System Came When the Doctor was Thirty

When Deng Duanniu was feeling a bit thirsty, Lu Cheng said, ”Brother Deng, hold on a bit, the delivery I ordered just arrived.”

Deng Duanniu was puzzled: ”Xiao Lu, you’re really quick-witted, aren’t you?”

”How much was it? I’ll transfer the money to you.”

”Brother Deng, it wasn’t much, less than a billion.” Lu Cheng joked as he answered his phone, turning his head, and then walked out of the doctor’s office.

Deng Duanniu: ”???”

When Lu Cheng returned, Deng Duanniu was no longer in the doctor’s office. After asking a duty teacher from the internal medicine group, Lu Cheng found out that Deng Duanniu went to perform a debridement.

The debridement procedure is usually routinely handled by the duty doctor in the emergency room. However, if the emergency room is dealing with a surgical emergency, then the duty doctor on the surgical observation ward will step in. If the ward duty doctor is busy, a call is made for backup, or the patient must wait.

Patients entering the debridement room are not particularly urgent.

When the internal medicine group is on duty, Yuan Xuela, the attending beauty, naturally wouldn’t make such a faux pas as ordering only two coffees when there are three people in the office.

”Sister Yuan, what flavor would you like? I’ve ordered three flavors: a velvet latte, a mascarpone, and a fresh coconut latte.” Lu Cheng politely let Yuan Xuela choose first.

”Dr. Lu, do I get one too?” Yuan Xuela was writing medical records, and she writes long records.

”Thanks, how old are you?” After taking a fresh coconut latte, Yuan Xuela turned and smiled at Lu Cheng.

”I just turned thirty not long ago, am I calling you Sister Yuan wrong? Then should I call you Teacher Yuan?”

”I knew it, Teacher Yuan, you don’t seem like you should be called Sister.” Lu Cheng laughed and said.

”Dr. Lu, you sure know how to talk—I’m already thirty-seven.” Yuan Xuela replied with a smile.

Just as they were chatting, Guo Lili, who joined the emergency internal medicine group with Lu Cheng, came in from outside: ”Sister Xuela, Director Long asked you to go over.”

Yuan Xuela asked unhurriedly: ”What’s up? Did Director Long want me to write medical records?”

Unlike in Long County People’s Hospital which has only one associate chief physician, the emergency internal medicine at State People’s Hospital has five associate chief physicians. Long Chunbo is just one of them.

”No, Sister Xuela, there’s someone over there who needs a chest compression.” Guo Lili said.

Upon hearing this, Yuan Xuela immediately stood up, became tense, and quickly ran toward the emergency room.

Guo Lili also left following her, turning back specifically to look at Lu Cheng, who returned the gesture with a smile.

Quickly, Deng Duanniu returned, swearing: ”Damn it, I thought it was some huge cut that made the nurse rush me.”

”If he comes any later, the wound would’ve healed.”

”No stitches needed?” Lu Cheng, having worked in the emergency department before, knew of similar patients.

Deng Duanniu gestured his pinkie finger: ”Just this long, this deep. I told him to go out, turn right, and buy a band-aid.”

”Brother Deng, Sister Xuela took the fresh coconut latte. You pick one.” Lu Cheng pitched the drink to Deng Duanniu.

”Anything works for me.”

Deng Duanniu said: ”Xiao Lu, if I was as sensible as you back then, I could have avoided many detours.”

Lu Cheng laughed and said: ”Brother Deng, I’ve taken detours too, it’s just drinks, let’s enjoy them together.”

”By the way, Brother Deng.”

Just as Lu Cheng spoke, hurried voices came from outside the doctors’ office, it was the voice of Liang Jianhong on duty in the emergency room: ”Quick, call Director Xiao!”

”Request consultation with general surgery and orthopedics, the patient has massive intra-abdominal bleeding, call for an emergency ultrasound consultation.” Liang Jianhong’s voice was loud and piercing.

Deng Duanniu hurriedly put down the coffee he had just picked up!

Calling Lu Cheng: ”Let’s go, check it out.”

Lu Cheng immediately put on a mask and followed behind Deng Duanniu.

As they stepped out the door, they saw a thin, middle-aged man lying on a stretcher at the entrance, with his abdomen resembling a frog.

This is either ascites or massive intra-abdominal bleeding.

Liang Jianhong was directing the doctor and nurse pushing the stretcher, along with the family members, toward the emergency room.

The family member was the patient’s wife, almost panicked to the brink: ”How is my husband?”

”Don’t block, he’s already in shock.”

”How did he get injured?” Liang Jianhong pushed the woman aside.

”It was just a bike accident, he thought it was nothing and rested at home. Then I couldn’t wake him for breakfast, and immediately called 120.”

”His belly didn’t bulge like this initially! Why didn’t you deal with it?” The woman said.

”With our minds? Move, we need to get into the emergency room, go pay and register.” Liang Jianhong questioned sharply.

Lu Cheng and Deng Duanniu quickly stepped forward to help.

Being experienced, Deng Duanniu touched the patient’s abdomen and said: ”Massive intra-abdominal bleeding, peritoneal irritation sign, must be taken to the operating room immediately.”

The nurse brought bad news: ”Blood pressure is around seventy, 74/55!”

”Open IV access immediately, heart monitor.”

”Heart rate is fast.”

Emergency room duty internal medicine doctor, Dai Jinzhong, approached, quickly said: ”Place a vein catheter, quickly push norepinephrine…”

”No time, we may need immediate bedside hemostasis. Has Director Xiao arrived?” Liang Jianhong asked.

”Director Xiao said he’s coming, he’s in an ICU consultation.” A nurse said.

Deng Duanniu said: ”Open the pack, let’s assist with the laparotomy first.”

”I’ll go get the family’s consent. Liang Jianhong, you take Lu Cheng, and get going.”

As soon as he heard Lu Cheng’s name, Liang Jianhong was stunned, but suddenly realized as he saw Lu Cheng already had gloves on, and after pouring iodophor on the patient’s abdomen, he picked up the scalpel handle and blade from the debridement pack.

Lu Cheng looked at Liang Jianhong with a standard pen-hold grip and carefully asked: ”Mr. Liang, shall we proceed?”

”Proceed!~” Lu Cheng’s swift actions somewhat froze Liang Jianhong’s thoughts, yet he still agreed.

Lu Cheng immediately lowered his head, started making an oblique incision about five centimeters long, then cut with no hesitation.

By the time Liang Jianhong managed to respond and assist, Lu Cheng had already cut into the muscle layer.

When Liang Jianhong reached out his hand, Lu Cheng had already broken through the muscle layer and had opened the peritoneum.

A large amount of blood pooled inside the abdomen, but since it wasn’t oxidized, it didn’t bear a black mark.

In a flash, while Liang Jianhong was searching for the potential bleeding point, Lu Cheng had already extended two hemostatic clamps inside and cleanly and directly clamped the splenic artery.

”No visible damage to the lower abdomen, delayed rupture and bleeding of the spleen, I’ve clamped it.”

”First open the IV access, immediately send to the operating room.” Lu Cheng quickly reported.

This whole process happened quickly—from the agreement to the operation completion, it took just a little over one minute.

Lu Cheng had already completed incision and hemostasis!

The nurse had been responsible for the IV access, not bothering, while deputy director Dai Jinzhong from the medical internal group unexpectedly glanced at Lu Cheng but continued to auscultate the patient.

Without Lu Cheng’s operational intervention, his auscultation could be more detailed.

Xiao Zhaoxi hadn’t arrived, and Deng Duanniu hadn’t returned either…
