Nile Builder: The Architect from the East

Chapter 57: Chapter 6: Wandering Physicians Return — Treatment Standards Established

The first wandering physician arrived on the day the healing hall’s roof was finally sealed.

He stood on the gravel road east of the brick‑drying yard, carrying a leather medicine case that had been worn soft at every corner, the camel‑hide straps frayed into a fuzzy fringe from years of rubbing against his shoulder. He was somewhere in his early forties, his cheekbones high and prominent, his eye sockets deeply shadowed, his lips cracked and bleeding in thin lines like the mud of the riverbank after a long drought. The hem of his robe was caked with layers of sand, and his sleeves were torn in several places, the holes revealing mismatched patches stitched with coarse thread by an unsteady hand. He stood for a long while at the edge of Akhetaten’s scattered ruins, staring at the half‑finished walls that still lacked their roof beams, at Shafi crouched by the pharmacy’s north ventilation slot with a measuring cord pinched between her fingers, at the newly plastered north wall of the main hall where several leather sheets bearing treatment standards had already been nailed. He did not step forward. He simply shifted the medicine case from his left shoulder to his right, then let it hang motionless, as if the weight itself had rooted him to the spot.

No one noticed him at first. The site was too alive with noise and motion. Kamon’s stone yard was rushing the third batch of foundation blocks, the chisels ringing against the rock in a sharp, rhythmic clatter; the kiln yard was spewing out new bricks faster than the masons could lay them, the heat shimmering above the stacks; Mery was tied up at the official‑warehouse site, her measuring rod in hand, her eyes tracing the lines of the granary floor; Lu Zhu had not lifted his head from his drafting board for an entire morning, his charcoal scratching back and forth across the papyrus in short, decisive strokes. When Shafi finally straightened from the ventilation slot, her spine aching from the prolonged crouch, her peripheral vision caught a dark shape on the gravel road. Her hand paused on the measuring cord, her fingers still holding the knot she had just tied. She finished marking the line she had been drawing — a single unbroken stroke — and only then did she turn around.

The man, seeing her turn, took one step forward and then stopped, as if a line had been drawn on the ground that he could not cross. ”Is this the former High Priest Imset’s healing hall?” His voice came out rough as sandpaper, each syllable scraping against a throat that had swallowed too little water for too many days.

Shafi did not answer immediately. She noticed a long crack running along the bottom of his medicine case, stitched with hemp thread that was a shade lighter than the dark leather — a later repair, the knots still tight and even, the needlework careful despite the rough material. ”Yes,” she said. ”Who are you?”

”My name is Sebek.” He set the case down carefully, as if the contents were fragile and precious, and from his sash he drew out a scroll of papyrus. The edges were frayed to a soft fuzz, and when he unrolled it, two places showed creases so deep that the fibres were nearly split through, the dark lines of wear running like veins across the surface. ”I practised medicine in Thebes for twelve years. I heard that Akhetaten was building a healing hall... I wanted to see it with my own eyes.”

Shafi took the papyrus. It was covered end to end with prescriptions and case notes, the characters slanting and crowded, but every herb’s dosage was plainly written — sometimes crossed out and rewritten in smaller script, sometimes squeezed sideways into the margin. Some of the phrasing was not from the formal medical canons; it was the rough tongue of oral tradition, of remedies passed down through families around cooking fires, never inscribed on temple scrolls. He had clearly never received training as a priest‑physician. But he had treated patients — many patients, by the look of the densely packed entries.

”What did you treat, in Thebes?” Imset’s voice came from the workshop doorway. He had walked out with lime mortar still smeared on his sleeves, the white dust clinging to the dark fabric like a second skin. He did not glance at the papyrus. Instead, his gaze fixed directly on the man’s hands. Sebek’s hands bore several old scars — deep parallel scratches, the kind made by a convulsing patient’s fingernails when someone pried their jaws open to keep them from biting their own tongue, the fingers caught in the gap and raked raw.

”Trauma. Fractures. Fever. Diarrhoea.” With each word Sebek spoke, Imset’s eyes moved across his hands — from the calloused fingertips to the bony wrists, from the wrists to the frayed cuffs. The cuffs showed several dark, irregular stains — old blood that no amount of washing had been able to lift, the fabric stiff and darkened.

”Come with me.” Imset turned and walked back into the workshop without waiting for an answer.

Sebek hesitated for a heartbeat, then picked up his medicine case and followed. By the time he crossed the threshold, Imset had already stopped before a temporary workbench — a rough timber table with a few tools laid out: a bronze scalpel, a wooden wedge, and a flat stone slab with fresh scoring marks across its surface. Imset took Sebek’s papyrus and spread it flat on the table, holding it up to the shaft of light that fell through a gap in the reed‑mat roof, the dust motes swirling in the yellow beam.

He read very slowly, his eyes tracing each line with the care of a man checking a wall for hairline cracks. At the end of every prescription, he pressed his fingernail once against the final character. The dried‑medicine powder still embedded under his nails — fine green residue from the morning’s chamomile‑sorting — left a pale, powdery impression beside each line, like tiny signatures pressed into the papyrus.

”This fever‑reducing formula,” Imset said, his index finger stopping on a particular line, ”the dosage for chamomile is written as ’one handful.’ How much is a handful? Your hand, or the hand of whoever happens to be dispensing the medicine that day?”

Sebek’s lips moved, but no sound came out at first. Then he said, ”I... I just grabbed it. By eye. It always worked before.”

”’By eye’ is not acceptable.” Imset’s voice was level, not harsh, but the words carried a finality that filled the small space, pressing against the walls. He reached under the workbench and pulled out a small bundle of dried chamomile, the leaves still holding their grey‑green colour, and spread it across the stone slab in a loose pile. ”Show me. Grab a handful.”

Sebek reached out and closed his fingers around the herbs. His hand was slightly larger than Imset’s — he scooped up nearly twelve stems in a single pass, the leaves crumbling between his fingers and falling back onto the stone in small fragments.

Imset reached out and grabbed his own handful. He scooped up eight. Two different hands, two different quantities, the difference plain and undeniable in the slanting light. ”This is why we need standards.” He laid the two handfuls side by side on the stone slab, the stems crossing each other like small armies drawn up for inspection, and said nothing more. ”Go on.”

Sebek stared at the two piles, his brow furrowed, silent for three full breaths. Then he gave a short, sharp nod — as if something had clicked into place inside his chest, a lock turning, and a new understanding taking root.

The second physician arrived on a flatbed cart that was carrying stone from the quarry — a woman in her early thirties, trailing a boy of ten or eleven who clung to the edge of the cart. She said she was not a physician, not really — she had been a helper at a small medical practice in Memphis. The practice’s owner had been conscripted for temple labour the year before and had worked until his heart gave out, and after that she had simply carried on, treating her neighbours, treating anyone who came to her door, sometimes for a handful of grain, sometimes for nothing at all. Her papyrus did not list which medical books she had studied; it listed numbers — how many patients she had seen each year, how many had recovered, how many had not, what medicines had worked, and for the ones that had not worked, how long the patient had lingered afterward. Every number had a mark beside it: a small circle for recovery, a diagonal slash for deterioration. She had kept this record for twenty‑one years, without missing a single month, without ever setting down her stylus.

When Imset read that sheet of numbers, the workshop went so quiet that the only sounds were the distant shouts of the kiln yard and the occasional, fleeting laughter of the sand‑sifting women on the riverbank — a sound that carried across the flats, light and unexpected. He looked up from the papyrus. ”You never studied the medical canons.”

”No.” The woman said it with her lips pressed tight, her jaw set, the way someone speaks a truth they have learned to brace against, her voice flat and unyielding. ”I can read — some. I learned the medicine names. The ones I didn’t know, I asked about. People told me.”

”How did you treat puerperal fever?”

”Washing,” the woman said flatly, without hesitation. ”Wash with cooled boiled water. Three times a day. If the fever does not break, use cloth soaked in vinegar — wring it out until it no longer drips, and do not soak the bedding. Medicine — only two kinds. Chamomile for fever, plantain for urine retention. Nothing else. If the mother takes too much medicine, her milk changes taste, and the baby will not nurse.” She recited it like a rule she had tested on a hundred women, each case burned into her memory, each failure a lesson she had not forgotten.

Imset listened without interrupting, then walked out of the workshop and toward the makeshift medical stall’s reed‑mat shelter. He Lu was changing a damp cloth on a child with diarrhoea, the child’s belly hollow and pale under the thin skin. He Lu looked up, and Imset said only: ”The woman from Memphis — take her on. Gynaecology is under your supervision. You set the terms yourself.” Then he turned and walked away, leaving He Lu to nod in the dust.

The third arrival was not a wandering physician at all — he was a young priest from Thebes, but not a priest of Amun serving the gods; he was a priest’s apprentice who had been expelled from the temple for reasons no one cared to ask about. What mattered was that he could recite thirty‑odd medical canons from memory, could write prescriptions in two different scripts, and could rewrite the canon’s vague ”moderate amount” into precise dosages measured in fractions — and he did it with a steady hand, as if the numbers themselves were the only truth worth holding onto.

But he could not treat patients. His fingers were too long, too thin, too cold — when he reached for a patient’s forehead, the patient flinched away, startled by the chill of his touch. It was the touch of a man who had never held a fevered child, never pressed a poultice against a sweaty chest, never felt the flutter of a weak pulse under his fingertips. After he had written his recommendations for prescription standards on a wet clay tablet, Imset said to him: ”Review today’s incoming medicine inventory.” The young priest entered the pharmacy, crouched before the first shelf, and began checking every entry against Shafi’s inbound ledger, starting with the first bundle of dried chamomile. He worked from sunrise until well past noon, without once standing up, without drinking a single sip of water, his eyes moving slowly down the rows of tags. When he finished the last page, he wrote in the margin of the ledger: ”Eight varieties received today. Every variety verified against the physical stock.” He did not sign his name. He did not ask what he should do tomorrow.

Imset, watching from outside the shelter, studied the young man’s bent back for a long moment — the spine curved from hours of crouching, the shoulders hunched as if bearing a weight only he could feel. He gave him no new tasks before the second week. Those silences — those ”no task assigned” silences — were themselves a test. The young priest did not come seeking instructions. He simply, in the gaps between inventory entries, reclassified every herb on the shelf with a new numbering system: starting from 001, every herb assigned a number, every number followed by its date of entry, its origin, its batch, and its estimated shelf life, written in ink so fine it might have been etched with a needle.

Ten days later, seven wandering physicians had come to the healing hall.

Two were rejected by Imset. Not because their skills were insufficient — but because, after dispensing medicine to a patient at the makeshift stall, they had not stayed to watch. The difference between good medicine and bad medicine is not the formula; it is time. You do not know whether a patient will react badly to what you have given them, whether the fever will spike or the pain will ease. The ones who stay — they are the ones who learn whether their prescription was right or wrong, who see the sweat break or the colour return. The ones who leave learn nothing but their own unearned confidence.

On the fourth night, Imset knelt before the pharmacy’s north wall with a freshly cut sheet of leather and a new charcoal stick. He wrote slowly — every character pressed deep into the grain, every rule checked against at least two patient cases before he committed it to leather. The oil lamp beside him flickered in the draft, casting his shadow huge against the wall, the flame bending and straightening like a living thing.

”Article One: All healing‑hall physicians shall wash their hands three times before each day’s consultations — once with clean water, once with abrasive sand, once with clean water.”

”Article Two: All prescriptions shall record the exact quantity of every herb. The words ’one handful,’ ’one pinch,’ and ’moderate amount’ are forbidden.”

”Article Three: Every prescription shall be retained on file, recording the patient’s name, age, symptoms, treatment, and outcome. Deaths shall be reported separately.”

”Article Four: Medicine prices shall be set uniformly by the hall and posted in the treatment area. No physician may add a private surcharge. Violators will be expelled from the healing hall.”

”Article Five: Patients with fever and throat swelling, or with severe diarrhoea, shall be isolated from other patients. They may not share a treatment room.”

”Article Six...”

By the time he wrote the thirteenth article, the charcoal had worn down to half its original length, the wooden shaft now stubby and blunt in his fingers. He set the charcoal down and held the leather sheet up to the lamp, tilting it to catch the light. The characters were not as even as Mery’s architectural drawings — a few near the end had been pressed too heavily, leaving small ink‑blots at the stroke‑ends; a few lines had been written with a blunted point, the strokes visibly thicker, almost furry. But he did not recopy it. That leather sheet — with its uneven strokes, its smudges, its palpable weight — was hung on the pharmacy’s newly plastered north wall as the original copy of Akhetaten’s first treatment standards. The plaster was still faintly damp, and the leather clung to it as if it had grown there.

Shafi was the first to verify the pharmacy against the standards. She stood before that wall for a very long time, holding her own copy of the leather sheet and her charcoal, checking every herb and every management requirement against the posted rules, her lips moving silently as she counted off the items. After the cross‑check was complete, she nailed a new leather sheet beside the pharmacy door. On it, in precise, evenly spaced characters, she wrote two lines:

”Inbound: register number, date, source, batch, quantity. Outbound: register date, recipient, medicine name, quantity, purpose.” In the blank space below, in smaller characters, she added: ”The pharmacy keeper and the duty physician shall conduct a joint inventory before each day’s closing. Any discrepancy exceeding two portions shall be investigated and documented on the same day.”

The charcoal was still glistening on the leather when the workers passing by saw her crouched by the pharmacy door, pulling the already‑stored herbs from their reed‑mat piles and re‑sorting them according to the new standards. She moved the dried chamomile to the first shelf below the ventilation slot — a height that kept it out of direct sunlight and away from ground moisture, where the air moved cool and steady in the narrow gap. Every time she finished a shelf, she hung a small wooden tag with a number on the shelf’s crossbeam. The tags had been cut from wall‑building scraps — different sizes and shapes, but every number written exactly the same size, the strokes even and straight, as if traced from a single template. On the back of each tag, she had marked a tiny registration line to indicate the tag’s correct orientation, so that no tag would hang crooked and confuse the eye.

The first change that followed the treatment standards was the patient records.

Before the standards — in the old days of the makeshift medical stall — a patient arrived, a physician glanced at them, felt their forehead, checked their throat, dispensed medicine, and the patient left. The next time that patient came back, the physician could remember the face of the one they had treated, but not what medicine had been used last time, or whether it had worked. The memory was a blur of faces, all alike in fever, all blurred by time.

The new standards had been in effect for two days before anyone took them seriously. The wandering physicians were used to prescribing by instinct — in Thebes, in Memphis, in every town along the river, patient records had never existed. A patient came, a physician looked, medicine was dispensed, the patient left. No documentation, no follow‑up, no comparison of one failed prescription against another.

The thing that changed everything was a child.

A shipbuilder’s daughter, about seven years old, had been running a low fever for seven or eight days. She had come to the healing hall before, and Sebek had been on duty. He had given her a fever‑reducing powder of dried chamomile and dried amaranth, in a packet for her mother to brew at home. Three days later, the mother was back with the child. The fever had not broken — it had climbed from low to high, the heat now pulsing through the small body. The child’s eyes were sunken, her lips cracked and dry, her skin so hot that it seemed to burn the mother’s palm.

Sebek stood at the treatment area, holding the child’s old prescription leather sheet, turning it over and over in his hands, his brow deeply furrowed. His expression was that of a man staring at a cracked wall — not knowing where to begin the repair, the mortar too old and the stones too loose.

Imset came from the pharmacy. He took the leather sheet, studied it for a moment, and did not ask Sebek why there was no record of the follow‑up. He simply picked up his charcoal from the workbench, pulled a fresh leather sheet beside the old one, and began to write, his hand moving in a steady, practised rhythm: ”Seven‑year‑old girl. Initial visit: seventh day after the river’s retreat. Chief complaint: low fever, loss of appetite. Treatment: dried chamomile three portions, dried amaranth two portions, decocted. Follow‑up visit: tenth day after the river’s retreat. Symptoms worsened: high fever, sunken eyes, signs of dehydration. Revised treatment: fresh reed‑root extract for rehydration, reduce dried amaranth to one portion. Observe for one day.”

He finished writing, pasted the leather sheet on the pharmacy’s exterior wall — the wall every physician had to pass on their way to the medicine stores, the wall that now bore the first real patient record in Akhetaten.

Sebek stood before that wall for a very long time. His lips were moving — he was silently memorising every character, every number, every logical step of the revision. Then he turned, walked back to the treatment area, and from his own wooden chest began pulling the memory of every patient he had seen in the past few days, writing them out one by one on fresh leather sheets, his hand growing steadier as he went. Some cases he remembered clearly — he had always remembered the ones he had cured, their faces bright with relief — some he could only sketch roughly, the details blurred by the passage of days. ”The old man with the coughing blood — what did I give him?” he asked He Lu.

”Dried arborvitae leaf,” He Lu said, securing a splint on a fracture patient without looking up, his fingers busy with the linen bindings. ”And licorice root — you told him to chew it. You said chewing worked faster than decoction.”

Sebek blinked. He had not expected He Lu to remember. He had not expected He Lu to add: ”The old man never came back. I don’t know whether he recovered or died.”

Sebek wrote ”Dried arborvitae leaf two portions, licorice root one portion, chewed — no follow‑up” at the bottom of that leather sheet, the characters now noticeably more precise than when he had first entered the hall — because Shafi’s inbound ledger was posted by the pharmacy door, and every physician who drew medicine saw those precise, quantified entries, the numbers clean and unarguable. When everyone in a place measures with the same rule, you find yourself measuring more precisely too, as if the rule itself reaches into your hand and guides your stroke.

The second change was the medicine lock.

Before this, the healing hall’s pharmacy had no lock. To be precise, nothing in Akhetaten’s construction site had locks. Workshops and material yards were open to anyone — a worker could walk in, pick up a chisel, take a handful of lime, and leave without a word. The medicine was the same. Whoever came to take herbs took them without signing, without asking, without leaving a trace. No one knew what had been taken, when, or by whom. No one knew how much remained. The shelves were a silent ledger that no one read.

Shafi was probably the first to notice the problem. During her first inventory count, she found the quantity of dried yarrow on the ledger was short by nearly half a palm’s worth — roughly three and a half portions, a gap that stared at her from the page like a missing tooth. Not enough to accuse anyone — there had been no records to begin with. She simply drew a question mark beside the ”opening inventory” column on the leather sheet and noted the discrepancy in the remarks, the mark light but deliberate, a silent warning to herself. Then she found a carpenter and had him make a wooden bolt. The bolt was not large — a single plank of acacia, planed smooth with a scraper, its edges sanded to fit the groove. Once installed, the door could only be opened from the inside, not pushed from outside. Every evening before sunset, she had a fixed routine: she copied all the day’s prescriptions into a single summary, tallied the total usage by herb, recorded the remaining balance on a leather sheet posted beside the pharmacy door, and then slid the bolt home. The bolt slot was freshly carved, and when it slid into place, it made a low, grinding rasp, the wood scraping against wood. That sound became the pharmacy’s nightly signature — over time, everyone in the healing hall knew: when they heard that rasp, the pharmacy was closed, and no one could enter without the keeper.

From the day the bolt was installed, it was opened more and more frequently.

First Sebek came knocking at midnight for dried chamomile — the high‑fever child in his care had spiked again, the heat rising like a tide through the small body. Then He Lu came before dawn for plantain seed — Nefer had admitted a woman with postpartum infection and urinary retention, the woman’s face pale and sweating, her pulse thin. He Lu’s hand was trembling as he slid the bolt — the tremor of exhaustion after a full day and night of patient care — but his fingers were still steady enough to find the shelf by feel, following Shafi’s labelled divisions, reaching the right herb on the first try, his fingertips brushing the dry leaves. After that, he never needed a lamp to find the pharmacy’s shelves again — not because he had a good memory, but because every one of Shafi’s tags hung perfectly straight, without a single crooked or shifted marker, each one a fixed star in the dark.

Before dawn, Sebek came again. As he drew the bolt, he noticed the door hinge had not been oiled — the hinge joint let out a sharp, grating squeal that cut through the pre‑dawn quiet. He would find the stores carpenter later for a lump of tallow, wrapped in a scrap of cloth. In the pharmacy, he navigated by moonlight filtering through the ventilation slot, the pale silver light falling on the second shelf row — the chamomile stores felt thinner than they had two days ago, the bundle smaller under his hand. He did not light a lamp because the texture of dried chamomile leaves was unmistakable — cool, rough, brittle — and in that hour, that coolness was more reliable than sight, a language his fingers had learned.

The rasp of the bolt sliding open and closed echoed many times that week. The frequency caught Lu Zhu’s attention. He sent a carpenter over. The carpenter added a wooden crossbar to the bolt, inserted diagonally into the bolt socket to lock the bar in place, carved a groove in the wall to hold it, and waxed the groove with beeswax before leaving, the wax smooth and yellow.

”Now physicians working the night ward can come and go without needing someone to help them with the door,” Lu Zhu said, not looking up from his drawings, his charcoal moving in short, decisive strokes.

Shafi built three more systems on this foundation. First, she wrapped a spare bolt core in linen and hid it in the pharmacy’s south wall, behind a loose brick on the second course — only she knew which brick, its edge slightly chipped. Second, she scored a fine line on the bolt shaft at the height corresponding to the busiest nighttime dispensing hour — she had measured it: roughly one and a half watches before dawn, continuing until skybreak, the line as thin as a hair. Third, she assigned a number to her own key and recorded it on the leather sheet — fixing the chain of custody between lock, door, store, and person in permanent writing, so that every opening could be traced, every hand accounted for.

The third change came in the second week.

Every morning, when the brick‑drying yard outside the healing hall still floated in a layer of cool mist, someone was already in the treatment area. Not a patient — it was the young priest’s apprentice from Thebes. He sat in the reed‑mat corner of the treatment area before daybreak every day, a worn papyrus scroll spread across his knees, line by line comparing the formulas in the medical canons against the actual prescriptions the healing hall was using, his eyes moving back and forth like a weaver’s shuttle.

He was not looking for who was right and who was wrong. He was looking for the correspondence between ”the words on the ancient scrolls” and ”the diseases in living patients” — what the scrolls called ”heat entering the pericardium” actually looked like in a patient; what the scrolls called ”dry fire damaging the fluids” actually appeared as in the morning’s case records; whether there was a prescription in the canon that could remove its rare herbs entirely, replace them with locally available substitutes, and still achieve the same effect. Every comparison he made, he wrote on a fresh sheet of papyrus. The characters were small, dense, precise, packed so tight that the sheet seemed to hum with them. The edges of his papyrus were worn soft — not from copying errors, but from being handled, read, and re‑read, the fibres soft as cloth.

Imset saw that paper one morning. He did not praise it. He simply added one line at the bottom of the page: ”To be added as Standard Article Fourteen: Wherever a canon formula prescribes herbs unavailable locally, the prescription shall note the substitute and the reason for substitution.” After writing, he crouched before the young priest and pointed to one line: ”You wrote, ’Licorice root’s cooling effect can be replaced by local sweet beet root, but sweet beet root’s lung‑moistening effect is weaker than licorice.’ Did you test this yourself?”

”No.” The young priest looked up. His eyes were faintly red from reading in the dim light of the reed‑mat corner, the whites veined with fatigue. ”I asked Shafi. She helped me slice the sweet beet root, dry it, grind it to powder, and steep them separately in water of the same temperature, to see which dissolved faster. Sweet beet root dissolved one and a half times as fast.” He paused, then added: ”But sweet beet root cannot replace licorice’s full effect. Because sweet beet root has no detoxifying property.”

”So?”

”So I need a third ingredient to fill the gap.”

”What do you propose?”

”I haven’t found it yet.” The young priest laid his papyrus flat across his knees, his fingertip pressing down on a blank space, the papyrus denting under the pressure. ”But I already know what that gap needs to do.”

Imset stood up. His knee gave a crack — he had been crouching too long, the joints stiff. He said nothing more. That afternoon, he pulled from his personal collection several pages of detoxification formulas from the medical canons — pages he had risked confiscation to copy privately in Thebes, some so weathered they were nearly translucent, the ink faded to brown — and laid them on the young priest’s reed mat, weighing them down with a smooth riverbank stone so they would not blow away, the stone cool and flat. Then he left without a word, his footsteps soft on the packed earth.

The young priest looked at those pages. His fingers, when he turned the first sheet, moved across the sand‑grained surface so lightly they barely grazed the fibres — the pages were too fragile, too old, to bear the weight of a careless hand. Then, squinting in the morning light, he began to read. With each page he turned, his index nail traced the final stroke of each character, leaving a faint, ghost‑like impression, as if he were memorising the page by touch.

A few days after the treatment standards were posted, the people living nearby finally began to take ”Imset’s healing hall” seriously.

Before that, most of them called it ”that big building by the river.” After the roof went on, it became ”that building with the roof.” There were people being treated inside, yes. But no one knew what the rules were. They had only heard that some people had been cured, that others had been given medicine without being charged, and that the medicine prices were posted on the beam above the treatment area — anyone who walked in could look up and see the prices, crooked but legible, from dried chamomile to dried dates, from children’s fever powder to gynaecological herbal baths. A fixed price. No one charged differently based on who you were. The beam was low enough that even a child could read it, the characters etched deep into the wood.

More people came. The workshop was converted into a waiting area; the reed mats were not enough. Among the new arrivals were patients who had already been cured. They built a small shade shelter outside the waiting area themselves — using scrap reed mats left over from roof construction, pieces that could not be used whole but could be patched together with palm‑fibre twine. The shelter leaned to one side, its corner posts not quite straight, wobbling in a strong wind, leaking in the rain — but they came every day to repair it, straightening the posts, retying the twine. They said: ”We don’t know how to heal people. But we can keep the sun off the ones waiting in line.”

When the waiting area was full, those who came late stood on the sand under the shade shelter. There were no seats on the sand. But there was something that had never existed before — an old woman sitting there, fanning a child in her arms with a palm leaf, saying to the person beside her: ”In Thebes, you had to bribe the priest just to get a place in line. Here, it doesn’t matter whether you have money. You just line up, and the prices are posted on the beam. I’ve checked them myself — not a single price has been changed.” Her voice carried, and a few heads nodded.

Shafi stood at the edge of the waiting crowd, listening. She did not speak. She just stood at the crowd’s edge and recalculated the total dispensable medicine on her inbound‑outbound ledger once more, her charcoal moving in short jabs. At the current rate, she had perhaps six days left. Tomorrow would be the seventh — from tomorrow, she would have to begin rationing daily dispensations, even as she continued processing incoming shipments. How much longer could that stretch? She calculated on the leather sheet — under the tightest rationing, perhaps another seven or eight days, if every dose was measured to the grain. After that, only emergency reserves would remain, a bare handful of bundles. No meat. Just bones. The numbers stared back at her, unforgiving.

From the pharmacy came the rasp of the bolt being drawn. The door swung inward, releasing the scent of stored herbs — dry, sharp, medicinal — into the afternoon air, the smell curling out like a living thing. Imset stepped out, stood at the edge of the shade shelter, and looked at the queue — counting how many new faces, how many returning, how many had the hollow look of long illness. He did not finish counting before a woman carrying a child called out to him — the child had come before, had improved, but now the fever had spiked again, up and down for two days, the mother’s face drawn with worry. Imset crouched, felt the child’s forehead, and his brow tightened slightly, the skin hot and dry. As he carried the child into the treatment area, the workers passing by stepped aside to clear a path, their tools falling silent for a moment.

As the sun set, the bricks still drying in the yard were stained orange‑red, the light long and golden. Shafi added a new line to her leather sheet: ”Day eight. Recalculated total dispensable medicine — under strictest rationing, sufficient until day eleven. After day twelve, emergency reserves only.” She wrote the characters with a steady hand, but the charcoal was nearly gone.

She hung the leather sheet back on the hook by the pharmacy door, slid the bolt home. The rasp of the bolt settling into the packed sand floor faded in the evening wind, the sound swallowed by the vast sky. In the distance, Lu Zhu was marking water‑pipe alignments on a wooden board with his charcoal. He drew a double line toward the pharmacy — once this pipe was laid, it would provide the pharmacy with steady hot water for decoctions and poultice preparation, the heat carried through clay pipes. He looked at the line and did not finish it. He knew the pipe would have to be laid before the medicine ran out, the water flowing faster than the herbs could be replaced.

Meru walked toward the ferry crossing, standing in the river wind for a moment, the breeze tugging at his robe. He called out to a Nubian cargo‑boat captain who was about to weigh anchor — a captain he had done business with before, carrying timber from the south, the boat low in the water. ”Coming up from Punt — have you seen any spice plantations along the way?” The captain looped his anchor rope around his hand, the hemp rough against his palm. ”Yes,” he said. ”Why do you ask?” Meru pulled from his sash the medicine list Shafi had compiled — the front side listing herb names and numbers, the back side a summary of the herbs most urgently in need of resupply from outside, each one marked with a small cross. ”Ask around in Nubia for me. If you find any, the price is negotiable.” The captain nodded, his eyes scanning the list.

The bow of the boat cut a white wake into the dark water, the oarsmen calling out as they adjusted course, their voices carrying across the current. Salt crystals had formed on the oar handles, white and granular, like frost in summer. One of the boatmen picked off a flake with his nail, tasted it. Salt. The taste of the wind. The river wind carried the ripple from the oars toward the reed beds on either bank, the water lapping softly. The reed stalks, soft and flexible, dipped as the water passed, then rose again, bending and straightening in a slow rhythm.

An old stonemason crouched by the quarry pit, stone chips piled into a small mound before his knees, the dust rising in the still air. A river‑worn pebble in his hand had weathered to a brittle shell — it crumbled at a touch, the same pebble that had been unloaded from a boat days earlier, dried in the sun for six days until it powdered, the grains falling through his fingers. He shifted to his fourth position in the pit, each spot yielding a different stone quality — some hard, some soft, some waterlogged, some too wet, the moisture seeping from the fractures. The stone in his hand, collected the day before from the south bank, had soaked overnight in a basin of water; when he took it out and squeezed, a white slurry coated his fingertip — too much clay, the sediment clinging like paste. He discarded the stone and the slurry, drawing a dividing line in the sand beside his knee, separating usable stone sources from unusable, the line sharp and final.

The lime‑sorter crouched beside that dividing line, marking a perpendicular cross with his fingertip, the lines intersecting cleanly. The mark meant: from here, take the south‑bank route for stone, saving the labour of hauling from upstream — that stretch was silted, the boats slow, a round trip taking two full days, the oarsmen exhausted. A day’s stone had to be split across two days of hauling; the kiln’s brick moulds were going unfilled, the clay drying in the sun.

Beside his mark, two brick‑yard workers were loading coarse sand into reed baskets. The sand, fresh from the riverbank sieve, was mixed with mussel shell fragments and small pebbles — the sieve holes were too large, letting through debris the size of a thumbnail. They did not know what those shell fragments meant for the kiln. In firing, the shells expanded at a different rate than the clay, cracking the bricks from within, hairline fractures that would widen with each freeze. In winter, the cracks widened and the brick faces flaked, the surface peeling like old skin. They worked mechanically — pour, sieve, pour, bending and straightening so many times that the motion became mechanical, like a mill donkey walking its endless circle, no one questioning the sieve’s gauge, the rhythm numbing the mind.

The kiln‑tender crouched on the warm ash outside the kiln, sorting the newly fired bricks by colour, his hands black with soot. Ash‑gray — overfired, hard enough but brittle along the edges, prone to chipping; dark red — underfired, only fit for foundation fill, the clay still soft; the ones with a dark glaze on the surface rang when struck — he counted fewer than four in ten that were truly wall‑worthy, their tone clear and sharp. The rest were either foundation filler or left beside the kiln for someone to use as paving, their edges crumbled. He stood from the warm ash, the wooden mallet in his hand still hot; he plunged it into a bucket of river water — a sharp hiss, a cloud of white steam, and he jerked his hand back, the heat stinging his palm. He said nothing, just turned the mallet over and continued soaking it, looking toward the river. The sand was not good enough. The stone was not good enough. The brick could only fire to this standard. Not a single thing in his hands was sufficient. The weight of it pressed on his shoulders.

”The drying yard — the new bricks aren’t coming out fast enough to keep up with the wall,” the bricklayer said. His lips were cracked from dehydration, his voice scraping like gravel, each word dry. ”The kiln’s fire isn’t hot enough. The sand isn’t sifted clean — the bricks come out cracked. Cracked bricks crack the wall.” He paused, then added, ”The healing hall’s wall can’t wait any longer.” He looked at the unfinished wall, its edge jagged.

Lu Zhu stood on the high ground of the drying yard, the batch of freshly fired bricks at his feet, their warmth still rising. The gray ones he had already set aside — too much vitrification, the surface and core too different in hardness, prone to cracking at the mortar joints, the fractures invisible until too late. The dark red ones he had not yet given up on. He crouched, placed one on a knee‑high stone, and struck it with his hammer. A light tap — the sound was dull, flat. Another tap — it cracked, the line running crooked. He picked up the broken piece, pinched the fracture surface — slightly darker than the brick face, the colour uneven. Not underfired. It was the clay itself: too much silt in the mix, uneven shrinkage during firing, the layers pulling apart. He brought the fracture to his nose and sniffed. No char. No char meant the kiln temperature had never reached the right point, the fire too low. The kiln‑tender had said the fire was not hot enough — but the fire’s problem was not just fuel or kiln leakage; unsifted sand also caused uneven heat transfer, the heat escaping through the gaps. And the sand’s source was the sieve; the riverbank stone’s weathered layer was too thin, the quarrymen could not find good enough material, the good stone already stripped.

”Tell Kamon — don’t bring the bricks from the gravel road up yet,” he said to a worker beside him, his voice level but urgent. ”Pull the old stonemason from the quarry pit and put him on the main kiln. Take a crew to the south bank for new samples. Soak every new stone overnight — any that crumble when squeezed are unusable. Reduce the river‑sand sieve holes by a third. Sift twice —” He stopped, drew a breath, then continued. He finished giving those instructions, then called out to the kiln‑tender who had been crouching by the kiln for hours: ”The dark red batch — return all of them to the kiln for refiring. Sort out the gray ones — use them for foundation fill, not for the wall.” The kiln‑tender nodded, his face smudged.

Then he walked toward the lowest brick stack — a batch that had been fired two days earlier and not yet inspected, the bricks still warm to the touch. He crouched before the stack and re‑sorted every brick into the three grades he had just defined, his hands moving quickly. The charcoal in his hand never stopped moving as he inspected; for every five bricks, he drew a tally mark on a wooden board beside him, the strokes accumulating. The tally marks on the wooden board grew and grew, a silent count of failure and salvage. When he finally stopped to count them, he realized the sand‑sifting problem was not a sand‑sifting problem at all — the upstream riverbank sand suitable for brick‑making had already been exhausted by every other construction site competing for it. They were sifting the same sand over and over, a futile circle. He stood up, wiped his brow, and looked at the river, the water glinting in the late light.
