrance, the women gathered around the kilns, one man pushing a flatbed cart he had borrowed from the brick yard, its boards still dusted with stone powder. He had used it to wheel his wife in. The wife was already too far gone to speak, her eyes opening once, looking at Imset, closing again. He had run the cart all the way from the drying yard, and when he grabbed Old He by the arm, his fingers tore a strip of linen from Old He’s sleeve — his grip was so tight he did not notice he had done it.

”Old He — do you still have medicine? My wife — can’t — please, just a little. I’ll pay. I’ll pay double!” The man’s voice was shaking. This was a stonemason — the kind of man whose shout could carry across a quarry floor and make a hundred laborers lift their heads. But here, now, his voice came out so thin it was barely holding together.

Old He looked down at his own medicine rack. There was nothing left on it. But before he could speak, the woman on the flatbed cart convulsed again. The cart lurched on the sand; her husband lunged forward and caught her shoulders, his hands gripping her, calling her name over and over. The woman, in the brief gap between convulsions, managed to open her eyes. She looked at her husband once. Then she looked away.

And the old plasterer — Imset — was already at her feet, the medicine chest he had carried all the way from Thebes open before him. He was running a fever himself, kneeling on the ground, his kneecaps grinding against the gritstone, and he did not feel it. All he knew was: this woman’s pulse was slipping, and she could no longer stand on her own. And this woman, between fever-shivers, reaching a wet hand out from under the blanket to touch his head, suddenly said, soft, ”I don’t think I can make it.” She was holding her stomach, her knees bleeding into the sand, and she did not move, kept saying: ”I’m sorry... I still have children at home...”

Imset did not look up. The stonemason beside him was saying — his voice breaking — ”I can’t work anymore... she’s already in fever...” But Imset did not answer him. The stonemason broke off, and the sobbing caught in his throat, the sound held back until it left a red welt on his own jaw. Then he said, ”Three children... the youngest is still nursing. What am I supposed to do?”

Shafi poked her head out from the medicine storage zone and took one look. She saw the man’s hands, still caked with mud. She went back to the medicine chest and opened the lid again. The dried chamomile — the last of it — was still there. She picked up a blank leather tally-sheet and, in the empty space where a new record began, wrote one character.

Chamomile, out of stock. Pomegranate-rind, only dregs. Licorice root — Old He had shared his private reserve, completely exhausted. The gap in supply. No one to fill it.

She wrote that single character, tucked the leather sheet into the medicine chest’s inner layer, and stood up. She did not tell Imset. Before she could open her mouth, a man from the brick-kiln yard came running, carrying his son — a boy, his head burning, lips cracked and bleeding. The boy’s father set him down at Imset’s feet and did not weep. The weeping, in this family, was done by the grandmother, but the grandmother was already gone — she had died before the boy fell sick. All the man said was ”Save him.” No please, no price. Just those two words.

Imset bent down and felt the child’s forehead. Then he stood and walked to the medicine storage zone. Shafi knew what he was reaching for. The chamomile. The last of it. She did not stop him. She only stood at the doorway of the storage zone and watched his hand go to the bundle on the rack — one brief pause, a breath’s worth of hesitation — and then his fingers came out. One dose for the boy. One dose, returned to the shelf.

Then he walked back. He put the dried herbs into the boy’s father’s hand. The father looked at the medicine, and before he could say anything, Imset spoke first: ”Boil it slow and low. Give him small sips. Make it last.”

When the father took the medicine, his fingers trembling so hard the packet almost slipped, he looked at Imset, then at the boy cradled in his own arms, and his lips moved. Nothing came out. He turned and walked. His boy, still trembling, held the medicine to his own chest, and then the man’s fingers — one at a time, slowly — closed around his son’s hand. He was holding the medicine, and he was holding the boy’s hand, and in the darkness he held both as if they were the same thing.

And at that same hour, the first rumor planted by Nekhem’s priesthood reached the encampment.

No one knew who had started it. It surfaced at the ferry-crossing laundry-washing area. ”Akhetaten is cursed. The sickness broke out at the stall of that former High Priest — the place where he set up his stall is the place where the sickness is worst. Don’t you think that’s strange?” No one gestured toward Imset while saying it. The rumor did not need a pointing finger. It needed only a pail of water, a sigh, a head shaken over a pile of wet laundry, and a stone washing-slab that would carry the words — scrawled crooked and deep — into the next morning’s light. Someone had been there in the night, and when dawn came, every woman beating laundry at the riverbank saw: the words had been scratched across the white stone slab with a shard of limestone and water. The letters wobbled, too jagged for a writing hand — a child’s hand, maybe, or someone pretending to be a child. They read: The river water is foul. The outsider must be expelled. The city must be purified. No signature. No witness.

Old He recognized the handwriting — that same handwriting that had filled twenty years of medical records in Thebes. Imset’s hand. Pressed down hard. On that single white stone slab, not one crooked stroke was out of place. Every line landed exactly where it was meant to land: on a fact anyone could see with their own eyes.

People gathered around the laundry-washing area and read it for a long time. A woman crouched down, her wet hand reaching out to touch the characters. Her fingertip came away smeared with wet lime. She wiped it on the hem of her sun-bleached skirt and looked at the slab one more time. Someone beside her asked, ”Is it real?” She did not answer. She only carried the stone slab with those characters carved into it to a higher spot — a place where the river water could not reach it.

Imset paid no attention to the rumors. He did not chase them. He stood at the door of the medical stall, and there — under the oil lamp that had burned through half the night — Lu Zhu spread the wooden board with the medical-hall plan. It was all there: the intake hall, the rear pharmacy, the quarantine zone, the isolation ward, the observation wing. Every flow arrow pointed from clean to contaminated. The sick and the recovering, the corpses and the living — every line on the board ran in only one direction. Lu Zhu’s finger traced from the pharmacy notation up to the south-facing courtyard — a courtyard whose dimensions he had already sketched out on papyrus and would carry, the next day, to the full-scale layout on the ground. The wooden board, the lamp flame. Imset, in the silence: his hand moved across the board’s surface, confirming every single line — measuring the distance from intake to pharmacy, from pharmacy to the rear isolation yard — as if every line he traced was a stone that would soon be laid.

Outside, Kamon had just set the last stone of the day onto the stone-pile. The moon, newly risen, had trodden a shallow track across the sand of the construction road. The hammer in his hand — that hammer — was waiting for the sun to come up.
