System Came When the Doctor was Thirty

Opposite Lu Cheng, Liang Jianhong didn’t respond, still in bewilderment.

Lu Cheng withdrew his hand from the patient’s abdomen: ”Dr. Liang, I think we can’t clean the hematoma on site; we should proceed to the operating room. What do you think?”

Lu Cheng asked Liang Jianhong again.

Fortunately, Lu Cheng had previously worked under Liang Jianhong, so Liang Jianhong knew Lu Cheng’s ”intimidating” reputation, and his thoughts were interrupted by Lu Cheng’s words: ”Oh, okay!”

Liang Jianhong didn’t even dare to open up Lu Cheng’s abdominal incision casually, because such emergency hemostasis was not something he had considered taking on in recent years.

Most people in the department, during shifts, usually first thought of calling for help rather than daring to take action easily.

So, even if Lu Cheng didn’t do well enough, Liang Jianhong still couldn’t cover for him.

In Liang Jianhong’s initial assessment, the mortality rate of this patient was over eighty percent, and all current rescue measures were palliative.

This is also the main reason why Deng Duanniu went to speak with the patient’s family immediately.

Director Dai Jinzhong from the internal medicine department finally completed a localized auscultation: ”Currently, the cardiopulmonary function is okay, the main issue is arterial bleeding.”

Dai Jinzhong is the deputy chief physician in the department, even though he is from the internal medicine group, he is also Liang Jianhong’s superior: ”Director Dai, we’ve already called the vascular surgery and general surgery departments.”

”The airway is clear,” reported Guo Lili, who was temporarily on duty.

Dai Jinzhong also didn’t have a good solution for now: ”Continue to monitor blood oxygen, notify the blood bank to bring the blood quickly.”

”Is the blood type available?”

”Not yet! Blood pressure is very low, can’t draw blood,” a nurse answered.

”Even if you can’t draw it, find a way quickly,” Dai Jinzhong hung the stethoscope around his neck.

The most important vital signs for an emergency patient are blood pressure, heart rate, blood oxygen, and respiration.

The patient’s cardiopulmonary function is fine; currently, the main issue is hypovolemic shock and low blood pressure.

Blood pressure medication is one aspect; blood volume must be replenished quickly.

The airway is fine, now the main concern is arrhythmia due to low blood pressure, and the root cause is still blood volume.

Slightly uneasy, Dai Jinzhong glanced at the patient’s abdomen; the incision made by Lu Cheng wasn’t large, and the view was not very clear, so all Dai Jinzhong could see were the tails of two hemostats and the hematoma inside the abdomen.

Whether the hemostasis was effective or standard, he had no way to verify.

”The intravenous access is established,” an experienced nurse reported.

”Immediately expand blood volume. Continue establishing other intravenous access; we’ll need dual access for transfusion later,” Dai Jinzhong continued to order.

”This patient’s blood pressure is low, making it a bit hard to establish venous access,” the nurse was not very willing.

”Even if it’s hard, you have to find a way; otherwise, we can’t afford the delay!”

As Dai Jinzhong spoke, Xiao Zhaoxi hurried in, already putting on gloves quickly: ”What’s the situation?”

Liang Jianhong replied: ”Active intra-abdominal hematoma, already in shock.”

”We urgently opened an abdominal incision. Xiao Lu attempted instrument hemostasis immediately, but the effect is hard to assess at this time.”

Hearing this, Xiao Zhaoxi nodded, glanced at Lu Cheng, and then immediately leaned closer to the patient.

Lu Cheng daring to take a gamble was certainly better than leaving the abdominal incision unattended.

This is an intra-abdominal hematoma, and exploratory laparotomy is necessary. An intra-abdominal hematoma is not like a pelvic hematoma; it cannot be opened rashly, or it will worsen the bleeding.

With an intra-abdominal hematoma, even if not opened, the patient could still die from shock.

Xiao Zhaoxi didn’t have time to put on the protective gear, just wearing his white coat, he rushed in.

Since it was the emergency room, negative pressure suction was available, although it’s for sputum suction and not entirely sterile.

But now, if the bleeding point cannot be found, the patient could die from shock, so Xiao Zhaoxi didn’t care about the extreme aseptic principles.

Picking up the negative pressure suction, he began to clear the hematoma.

About thirty seconds later, Xiao Zhaoxi could barely see the situation inside the abdomen clearly.

After the remaining blood was cleared, no more blood spurted out. There might be oozing, but the amount of bleeding from oozing was almost negligible compared to this level of blood loss; this requires delicate hemostasis in the operating room.

In other words, the hemostatic operations that should be completed in the emergency room had already been resolved by Lu Cheng before I arrived.

This was completely different from the tag of ”personal connection” that Xiao Zhaoxi had placed on Lu Cheng.

Lu Cheng’s ”connection” source was himself, it was Mu Leng who mentioned it to him, so Xiao Zhaoxi’s expectation of Lu Cheng’s ability was not excessive.

Lu Cheng didn’t follow the general rules of being a ”connection,” he was a bit excessively capable.

”Call and continue urging the consultation, send the patient to the CT room first,” Xiao Zhaoxi ordered.

Dai Jinzhong hesitated a bit: ”Director Xiao, this patient has severe blood loss.”

”Severe blood loss needs prompt transfusion and blood volume expansion after hemostasis. The hemostatic operation is complete; transferring by stretcher won’t increase risks.”

”Next, we need an accurate diagnosis. If there are data to aid in diagnosing, it must be done; didn’t I always tell you this?” Xiao Zhaoxi’s voice was stern.

”Yes.” Dai Jinzhong lowered his head, no longer making suggestions.

But his gaze shifted to Lu Cheng.

Although Xiao Zhaoxi is the director and somewhat domineering, he is not unreasonable; his strong stance must have valid reasons.

The core basis was that the hemostasis was too fast and too effective, so effective that Xiao Zhaoxi had no chance to spot or fill any gaps.
