System Came When the Doctor was Thirty

Lu Cheng is a newcomer, a rookie unknown even to Dai Jinzhong.

His abilities are not weak.

As the on-duty internist in the emergency room, Dai Jinzhong certainly doesn’t have to personally escort the patient.

Liang Jianhong has already set off with Lu Cheng, leading the team, with the nursing team carrying portable oxygen pillows and moving the monitors onto the push bed.

The group hurriedly wheeled the bed towards the CT room outside the emergency room.

This type of emergency belongs to the super emergency category; the medical team has the right to jump the queue at the emergency CT room, only waiting for the patients inside to finish their examination.

...

After the group left, Dai Jinzhong thought for a moment and asked, ”Director Xiao, this Lu Cheng was just selected from surgery, right? Seems very clever and agile.”

Xiao Zhaoxi nodded: ”Yes, he just joined the department today.”

”We’ve picked up a gem, though we haven’t completely assessed his capabilities yet. It’s clear, however, that he is above the average level for his age and rank.”

Lu Cheng is not the protagonist.

Even though Xiao Zhaoxi knows Mu Leng, Lu Cheng is regarded as a junior, and Xiao Zhaoxi’s attention on Lu Cheng is still limited.

He only knows that Lu Cheng has won an award in a microsurgery skills competition, though he hasn’t paid attention to the exact level of the award.

Microsurgery is microsurgery; emergency medicine is emergency medicine, just icing on the cake.

Of course, capable people are easily noticed for their outstanding qualities.

Lu Cheng’s brilliance is being revealed bit by bit.

It’s mainly Guo Lili from internal medicine who is paying more attention to Lu Cheng: ”Director Dai, this Lu Cheng ranked first in the surgical written exam, and it seems his comprehensive scores ranked first too.”

”Definitely a top student.”

Dai Jinzhong said: ”Now that we are working, such titles as ’top student’ are no longer appropriate. In our clinical work, those with exceptional skills should be called skilled workers, mature workers, team leaders, bosses.”

”We all rely on Director Xiao for a living.”

Guo Lili looked puzzled at Dai Jinzhong, not quite understanding his words, as Dai Jinzhong was about to say more.

Xiao Zhaoxi said: ”Let’s not mention the past grievances, I’ve said to let go of the internal medicine side, and everything will be handed over to Director Wu from now on. Don’t expect me to clean up your mess anymore.”

”I don’t have that much energy.”

After the mutual compliments, Xiao Zhaoxi thought to ask Lu Cheng: ”Xiao Lu, how did you determine that the damaged blood vessel was the splenic artery?”

Lu Cheng shook his head: ”Abdominal blunt trauma is a type of non-open injury, with a history of trauma, it’s unlikely to be a dissection or artery aneurysm rupture.”

”The probability of organ rupture, specifically the spleen, is larger.”

”The cause of shock due to delayed splenic rupture is relatively more common.”

Xiao Zhaoxi nodded slowly: ”Your thinking is quick; your clinical reasoning is clear!”

”What about shock without a history of trauma?”

Lu Cheng could tell that Xiao Zhaoxi was testing him: ”Unexplained shock of a non-traumatic nature requires consideration of aneurysm rupture or arterial dissection, cardiac perforation, etc.”

”We’ll have to wait for a consultation on this.”

This is currently beyond Lu Cheng’s ability range.

Cardiac surgery and vascular surgery emergency operations aren’t something Lu Cheng can handle all at once.

”He’s a skilled hand. Congratulations, Director Xiao,” Dai Jinzhong said straightforwardly to Xiao Zhaoxi.

But Xiao Zhaoxi shook his head: ”In our emergency department, knowing too much isn’t always a good thing; it can lead to trouble and disputes.”

Ensuring there were no patient relatives around, and with the internist patients in the rescue area already transferred to ICU, Xiao Zhaoxi said, ”Sometimes, doing our part incurs no risk, but doing things beyond our part might cause trouble.”

”I’ve heard this saying from a professor in Xiangya Hospital’s emergency department, that emergency doctors should only show about thirty percent of their capabilities.”

”Not seeking achievements, just avoiding errors.”

”What I actually recommend is that Xiao Lu and Liang Jianhong do well in the exploratory laparotomy and then wait for specialist doctors to arrive.”

Xiao Zhaoxi’s words, half advisory and half instructive, are of real significance.

Dai Jinzhong didn’t respond, instead, he said: ”Director Xiao, let’s go check the CT together.”

Once the CT examination is done, it can be accessed in the viewing system; there’s just no imaging report yet.

When a few people turned to the computer, the CT images were already available.

Xiao Zhaoxi looked at them briefly, not finding a dissection or other malignant emergency condition, then relaxed: ”It is indeed more of a pure splenic rupture.”

”Let the people from Hepatobiliary Surgery handle the resection later.”

Lu Cheng bit his lip but did not mention attempting to save the spleen.

What Xiao Zhaoxi just said was a suggestion.

Do more, err more; do less, err less; do nothing, no error.

Clinical practice is a big community with established rules of operation, and Lu Cheng is far from the point where he can defy rules and make rules yield to him; there’s no need to complicate matters.

This patient surviving and following the main emergency measures for treatment is already fortunate.

An emergency laparotomy for hemostasis is a crucial intervention, regardless of how well Lu Cheng performs, there is no responsibility.

After all, doing nothing means death.

The people from Hepatobiliary Surgery and General Surgery have arrived, and soon the patient was taken into the operating room by Hepatobiliary Surgery.

Deng Duanniu appropriately provided the signed documents:

”Director Xiao, the patient’s family has signed, the fees have been initially paid, though it’s just ten thousand for now.”

”It’s enough for now, the remaining costs can be settled later.”

”Liang Jianhong, write up the emergency procedure rescue record, including the laparotomy operation and indications in the emergency room.”

Liang Jianhong nodded: ”Alright!~”
