Nile Builder: The Architect from the East

Chapter 84: Chapter 6: The Field Hospital Rises — The Old Square Becomes a Clinic

Neht’s pharmacy hut was knocked up in the small hours of the night.

The knockers were two able-bodied men carrying a door plank between them, their arms locked under it, their breath smoking in the cold. On the plank lay a man, burning with fever, his breath coming short, the wound on his left shoulder swollen until the skin shone, the pus soaking through the bandage strips. The man carrying the front of the plank was so frantic he could hardly get the words out: ”Uncle Neht! Quick! A-Ka... he’s been burning for two days!”

Neht opened the door in his coat, the lamp in his other hand. The lamp was a clay bowl lamp, the wick trimmed, the flame small — he had learned long ago to waste nothing, and oil was precious. He held the lamp over the plank and looked — and his face changed. He had treated wounds in this city for half a lifetime, arrow wounds and knife wounds and the wounds of falling stone; one look told him the wound had been neglected too long. The lamp trembled in his hand, and the shadow of the wounded man swayed on the wall.

”Why didn’t you come sooner?” He let the men in.

”He said it was a small wound.” The carrier said. ”Washed it, wrapped it, went back up on the wall. This month past, the wound healed and split, healed and split — he carries mud-brick every day, and the scab never had a chance to set. Every time it split, he said it was nothing, and went back to work. Last night he came down off the wall and couldn’t stand.”

Neht asked no more. He lifted the bandage strips from A-Ka’s shoulder — the strips were stuck fast, the dried discharge gluing them to the skin, and he worked them loose with water, a little at a time — and a smell came out, like fish left too long in summer, heavy and sweet and wrong. The lamp trembled again, and he set it down on the table to steady it.

”The city’s infirmary has only this room of mine left.” He said, low, as if speaking to himself. ”The wound medicines ran out long ago. All I have here is salt water, and a little burnt plant ash.”

Inside the room, the wounded of the past days lay crowded on reed mats — arrow wounds from the wall-head, a leg crushed by fallen brick, an old man whose face had been grazed by a stray arrow, the graze turning angry at the edges. The mats lay so close that a man could not turn without touching his neighbor; the groans rose and fell, like a roomful of leaking sacks, and the air was close with the smell of wounds and sweat, and of the boiled herbs that had done all they could.

”Wait.” Neht said. ”Your master builder sent word the other day — the new capital’s medical team is coming in. If they get in, there’s hope.”

”When?” Someone asked. The voice came from the dark of the mats, thin and young.

Neht did not answer. He looked toward the west gate. There, only the black weight of the night. He had looked at that night for three nights running, waiting for a boat that had not come; the looking had worn a place in him.

In those days, the city’s wounded grew more numerous, day by day.

During the water shortage, people had drunk the leftover water of the aqueduct to save effort — and doubled over with stomach pains, their faces gray, unable to stand, the pain gripping them in waves. On the repair sites, men crushed their feet, wrenched their backs, cracked their heads on falling brick — almost every day, the stretchers came down off the wall, the injured men’s comrades carrying them with the tools still in their belts. After that, the rebels’ roving riders circled the city every night, and stray arrows flew onto the wall-head from time to time, coming out of the dark without warning, the shafts thudding into the crenellations; the night watchmen came down wounded, two or three a day, the arrows still in them. Old wounds not yet healed; new wounds piled on top. The city’s hurt was a ledger no one had time to balance.

Neht’s pharmacy hut had long since been stretched beyond its strength. When his styptic herbs ran out, he pressed burnt plant ash on the wounds; when the ash ran out, he taught people to use spider webs — stretching the webs over the cuts, pressing them down with his thumb. Someone asked him: can spider webs really stop bleeding? He sighed, and said: ”They can. My grandfather’s generation got through on them.”

”But your grandfather’s generation,” the man said, ”wasn’t at war.”

”They were.” Neht said. ”They fought worse. Which is why they all died young.”

That silenced the room. In the dark, only the heavy breathing of the wounded — one breath, then another, the room breathing like one great lung.

Neht counted those breaths in his heart. When the count grew long, he murmured: ”Priest Imset — come quickly.”

Imset’s boat arrived that very night.

Two flat-bottomed boats, hugging the reed beds of the west bank, running downstream with the current. The lamps on the boats were masked with black cloth, leaving only a slit of light, a thread of it lying on the water. The forward holds carried the men; the stern holds carried the boxes — twelve medicine chests, gauze and splints and clay pots, wrapped tight in oilcloth, the bundles lashed so that they could not shift. Shafi sat among the chests, one hand on the rope that bound them, and did not let go all night. The rope was warm from her hand by the time the boats touched shore.

They had waited half a month for this crossing.

After Lu Zhu had gone south, Imset had built up the new capital’s healing hall, entry by entry: wound medicines, three shipments on the trade roads, the packets counted into the chests with the same care he gave to a diagnosis. The bleeding-cloth was woven by the women working in shifts, the looms never still, the cloth cut and rolled to the same width. Even the clay pots for brewing medicine were fired anew in a kiln to the clinic’s measure, the rims glazed so the medicine would not eat the clay, the pots stacked in the kiln and drawn out warm. When the foundations were thick, Thebes’s letter had arrived — the city’s wounded were growing in number; what was lacking was not doctors, but medicines, and a place to keep them.

”Teacher,” Shafi had asked, the night before the departure, ”how much do we bring?”

”As much as we can carry.” Imset had said. ”We’ll only know if it’s enough when we get there.”

Shafi had asked nothing more. She had gone through the medicine chests that night: wound medicines at the bottom, bleeding-cloth in the middle, splints strapped to the inside of the lids. Every chest numbered, the numbers written with the same careful hand she used for accounts. What was in chest number one, she could recite with her eyes shut — and she did recite it, once, to the dark, before she slept.

When the boats passed the river stretch guarded by the rebels’ river soldiers, the poleman A-Tu touched the water with feather-light strokes, the poles going in and coming up without a sound. Everyone on the boats held their breath; only the sound of the water remained, stroke by stroke, whispering against the hulls, and the far hoot of a night bird.

”Almost there.” A-Tu said, low. ”Past the channel ahead, and it’s our ground.” His pole went in without a sound, and the boat slid forward, the reeds closing behind it. The water was black and still, and the boat moved through it like a thing that had learned the river by heart.

The boats were the same boats that had carried the craftsmen south; the hulls were the same, but the cargo was different — instead of lime and straw, the holds were packed with the city’s chances. Imset sat at the bow, saying nothing. His eyes were fixed on the south — there, in the black shape of the wall, a few torches flared and dimmed. He knew that under those torches lay his patients; he had been counting the fires of this city since the river bend, and the counting had told him how many were still burning.

The boats touched the bank at the first cockcrow.

The soldiers waiting at the landing had been there since before dawn, their torches hooded, their eyes on the water. Shafi was the first off, jumping ashore before the hull had stopped moving and turning at once to carry the medicine chests, her feet finding the bank in the dark. Someone moved to help her; she would not let him: ”This chest holds the wound medicines. It must not be jostled.” She carried the chests up herself, one by one, and the soldiers stood back and let her.

Lu Zhu was waiting for them at the site chosen for the hospital.

The Old Market Square stood at the city’s center, not far from any of the four gates. In the old days the square had been the market ground, loud with the cries of sellers from dawn to dusk — the grain sellers at the east edge, the potters at the west, the cloth and the fish in between. Now it stood empty — a few old hitching posts, their ropes long gone, their wood gray and split; a stretch of ground bleached white by the sun, packed hard by a hundred years of feet. Imset stood in the middle of the square, looked around once, and nodded. He stood where a patient’s stretcher would stand, and looked at the ground as if reading it.

The place had been chosen by the chief officiant.

The night before, Lu Zhu had asked the chief officiant which ground in the city would serve. The chief officiant had said: ”The Old Market Square. When I was young, I helped put up relief shelters there — the ground has one virtue: everything around it is high, and it is low, but low without flooding. The dirty water of the market used to soak away overnight, and the ground was dry by morning. The old people said the square stands on a layer of sieve soil.”

”Sieve soil drains fast.” Lu Zhu had said. ”Drains fast, and the ground stays dry. A hospital fears damp — a damp floor is a wound’s second enemy, and the first is the blade that made it.”

”So I said: that is the place.” The chief officiant had said. ”The square has soaked away the city’s washing water for a hundred years. It can soak away the wounds’ water too.”

In the first gray of the morning, Lu Zhu had brought his people to look it over. He squatted, scooped up a handful of soil, and worked it between his fingers, feeling the grain of it; then he splashed half a gourd of water on the ground. The water pooled a moment on the surface — and then slowly sank, the pool shrinking as the earth drank it, leaving only a darker stain.

”Good ground.” He had said. ”It saves us the work of raising it.” He pressed the wet stain with his heel, and the water came up around his foot and sank again, and he nodded once, the nod of a man who has read the ground and been answered.

”Then it is here.” Imset said, standing in the square. ”High ground, no standing water. Wounded from all four gates can be carried in. Leave the middle open; build the rooms on three sides.”

”Three sides?” Lu Zhu asked.

”Treatment on one side, wards on one, medicines on one.” Imset said. ”The same principle as the healing hall — people keep to people’s paths; medicines keep to medicines’. The open middle is the flow. A wounded man comes in, passes the treatment bay, and is sorted into the wards — the critical to the left, the light to the right, so the two streams never collide. A room is a machine for saving; the flow is its axle.”

”The hospital’s drawings,” Shafi said, drawing a sheepskin from her breast and unrolling it, ”I finished on the boat. Scaled down from the new capital’s healing hall to three sides. Look —”

Lu Zhu looked at the drawing in the morning light. The lines were fine and even, drawn with the same patience she gave to her accounts: the treatment bay, the ward, the medicine bay, each in its own grid, clear and distinct, the doorways marked with their widths. The medicine bay was marked raised half a foot — the granary’s damp-proofing method, carried over whole.

”A good drawing.” Lu Zhu said. ”The work begins today.” He handed the sheepskin back to Shafi, who rolled it and tucked it into her breast with the same care she gave to her register.

When the light came, the square was transformed.

The craftsmen who came were Kamon’s crews; the people who came were the city’s folk, who had heard it was ”a place for saving lives” and hurried over — the women with their sleeves rolled, the children running ahead, the old men coming to see with their hands clasped behind their backs. The sound of ramming, the sound of adzing wood, the sound of reed matting being shaken open — all joined in the square at once, a sound the city had not heard in weeks. The sound drew more people as it rose, until the square’s edge was lined with watchers who had come to help and stayed to learn.

The materials had been assembled before dawn. The mud-brick was the wall site’s surplus, stacked square and even, the bricks still cool from the shade, counted out in stacks of ten. The timber was the yard’s stock, enough for thirty uprights, the poles chosen for straightness and bound in bundles, the bark peeled so the rope would hold. The reed matting came from the stores at the landing, rolled and carried on the boatmen’s shoulders, the rolls as tall as a man. The sailcloth — A-Tu had torn it from his own boats, the spare sails of both his craft, given whole, the cloth still smelling of the river. He said: ”A boat on the water has its sail; a roof over the square has its cloth. Both of them are ’shelter.’”

”This sail of yours,” Kamon said, ”is your livelihood.”

”A boat does not sail by its sail; it sails by its pole.” A-Tu said. ”And this city does not sail by sails — it sails by the place you are building here.”

Old Montu led the stonemasons of the south in laying the floors. They lifted the stone slabs out of the old drainage ditch at the square’s edge, washed them one by one in the trough, and laid them before the door of the treatment bay — the ground the stretcher-bearers would step on first, which had to be flat, and steady, and true.

”The feet of the carriers,” Old Montu said, setting a slab and testing it with his weight, ”must not waver by a single step. Every stone on this ground has to obey.”

The women wove the reed matting; the children carried the chopped straw. A half-grown boy ran with a bundle of straw, tripped, and fell — the bundle flying out of his arms. He got up, and looked at the straw first, then at his own knee; the straw had not come loose, and he grinned, and ran on.

The hospital’s walls were not raised high — waist-high. The walls were mud-brick, two layers thick, the space between filled with chopped straw — against the wind, and against the night damp, the straw bedding the bricks so that the two faces worked as one; a man standing inside could rest his elbows on the top and look over. The walls did not reach the top; the roof was separate: timber uprights, crossbeams, and over the beams, coarse linen stretched, the cloth rising in long planes like the hull of an upturned boat. The cloth was the landing’s sailcloth, old but tight-woven, the weave close as a basket; the sun filtered through it soft, and the light on the faces beneath it was gentle, the color of old honey.

”This roof,” Kamon said, looking up, ”is better than matting. The sun cannot burn through it; the night dew cannot soak it. Sailcloth for a roof — only the days of a siege could think of it.”

”The sail has carried boats across the river,” A-Tu answered from below. ”Now it comes ashore to shelter men. It is the boat’s other end.”

Imset took no part in the banter. He walked along the walls, looking at everything: whether the wall-foot stood clear of the ground, whether the doorways opened true, whether a stretcher could turn without scraping, whether a man standing at the door could see the full length of the room. Where something was wrong, he squatted and drew in the sand with a twig for the craftsmen to see — the drawing speaking louder than words.

”Here,” he said, pointing at a doorway, ”widen it by two spans. When a stretcher comes through, the two carriers must pass side by side. Too narrow, and the wounded man’s feet will strike the wall.”

The craftsmen measured, nodded, and tore down half the wall to rebuild it.

The next day’s work was raising the beams and stretching the roof.

The carpenter gang set the thirty uprights inside the wall line, lashing each to its neighbors with hemp rope, crossed and re-crossed, bound once and bound again, the knots checked by the oldest of the carpenters before a beam was laid. The swarthy man brought the ramming chant to the square — ”Heave — ho!” The chant and the mallets’ thud mingled, shaking the dust off the sailcloth above, and the dust drifted down in the light.

”Gently!” Shafi called from the medicine bay. ”There’s dust on the medicine chests!”

The swarthy man turned his head and laughed at full voice: ”Don’t worry, girl! This chant has aim — the dust falls down, not onto the medicines!” When he called the next count, sure enough, he lowered the pitch by a notch, and the mallets followed him down. The whole square laughed, and the rhythm of the work never broke.

The sailcloth went on last. Four heavy ropes, a stone at each corner, stretched the cloth flat and even, the stones set so that the cloth could not lift in the wind. The extra cloth at the corners was cut off — and Shafi kept it, folding it carefully. She said: for wrapping wounds later; cloth this tight-woven was short in the city, and nothing that could save a life was wasted.

”This hospital,” Imset said, standing under the finished roof and looking up at the cloth that moved faintly in the wind, ”is the city’s third roof from today. The wall is the first roof; the granary is the second; the hospital is the third. As long as the three roofs hold, the city holds.” He said it quietly, the way a man states a thing he has been thinking about for a long time, and the people working in the square heard it and carried it home with them.

Shafi watched the medicine bay more closely than anything. She had the floor raised half a foot, spread with gravel, then reed matting, the layers packed with the same care as a granary’s floor. In the wall-foot, four vents were cut, with tile shards angled outside them — air in, rain out, the angle set so that a driving rain could not enter. The medicine chests were never set on the floor: they stood on wooden racks, layer by layer, three spans back from any fire.

”Medicines are like grain,” she told the women helping her, the women nodding as they worked. ”They fear damp, they fear insects, they fear being smothered. Damp molds them; insects hollow them; smothering turns them stale. Any one of the three can kill a man.”

A woman listened, absorbed, and said: ”Girl, that’s just what my mother-in-law says about drying dates.”

”The principle is one.” Shafi said. ”Drying dates stores the sweet in the mouth; storing medicines stores the root of life.”

By noon, the three walls stood half a man’s height, and the news was all over the city.

A-Ka was carried to the square when the news was at its loudest — and by then he was burning so hot that he was talking nonsense, his words running together, his eyes open but not seeing.

Neht followed behind, and gave Imset the account of the wound as they walked: an arrow wound, in the left shoulder, washed with water at the time; now festered, burning for two days. He gave it plainly, the way one craftsman hands a tool to another, and Imset took it the same way. Imset said nothing; he only had Shafi carry A-Ka into the treatment bay, whose walls were only half built, the daylight coming in over the unfinished bricks.

He undid the bandage strips and bent over the wound. He looked at it for a long time — so long that the people around held their breath, the only sound the rattle of A-Ka’s breath and the far beat of the rammers. When he straightened, his face was the face of a man who had made his judgment.

”There is hope.” He straightened. ”The pus must be cleaned, the dead flesh cut away; when it is cut, wash three times with salt water, and dress it with honey.”

”Honey?” Neht stared.

”Honey.” Imset said. ”Honey does not spoil, and insects will not come near it. The old medical books call it ’the gold of wounds.’” He paused, and his voice was level: ”It must be clean honey — the city has many date trees; the households that gathered honey in the spring still have half a jar or so put by. Half a jar of honey will dress more wounds than a chest of anything else.”

Neht’s eyes lit up for a moment — honey, real honey, the one thing that could save the boy — and then dimmed: ”But in this city... I am the only one who can clean a wound, and my old eyes are going. The knife shakes in my hand some mornings.”

”From now on,” Imset said, ”there are us. The hospital stands; the wounds of this city will have someone to care for them.” He said it without raising his voice, and Neht heard in it the weight of a promise made to a city — the kind of promise that is kept with one’s own hands, wound by wound, until the last wound is healed.

In the first days of the hospital’s work, Neht came every day.

He did not sit; he stood outside the treatment bay and watched. Watched how Imset washed a wound, the water poured and poured again; watched how he used the knife, the blade moving without hurry; watched how he looked at a tongue, tilting the patient’s head to the light. Watching, the old man’s eyes grew hot, and the watching became a kind of apprenticeship. One day he asked: ”Priest, I have a stupid question — when you clean a wound, how do you know how deep to cut?”

”Look at the color of the dead flesh.” Imset said. ”Black — cut until you see red. Red — leave it. Red flesh is alive; black flesh is dead. The knife goes between the living and the dead; cut one stroke too deep and you damage the sinew, one stroke too shallow and you leave the evil behind. The wound teaches the knife; the knife only has to listen.” He held up two fingers. ”And always — clean the blade before, and clean it after. A dirty blade is a wound’s best friend.”

Neht nodded, and mouthed the words to himself like a schoolboy learning a lesson — ”black, cut to red; red, leave it; the knife between the living and the dead” — repeating them under his breath as he walked home. The next day he came again — and under his coat he carried a small bronze knife, polished bright, the blade worn thin by forty years of whetting.

”My master passed this to me.” He said. ”It has followed me forty years. From now on, I’ll assist you in the cleaning of wounds.”

Imset took the knife, looked at it — turning it in the lamplight, weighing it on his palm — and handed it back: ”A good knife. But your eyes are worth more than your knife — from now on, you stand the first post of triage. What kind of wound it is, how grave — one look from you is worth ten questions from anyone else.”

Neht was still for a moment — and then he straightened his back, the old spine coming up joint by joint. In that instant, this old pharmacist, bent by half a lifetime of the siege, looked twenty years younger, and the years came back into his eyes.

”Done!” He said. ”These old eyes of mine will stand the city’s first post.” He said it loudly, the way a man says a thing he means to keep, and from that day he stood at the gate of the treatment bay in the mornings, his eyes moving over every stretcher that came in.

The chief officiant came once as well. He did not treat anyone; he only stood in the open middle of the square and looked at the three-sided sheds. He looked for a long time, and then he said: ”The old temple had a pharmacy too. God-medicines and man-medicines, locked in dark rooms, never seeing the sun from one year’s end to the next. This hospital is better — the wounded lie in the light, and the work of saving is done in the light. Where there is light, there is no room for the furtive.”

”Sickness does not like the light either.” Imset said. ”Which is why it cannot escape the sun.”

The chief officiant nodded. As he was leaving, he set down a small packet of myrrh in the medicine bay, unwrapping the cloth to show the dark resin, the smell of it rising through the room: ”The last of the old temple pharmacy’s stores. It will save one more life here than in a dark room.” He said it without ceremony, and walked away, and the myrrh was entered in Shafi’s register that same hour, with the chief officiant’s name beside it.

That day, the hospital floor received its first layer of clean dry straw.

By evening, the three mud-brick walls stood to their full height. The beams were raised, the sailcloth stretched, the reed-mat partitions hung. Three rooms stood around an open yard — and in the middle of the yard, a wooden post had been set, and on the post hung a lamp. Imset said: this lamp burns through the night.

”The hospital’s lamp,” he said, ”is like the torches on the wall. While it burns, people are settled.”

At night, when the first lamp was lit, the edge of the square was lined with people.

They had come to see the light — the first new light the city had lit in weeks that was not a torch. No one spoke. The wind came off the river and moved the sailcloth roof in long, slow swells — like three great birds with folded wings, resting on the breast of this city, breathing softly. The people stood a long time, and then went home one by one, and the lamp burned on.

The hospital’s nights were organized into shifts, too.

The night watch ran three shifts, two men to a shift — one healer, one medicine boy. The healer kept the wards, listening to the wounded breathe, walking the rows in the lamplight; the medicine boy kept the medicine bay, the lamp on the racks, counting the chests once a double-hour, his fingers touching each lid.

”The night is the cruelest hour.” Imset told the night watch. ”A wounded man’s fever rises mostly at midnight. Feel his forehead once every half double-hour — it works better than any medicine at dawn.”

On the first night, Shafi stood the watch. She carried a half-filled oil lamp from one end of the ward to the other. The snoring, the grinding of teeth, the cries from dreams — she listened to them all. One wounded soldier cried out in his dream: ”Fire! Fire!” She went to him, tucked his hand under the blanket, and said, low: ”The fire is out. Sleep.”

The soldier turned over, and was quiet.

At dawn, Shafi wrote in the register: one night of peace; one fever down; two coughs eased. The lamp was trimmed and set for the day watch, and the hospital’s first full night was recorded in the same even hand as its first day’s work.

The hospital opened its doors on the third day.

The first thing was the rules. Imset had a wooden board made, whitewashed, with four large characters written on it:

”The critical first; the light after.”

The board was nailed at the door of the treatment bay, at the height of a man’s eyes, so that whoever carried a wounded man in passed under this board first — the four characters above the door like a lintel. Inside the door, Shafi stood behind the triage table with two medicine boys. The triage table was an old door plank on trestles; on it lay three things: a lamp, a register, and a box of whittled sticks — the sticks cut and smoothed the night before, the red ones and the yellow ones in separate compartments.

”The critical get a red stick; the light get a yellow stick.” Shafi told the carriers. ”Red sticks go straight into the left half of the ward. Yellow sticks wait in the open yard. Visible wounds — let Teacher Imset take one look first.” She spoke the way she kept her accounts: plainly, once, without repeating — and the carriers learned it the first time, because the wounded were in their arms.

On the first day, twenty-three were carried in. Twelve arrow wounds, five crush wounds, two burns — and four who were doubled over with dysentery, too weak to stand, their faces the color of old paper: in the days of the water shortage, some had drunk the leftover water of the aqueduct. The four lay in a row on the mats, and the smell of them told the story before any of them spoke.

”The aqueduct’s water must not be touched again.” Imset looked at those four, and his face darkened. ”The rebels threw dead sheep into the channel. A man who drinks that water will have his guts ruined. Add a clause to the notices at the four gates: all well water, boil before drinking.”

Lu Zhu posted the notice that same day. The man pasting it sighed as he worked the brush: ”First the grain bulletin, now the water order. This city’s walls are becoming rules pasted on paper.”

”Rules pasted on paper,” the chief officiant said beside him, ”are harder to break than a city taken by the blade.”

The first day was a day of disorder, too.

The stretcher-bearers did not know where to go, standing at the gate with their loads, looking left and right; the lightly wounded crowded beside the gravely wounded, the groans and the complaints mingling; the families in the yard stopped the medicine boys with questions, and the questions went unanswered. The queue at Shafi’s triage table knotted into a lump. A lightly wounded man, tired of waiting, pushed into the ward on his own — and collided with a stretcher carrying a gravely wounded man; both went down on the ground, the stretcher tipping, the wounded man crying out.

Imset came out of the treatment bay. He did not lose his temper — the temper had been spent, years ago, in the years of his disgrace, and what was left was a patience that had outlived it. He had the medicine boys help the men up; he had Shafi move the triage table back three paces, so that the queue had room to breathe; and he carried a wooden stool into the middle of the open yard and sat down on it, where every eye could see him.

”From today,” his voice was not loud, but the whole yard heard it, ”a wounded man entering this gate passes by me first. The critical — I give a red stick, and the medicine boys carry him straight to the left half of the ward. The light — a yellow stick, and they wait in the yard. Whoever walks out of line again sits beside me, and keeps me company for half a double-hour.”

The crowd quieted. The man who had pushed into the ward went red, and went to the edge of the yard and squatted there by himself, and no one laughed at him — the rule had been spoken, and the rule held.

The rules were established one by one, like that.

The flow was run the next day.

From the wall to the hospital, Imset led the stretcher crew over the route three times, walking it himself first, counting the paces, marking the places where the street narrowed and the stretcher had to tilt. The crew was twenty men, picked from the able-bodied and the boatmen — the boatmen among them moved like men who had carried loads on moving decks all their lives, their footing sure on the worn stones. He had them run the route carrying stretchers loaded with stones, the stones wrapped in cloth so they would not shift, while he stood at the square’s edge and counted.

”First run, from the south wall to the hospital, a fast walk — half a stick of incense.” He said. ”Too slow. A wounded man bleeds a little more with every step. Second run: change shoulders at the change points, pass through the gates at the pass points—” he waved a hand, ”again.”

The third run took less than half a stick of incense. Imset did not say ”good.” He said only: ”Tomorrow — ten breaths faster.”

”Teacher,” a young stretcher-bearer said, low, ”the stones in the stretcher don’t cry out.”

”The ones who cry out,” Imset said, ”are exactly why it must be fast.”

By the third day, the carriers, the medicine runners, and the waiting wounded each kept to their own paths, and none hindered another — the yard moving like a well-run landing, everyone knowing their lane. On the post that held the lamp in the yard, Shafi had tied three straw ropes — the red rope’s path ran to the left half of the ward; the yellow rope’s path to the right half; the middle lane stayed empty, for the urgent to pass without fear of collision. The ropes were tied at the height of a man’s chest, so that a carrier could see them without stopping.

”These ropes,” a boatman carrying a stretcher said, ”are like the channels on the river. Each boat in its own lane — and no one collides.”

The medicine accounts were turned in by Shafi on the third night.

She had begun keeping them on the boat, the register open on her knees in the dark hold. The twelve chests: wound medicines, so many packets; bleeding-cloth, so many rolls; splints, so many pairs; honey, so many jars; common herbs, so many bundles — each with its number after it, the numbers in her small even hand. In the city, she had added the remains of Neht’s pharmacy to the account, and a few entries more, each one checked twice. When the last entry was written, she drew a line under it, and above the line she wrote:

”At today’s rate of use: wound medicines, seventeen days; bleeding-cloth, twelve days.”

Neht’s pharmacy’s stores were heartbreakingly thin: half a bundle of dried mugwort, a jar of plant ash, a few handfuls of roots she could not fully name. Shafi weighed them, recorded them, one by one — and at the end, drew a small circle after Neht’s name.

”What is that mark?” Neht asked.

”Merit.” Shafi said. ”Half a bundle of mugwort has saved people in this city too. When the war is over, this register will go into the archives — and anyone who reads it will see at once whose stores were the foundation, in the days when medicine was thinnest.”

Neht said nothing. He turned away, and wiped his face with his sleeve. Whether it was sweat he wiped, or something else, no one asked — but the old pharmacist’s shoulders were a little straighter as he went back to his rack of herbs, and his hands worked a little more surely for the rest of the day.

Imset read the accounts, and added a line at the edge: ”The critical take first; the light take less; save where you can. Bleeding-cloth — when it runs short, tear up old clothes and boil them in salt water.”

”Can boiled old clothes be used as new?” Shafi asked.

”Boiled cloth is clean cloth.” Imset said. ”Clean matters more than new. The wound does not ask where the cloth was woven; it asks whether the cloth is clean.”

From that day, the hospital had one more rule: the lightly wounded were not bandaged — medicine only, and left to the air, the wounds breathing; the bandage strips that were used were boiled after each use and dried for the next, spread on the walls of the yard in the sun. No one felt it shabby — the smell of cloth boiled three times had become the most reassuring smell in the city, the smell of care made visible.

The grading of medicines was established, too.

The critical, with red sticks, drew wound medicines and bleeding-cloth without stint; the light, with yellow sticks, drew half measure. Food was medicine to carry the rest — barley porridge, date broth, rich soup boiled from fish bones, each one a medicine in its own right, the broth going down the ward in steaming bowls. Shafi folded Neht’s pharmacy into the hospital’s stores, and the old man was glad of the lighter work — he squatted before the medicine racks all day, sorting the herbs, cutting them, packing them into bundles, his hands working with the patience of forty years.

”I’ve looked at wounds for half my life,” Neht said to Shafi, ”and this is the first time I’ve seen medicines with shifts and ranks. With ranks, the medicines come alive.”

”Medicine isn’t medicine,” Shafi said. ”It is life. With ranks, life lasts a few days longer.”

The city’s herbs still had to be found.

Neht took the two medicine boys along the foot of the wall, walking slowly, his eyes moving over the ground — the habit of half a lifetime, seeing what others walked past. In the city’s wild patches grew things that looked like nothing much: wild garlic with its thin leaves, onion shoots in the corners of abandoned gardens, bitter wormwood in the dry places. The old man looked at each one carefully, and dug only what he was sure of, testing the leaves between his fingers before the spade went in.

”This wild garlic,” he said, ”pounded and laid on a wound, is stronger than plant ash. The old folks called it ’the knife of the fields’ — it kills the sickness outside, and does not kill the flesh.”

”We’ll take it.” Shafi said. ”As much as there is.”

”And the house gardens.” Neht said. ”Onions, garlic — whoever grows them, spare a few heads. They are medicine too.”

The word went out, and the garlic came in the same day. An old woman brought half a basket, the bulbs clean and firm, still smelling of garden earth: ”These were meant for pickling. But the hospital needs them first — the wounded come first. Garlic can be grown again; a man cannot.” Others came after her with onions and garlic, and the medicine bay filled with the sharp smell of the gardens of the city.

Sister Wei pounded the garlic into mash, working it in a stone bowl with a wooden pestle, the smell rising sharp in the yard; she wrapped it in clean cloth, and labeled each packet. The labels were charcoal on wood shavings, the writing not pretty, but careful — each character formed slowly, the way a person writes who means every word: garlic mash, for wounds, change twice a day.

The wounded’s accounts grew thicker, day by day.

But strange to say, the hospital’s days were not all gloom. The wounded crowded into the three rooms, reed mat to reed mat, and no one thought himself above the man next to him; the groans were answered with other groans, and the moans with small comforts. A soldier of the guard pushed half his bowl of barley porridge to the laborer beside him whose leg was broken: ”Your leg has to grow bone. Mine is only an arm — it can still move.”

The laborer pushed it back: ”You have to go back on the wall. I lie here; I don’t use much grain.”

They pushed it back and forth, and in the end they shared it, the bowl passing between them in the lamplight. An old woman nearby wiped her eyes, and gave the two soldiers the jar of date broth she had brought for her own, setting it down between them without a word.

”This city,” she said, ”is poor in medicine, but rich in heart.”

The hospital’s yard drew people who had no business there, too.

Children. A few half-grown boys of the south came running every day after their lessons to look at the lamp, standing at the square’s edge until dusk. Once, a child had woven a grasshopper out of straw — the weaving careful, the legs jointed — and slipped it under a wounded soldier’s pillow. When the soldier woke and saw it, he held it up to the light and looked at it for a long time, turning it in his fingers — and then he laughed. He said: my son can weave those too.

Old men. A white-bearded old man came every evening, and did not go in — he sat at the edge of the yard on a stone, and watched. Someone asked what he was watching. He said: ”I watch the lamp. While the lamp burns, I feel that we old bones, good for nothing, are still remembered by someone.” He came every evening after that, and the stone was warm where he had sat.

A water seller came every few days — A-Bu, with a bucket of fresh well water hanging at each end of his yoke, saying it was for washing the bandages. The buckets were full to the brim, the water still cold from the well, and he carried them the whole way without spilling a drop — the skill of thirty years on his shoulder. Shafi tried to pay him. He put on a stern face: ”I sell water to everyone — everyone but this hospital. This is a place where lives are traded for lives. These two buckets of mine are a little of my life, and it is freely given.”

The women came in shifts. Sister Wei had come into the city on Imset’s boat — she had been a medical hand on the western line, binding arrow wounds in the desert for three years, her fingers knowing the knots before her eyes confirmed them. When she heard the city’s hospital lacked hands, she asked General Horen for leave, packed two bundles, and boarded the boat. In the city she came twice a day — cooking porridge, carrying water, washing bandages, turning the reed mats, working without being asked, the way she had worked in the desert. Once a woman wept over the cloth she was scrubbing; the others tried to comfort her, and she said, through her tears: ”I’m not crying for myself. I’m looking at them, and remembering my brother — if he could have waited for a hospital like this...”

No one answered. After a long moment, Sister Wei said: ”In the old days, wounds had no one to care for them. From now on, they all will. Every strip of cloth we scrub is a debt repaid for the ones who didn’t wait long enough.”

A-Ka’s wound closed on the fifth day.

When Imset cleaned it, A-Ka bit down on a wooden stick and did not make a sound — the stick was gouged deep with his teeth by the time the work was done. The dead flesh cut away filled half a bowl, dark and rank. Salt water, three washings; honey laid on, the jar tilted over the wound; oilcloth over. For the first two days he burned and talked nonsense — shouting ”wall,” ”crenellation,” ”the arrows are coming,” his arms moving as if he were still on the wall. On the third day the fever broke, and he opened his eyes on the sailcloth roof, and his first words were:

”Is the wall... finished?”

”Finished.” The medicine boy tending him said, dipping a cloth in water and pressing it to A-Ka’s forehead. ”While you lay here, the north wall’s crenellations were all made up. The wall is whole, and so are you.”

A-Ka grinned — and the grin pulled at the wound, and he drew in a sharp breath: ”Good. Then this arrow didn’t cost me nothing.”

When Imset came to change the dressing, A-Ka said: ”Priest — this arm of mine, will it hold a chisel again?”

”It will.” Imset said. ”The wound is in the shoulder, not the hand. Healed, the chisel is yours again. Only — for the first half year, don’t fight anyone for the heavy work.”

”I won’t.” A-Ka said. ”So long as I can hold a chisel, I’ll be a laborer for life, and be glad of it. The chisel and I know each other’s weight.”

On the fifth day of the hospital, Lu Zhu came.

He walked a full circle around the three rooms, looking at the foundations, the drainage, the raised floor of the medicine bay — his eyes moving over the work the way they moved over a wall, reading it. He tested the wall-foot with his hand, pressed the medicine bay’s floor with his heel. When he had finished, he stood a while in the yard, and said to Imset: ”This hospital is cast from the same mold as our healing hall. But there is one place more vital here than there.”

”Which place?”

”The drainage ditch.” Lu Zhu said. ”The city’s water is tight — but the hospital’s water cannot be spared. The water that washes wounds, the water that boils cloth — where it goes when it is done, you have thought it all through. That one ditch is this hospital’s life.”

”Thought through.” Imset said. ”The ash pit at the yard’s east corner burns the soiled cloth; the seepage trench outside the south wall carries the wash water. Two water-roads, and they never mix.”

”Then it’s well.” Lu Zhu said. ”The wall stops arrows; the granary stops hunger; the hospital stops death. These three places are the city’s three breaths — and I have seen all three drawing air this week.”

”Three breaths,” Imset repeated, ”and not one may be cut.”

The two men stood in the yard, looking at the lamp that burned even by day. The flame was very small, swaying in the wind — but it did not go out.

On the seventh night, something happened.

Not in the hospital — beyond the wall. The rebels had changed their way of war.

The new way was Panhesy’s. His scouts reported: the new capital’s medical team had entered the city, raised three sheds in the square, and carried men into them all day — the stretchers going in and out without stopping, the lamp burning through the night, the smoke of cookfires rising behind the sheds. Panhesy stood before the sand table for a long time, tracing the square’s position with his finger, his thumb pressing the sand at the city’s center; then he went to Nekhem’s tent for council.

The man in white told the story of the medical team in more detail, his voice even, the facts laid out like goods on a counter. Among those who had entered the city was Imset — the priest who had been wronged in Thebes all those years ago, his family’s name broken by men of the old temple. He had come with his girl apprentice, with twelve chests of medicine, by the West Bank’s water road, through the west gate in the small hours — the same gate the city had been won through, in the same dark.

”Imset.” Nekhem repeated the name. ”His whole family was disgraced in those years — by men of my old temple. Now he comes back, raises his sheds in the square, and the people he saves are the same people of this city.” He said it without heat, the way a man notes an irony that does not move him. His fingertip found the emblem under his sleeve and began its slow circle. ”The past has a way of walking back through the gates it left by.”

”Can this man be spared?” Panhesy asked.

”Spared or not — it depends on the city.” Nekhem said. ”When the city is taken back, the men he has saved are still my men. A saved man is a man who owes his life — and life-debts can be collected, when the city is ours again. What matters now is not one man. It is their medicines.”

”High Priest — the city’s wounded have someone to treat them now. Treat one, and that is one more man on the wall, one more arm for the ramparts, one more pair of eyes on the night watch. Our arrows have been spent for nothing.”

”So?” Nekhem asked. The two characters hung in the tent, and Panhesy heard in them that his general was not convinced yet.

”Fire arrows at night.” Panhesy said. ”Not to break the wall — to wound the men. What the fire does not kill, it burns; a burned man is a man taken out of the fight, and a burned man’s brother is a man who hesitates on the wall. For every one they save, we wound three. We’ll see which runs out first — their medicines, or our arrows.”

Nekhem listened, and was silent for a long time. His fingertip rubbed the emblem, circle after circle, the old worn motion of his thinking. The tent was still; the lamp flame leaned and steadied.

”Their medicines,” he said, slowly, ”are limited. So are their hearts. When the wounded mount and the medicines fall, the hearts will waver — and a wavering heart is a gate half open. This way will serve.” He raised his eyes. ”But one rule — burn freely, but do not burn the city. Thebes, taken back, must still be lived in. The wall must not be ruined. A city won and burned is a city lost twice.”

”Your general understands.” Panhesy said.

The fire arrows were made on the spot. The cloth wound around the shafts was torn from old robes, ripped into strips and wound tight, each strip tested by a tug before it was wound. The oil they were dipped in was drawn from the camp’s oil jars, the jars tipped and set back, one by one, the level in each falling a finger’s width. The men who made them squatted behind the tents, wrapping and dipping, their hands quick and practiced, the finished arrows laid in rows to dry, the rows growing long in the firelight. Someone said: this oil costs money, burning on the arrows. The centurion making them did not raise his head: ”Spend the oil, save the lives. The trade is a fair one.”

At the third watch, a roar of battle cries rose from the south. The drums on the wall beat fast, the sound running along the wall-head, and the torches flared as the watchmen ran to their posts. This time the rebels did not charge the ditch, and they did not fill the trench. They stood off, out of arrow range of the wall, and loosed their shafts at the wall-head: the arrows wrapped in oil-soaked cloth, catching flame as they flew, trailing fire through the dark like falling stars. Each arrow struck the wall and left a line of fire running along the stone, the flame licking up the face. The defenders beat at the flames with cloths and earth, their hands blackened; the second wave came while they were beating at the first, the arrows hissing over their heads, one striking a torch and bursting into a shower of sparks. Fire and arrows by turns, the night rang with it for half the night, the wall-head flickering like a hearth, the smoke rolling over the parapet.

Near dawn the rebels withdrew. The wall-head had a dozen-odd new wounded — all arrow wounds and burns, the cloth of their clothes scorched, the skin red and blistered, the burns weeping under the broken skin. The stretchers carried them into the hospital with the blood darkening the planks, and the hospital’s lamp was still burning when the first of them came through the door, the light falling on their faces.

Shafi stood behind the triage table, and her tagging hand did not shake. She tagged and counted as she worked, her voice level, the sticks going in as the stretchers came past: ”Seven red. Six yellow... Wound medicines — three more packets.” The medicine boys moved without being told, carrying the red-tagged to the left half of the ward, and the ward filled, the mats filling row by row.

A-Ka reached the hospital door at the sound of the second wave of arrows.

He wore an old coat, thrown over his shoulders, his left arm still in its sling, the bandage white against the dark cloth. When the stretchers came past, he moved to lend a hand — and Sister Wei caught him by the arm: ”Your shoulder isn’t healed. What are you doing carrying stretchers!”

”I can pass water with one hand.” A-Ka said. ”Let me stand here. Standing here, my heart is settled.” He said it simply, and Sister Wei heard the truth in it.

Sister Wei did not stop him again. All that night, A-Ka passed water, bowl by bowl, with his one good hand — going from stretcher to stretcher, holding the bowl while the wounded drank, taking the empty bowls back, his sling catching the lamplight each time he bent. By dawn his right arm was too sore to lift — but his eyes were still bright, and the bowls had never once stopped.

”The stores...” A medicine boy began, and stopped, his hand on the rack, his voice trailing off at the sight of the nearly empty shelves.

”Take them.” Shafi said. ”Tomorrow’s problem is tomorrow’s.” The medicine boy’s hand came off the rack, and the last packets went into the ward on the stretcher trays, and no one in the hospital said out loud what the register would say at dawn.

The light came. The hospital’s lamp had lived through another night — the flame small, the oil low, but burning. The lamp had become the city’s habit: a man passing the square at any hour looked for it, and found it, and went on.

Shafi sat in the medicine bay, going over the accounts again. The register lay open; every entry in and out, she passed her eye over once more, the charcoal in her hand. When she reached the line for ”wound medicines,” her brush stopped.

Behind those characters stood a number — much smaller than the number of seven days ago. The difference between the two numbers was the shape of the night’s work: twelve arrow wounds, two burns, the dressings changed, the packs opened.

She raised her head and looked out through the medicine bay’s doorway. In the yard, on the post, the lamp still burned. The daylight had come in, and the lamp’s light looked thin beside it — a small flame holding its own against the morning.

”Teacher,” she said, low, ”the medicines are nearly gone.”

Imset came over from the treatment bay and stood behind her, looking at the number. He did not touch the register; he only looked at it the way he looked at a wound — long, and without flinching.

”I know.” He said. ”Before the medicines run dry, someone will bring more.” He said it the way he said everything — with the certainty of a man who had waited out worse things than a shortage, and seen the boats come anyway.

”Who?”

”Meru.” Imset said. ”He promised. The caravan roads have always been roads that get walked — a road is not a road until it has been walked on, and Meru has never once left a road unwalked.”

Shafi said nothing. She closed the register, and re-pasted the slip that read ”the critical first; the light take less,” pressing its corners flat.

And in the works south of the city, Horenheb had seen the fire on the wall-head too. He stood behind the earth wall, watching the flames beaten out one by one, watching the figures on the wall-head — counting them in the firelight, and not one missing. He counted twice, the way he counted his own men after a fight, and the second count matched the first. The fire had not taken a single man of the wall.

”Pass the order.” He said. ”From tomorrow, the roving riders go out to hunt their archers. They loose fire every night; we wait for them every night. A line of archers that cannot draw in peace is a line of archers that will miss.”

An old centurion of the western line answered. He had stood the line for thirty years, his face carved by the desert wind, and he touched the blade at his belt, his thumb finding the edge: ”General, on the western line we guarded the border — when the wolves came, we fought. Here at the south, it’s the wolves circling, and we keep the watch. One principle — we must not let them catch their breath. A wolf that catches its breath is a wolf that plans.”

”Let them catch their breath,” Horenheb said, ”and they will think of something else. And when they think of something else, it will be more than fire. Fire is only the first idea of a tired enemy.”

He looked toward the wall-head. The fires were out; only the lamp in the direction of the hospital still burned, a small light in the gray dawn, like a star that refused to sleep.

Under that star, Shafi went through the medicine chests again. The chests were still twelve, numbered one to twelve, not one missing; the numbers on the lids were still legible, written in her own hand. She opened each one, looked at what was inside, closed it again — the motion of a habit that would not let her trust a number she had not seen. But when her hand passed over the rope knots of each chest, it was a little slower than before — she knew that what the chests were missing, and what the register was missing, were the same thing, measured in the same small numbers.

”Wait for the boat,” she said, low, to herself. ”Wait for Meru’s boat.” The words were a habit now, said to the dark of the medicine bay, and the habit was the only thing between her and the number in the register.

On the river west of the city, just past the Zi hour, A-Tu’s small boat was coming back along the reed beds. The lamp at its bow was masked with black cloth, letting through only a thread of light; the thread lay on the water, drawn out long and thin — like a line paid out into the dark, one end tied to the city, the other to the night beyond the river. The boat moved without a sound, and the city slept on its bank, and the thread of light was the only thing awake.

(End of Chapter 6)
