Chapter 116: Chapter 114: Diagnosis Confirmed! Impressing 2 Professors

When they heard Dr. Yan Xi’s examination results, the two professors in the meeting room were both surprised and impressed.

Shen Guofeng even gave Wu Ting a thumbs-up. ”Impressive!”

A smile appeared on Ma Mengchao’s face as well. The more brilliantly Wu Ting performed, the more it proved he was worth mentoring.

Shen Xiaofei was also pleased. Confirming the patient had early-stage cholangiocarcinoma provided a clear direction for treatment and prevented a misdiagnosis.

”We really have Dr. Wu to thank this time. Otherwise, we old-timers would’ve been thoroughly embarrassed.”

Shen Guofeng and Ma Mengchao felt a surge of emotion.

”The younger generation’s minds are certainly more flexible and agile.”

”Now that we’ve found the root of the patient’s condition, the next step is to design a surgical plan.”

Professor Ma Mengchao was very experienced in treating this type of cholangiocarcinoma.

”For obstructive jaundice caused by cholangiocarcinoma, the ideal treatment is PTCD, or percutaneous transhepatic cholangiodrainage. We use it to drain the bile, which in turn improves the patient’s liver function.”

”However, this patient’s case is particularly unusual. They’ve already developed a gallbladder perforation, which has led to a liver abscess. A standard drainage procedure won’t be enough to solve the problem.”

”If we can’t relieve the obstruction, achieve complete drainage, and control the bile excretion before performing a radical resection of the cholangiocarcinoma, we risk damaging the liver. If it’s not handled well, the patient might even need a Liver Transplantation. That’s an unacceptable outcome for both the surgeons and the patient.”

After laying out his analysis, Professor Ma began considering a new approach.

”The best option would be an auxiliary measure to complement the drainage procedure—something gentler that can continuously resolve this obstructive jaundice!”

”Could we possibly use a National Medicine regimen to manage it?”

Professor Ma didn’t know much about National Medicine himself, but that didn’t stop him from proposing the idea of integrating it with modern medicine.

Professor Ma was very open-minded.

Shen Guofeng’s eyes lit up as well. It was an excellent idea.

”Speaking of integrating National Medicine with modern medicine, Dr. Wu is an expert in that area. His integrated Tracheotomy and the recent integrated laparoscopic cholecystectomy were both astounding. I wonder, Dr. Wu, do you have any thoughts?”

Since this was a consultation where they were all brainstorming, Shen Guofeng turned to ask for Wu Ting’s opinion.

Director Shen Xiaofei also looked toward Wu Ting, his interest in the young doctor piqued.

Dr. Yan Xi put down her pen, pausing her meeting notes to look at Wu Ting.

Dr. Ling Ling also turned her head toward Wu Ting, a hint of envy mixed with her admiration. After his diagnosis, Wu Ting had clearly earned the right to discuss matters on equal footing with the two professors.

Unlike herself and Yan Xi, who could only listen attentively and take meticulous notes.

Wu Ting met their gazes with a smile, his voice calm and composed as he spoke.

”That’s an excellent point, Professor Ma. I also believe that integrating National Medicine with modern medicine is a very promising direction. I’ve actually done some research and have experience with certain decoctions in the field of National Medicine.”

”Previously, while jointly researching the use of qi’s ascending and descending mechanisms in laparoscopic cholecystectomy, I had a long talk with Director Huang Hong of our hospital’s National Medical Department and learned about many relevant decoctions from her.”

”One of them, Yinchen Lidan Soup, can effectively treat obstructive jaundice. However, it’s not widely known because it’s complex to prepare and expensive. It’s quite obscure, so even many National Medicine practitioners are unfamiliar with it, let alone Western-style doctors.”

”The Inner Scripture states, ’When dampness and heat intermingle, jaundice will afflict the people.’ This is because an external contraction of damp-heat toxins causes damp turbidity to stagnate in the body. The functions of the spleen, stomach, liver, and gallbladder become dysregulated, ultimately causing bile to stray from its proper path and overflow into the blood.”

”Therefore, National Medicine’s approach to treating jaundice involves therapies that invigorate blood and dispel stasis. The core principle is that if blood circulates freely, jaundice will easily dissipate. This treatment philosophy centers on resolving blood stasis and phlegm-toxins.”

”Yinchen Lidan Soup is based on this very principle. By promoting diuresis to reduce jaundice, clearing heat and detoxifying, and regulating qi to resolve stasis, it also has a significant therapeutic effect on cholangiocarcinoma.”

Wu Ting carefully explained the principles to the two professors and Director Shen Xiaofei. Then, he began to write down the formula for Yinchen Lidan Soup, describing the decoction in the simplest possible terms:

”The main ingredients are 30 g of capillary artemisia; 15 g each of gardenia fruit, Curcuma, red sage, sophora root, polyporus mushroom, Half-branch Lotus, and spreading hedyotis; 12 g each of bupleurum root and turmeric tuber; and 6 g of rhubarb.”

”The base of Yinchen Lidan Soup is another formula, Yinchenhao Tang. Its primary function is to treat the internal accumulation of damp-heat. The main therapeutic goals are to clear heat, promote diuresis, unblock the Sanjiao, or Triple Burner, and purge heat to expel stasis...”

”...Adding bupleurum root and turmeric tuber as auxiliary herbs helps soothe the liver, regulate qi, and promote blood circulation...”

”...Modern pharmacology shows that both Curcuma and red sage have blood-invigorating and stasis-dispelling properties, which can improve the microcirculation of the liver to a certain degree...”

”...Sophora root and polyporus mushroom enhance the effects of drying dampness and promoting diuresis, while Half-branch Lotus and spreading hedyotis have detoxifying and nodule-dispersing properties...”

”...When all the herbs in the formula are used together, they achieve the therapeutic effects of clearing heat and promoting diuresis to reduce jaundice, drying dampness, detoxifying, and resolving stasis!”

Listening to Wu Ting’s detailed explanation, even those unfamiliar with National Medicine found themselves nodding frequently.

Even experts like Ma Mengchao and Shen Guofeng were filled with admiration for Wu Ting’s integrated therapeutic approach.

”Incredible, Dr. Wu. While our experience in the surgical field may far exceed yours, your understanding and integration of National Medicine is truly humbling. I’m deeply impressed!”

The more Ma Mengchao and Shen Guofeng interacted with Wu Ting, the more they realized just how talented he was.

Yan Xi and Ling Ling felt an almost inexplicable sense of awe. A genius who could seamlessly integrate surgery with the theories of National Medicine was a rare sight, even at top institutions like Haijin Port and Union.

The two women furiously scribbled down Wu Ting’s treatment plan, paying special attention to the ingredients of the Yinchen Lidan Soup.

”Alright, let’s proceed with Wu Ting’s plan. We’ll start with the first phase of treatment for the patient’s obstructive jaundice. Once the bile is draining and liver function has stabilized, we’ll perform the radical resection for the cholangiocarcinoma!”

”Dr. Wu, how proficient are you with PTCD—percutaneous transhepatic cholangiodrainage?”

Ma Mengchao’s interest in Wu Ting was growing; he wanted to see the full extent of the young doctor’s capabilities.

”Of course. I can do it.”

Wu Ting accepted without hesitation. ’I’m looking forward to operating with Professor Ma and the others,’ he thought, ’and hopefully picking up a few more skill tags.’

’Today’s consultation, the diagnosis, the treatment plan... it was all accomplished by standing on the shoulders of giants.’ Wu Ting was realizing more and more just how much communicating, learning, and operating alongside these experts was accelerating his own growth.

Seeing Wu Ting’s confident and composed demeanor, Professor Ma and Shen Guofeng were both very pleased.

”Director Shen, let’s proceed with Wu Ting’s phase-one plan. We’ll implement a combination therapy of percutaneous transhepatic cholangiodrainage and the Yinchen Lidan Soup. Once the obstructive jaundice is resolved, I will personally handle the scalpel for the phase-two radical resection of the cholangiocarcinoma!”

A complex case like cholangiocarcinoma complicated by obstructive jaundice and a liver abscess couldn’t be resolved in a single step; the treatment plan had to be a multi-stage process.

They could complete the percutaneous transhepatic cholangiodrainage today, while having the National Medical Department of Jinggang University Affiliated First Hospital coordinate the Yinchen Lidan Soup therapy.

Afterward, they would record the patient’s daily bile drainage and simultaneously monitor vital signs, including blood work, liver and kidney function, and electrolytes.

As soon as the patient was stable enough for the second-stage radical resection, they could proceed with the surgery.

In the meantime, while waiting for the patient to be ready for the second stage, Professor Ma, Professor Shen, and the others could use the opportunity to conduct an inspection of City First Hospital.

[DING! Congratulations, Host, on completing a high-level consultation with Professor Ma Mengchao, Professor Shen Guofeng, Director Shen Xiaofei, Dr. Yan Xi, and Dr. Ling Ling. You have successfully diagnosed a difficult disease and proposed a comprehensive treatment plan. You have acquired a tag from Dr. Yan Xi: Indocyanine Green Fluorescence Technology (White). This is the research area of Dr. Yan Xi’s doctoral thesis, which is still in its early stages and has not yet yielded mature results. Note: This auxiliary technology has enormous potential and can be combined with various Surgical Techniques!]

[You have acquired a tag from Dr. Ling Ling: 3D Image Visualization Technology (White). This is the research area of Dr. Ling Ling’s doctoral thesis, which is still in its early stages and has not yet yielded mature results. Note: This auxiliary technology has enormous potential and can be combined with various Surgical Techniques!]

[Recommended Tag Synthesis: Indocyanine Green Fluorescence Technology (Yellow) + Cholecystectomy (Yellow) + Laparoscopic Technology (Yellow) = Real-time Fluorescent Laparoscopic Cholecystectomy (Purple). A brand-new laparoscopic cholecystectomy technique. Through real-time indocyanine green fluorescence navigation, the extrahepatic biliary tract can be clearly displayed. Under the fluorescence mode of the laparoscope, the extrahepatic biliary tract can be easily identified, making the LC procedure more visible, precise, and safe.]

[Note: In the pursuit of Daytime Laparoscopic Cholecystectomy, more haste means less speed. Inexperienced junior physicians inevitably cause iatrogenic biliary tract injuries during this procedure, which causes significant trauma to both the patient and the surgeon.]

[Real-time Fluorescent Laparoscopic Cholecystectomy allows for accurate intraoperative identification of the extrahepatic biliary tract, effectively reducing the incidence of iatrogenic biliary tract injury. It can also significantly shorten the learning curve for young physicians, making the surgery more precise, swift, and perfect, aligning with the ERAS (Enhanced Recovery After Surgery) philosophy of the minimally invasive surgery era.]

[This holds great significance for the concept of daytime surgery!]

[Missing Tag: Indocyanine Green Fluorescence Technology (Yellow), a mature version of the technology. Note: This auxiliary technology can be combined with various Surgical Techniques and has broad applications.]

’Looking at the tags that dropped this time, it’s a bit of a shame,’ Wu Ting thought. ’None from Professor Ma or Professor Shen. I would’ve even settled for one from Director Shen Xiaofei!’

’These two beauties are nice to look at, but not very useful!’

’They’re just PhD students, after all. Their tags are still white-tier.’

’Good thing three white tags can be upgraded to a yellow one!’

’Looks like I’ll have to interact more with these two doctors to upgrade those tags to yellow as soon as possible.’
