Doctor: I Can Synthesize Infinite Entries

Chapter 104: Chapter 103: Turnover Crisis, Running Out of Beds?

What followed was the classic laparoscopic cholecystectomy procedure, a true test of the technician’s fundamental skills.

The anesthesiologist administered general anesthesia with endotracheal intubation.

The patient was placed in a supine, reverse Trendelenburg position.

Afterward, Wu Ting began the classic three-port technique on the patient.

The first port was below the umbilicus. After making the incision, he inserted the trocar and began insufflating gas to establish a pneumoperitoneum.

He inserted the laparoscope camera, and Dr. Qin began to assist by holding the scope.

Watching Dr. Qin skillfully assist Wu Ting, Director Huang couldn’t help but feel a wave of emotion.

’What treatment! An ordinary junior doctor wouldn’t even dare to dream of this.’

Wu Ting began the exploratory laparoscopy with practiced ease:

”No abdominal adhesions, no masses...”

”No lesions on the liver or spleen...”

”No gastrointestinal adhesions, no tumors...”

”Confirming no bleeding at any of the puncture sites...”

Wu Ting conducted a meticulous and comprehensive exploration. Qin Donglai watched intently, forced to admit that Wu Ting’s fundamental skills were truly solid.

Next, Wu Ting carefully observed the size of the gallbladder, the extent of the inflammation, and its anatomical relationship with the surrounding organs and tissues until he had a crystal-clear picture in his mind.

Following Wu Ting’s guidance, Qin Donglai began to help by retracting the fundus of the gallbladder, pushing it together with the liver toward the upper right quadrant.

”Beginning blunt dissection of the gallbladder...”

Wu Ting began the procedure with proficiency.

Qin Donglai found himself admiring Wu Ting’s movements—gentle where they needed to be, yet swift and decisive when necessary. He didn’t damage the colon or duodenum in the slightest.

’Incredible. Staying this close to the gallbladder requires profound skill and immense confidence. This approach minimizes bleeding and collateral damage to the greatest extent possible.’

”...Dissecting forceps... Electrocautery hook...”

Wu Ting gave clear commands, and the scrub nurse promptly handed him the instruments.

He quickly finished with the anterior and posterior serosa and loose connective tissue at the corner of the gallbladder, then transected it with the electrocautery hook...

Next came the most critical part: the dissection of the triangle of Calot.

Wu Ting’s hands remained steady as a rock. Qin Donglai was also focused, his eyes glued to Wu Ting’s every move. After all, this was his first time performing a cholecystectomy with him.

In no time, Wu Ting had isolated the cystic duct and cystic artery...

”Titanium clips... Electrocautery scalpel... Scissors...”

In contrast to Qin Donglai’s tension, Wu Ting looked like a seasoned veteran, proceeding without the slightest hesitation.

After confirming the relationship between the cystic duct, common hepatic duct, and cystic artery, he clipped the cystic artery and transected it with electrocautery.

He clipped the cystic duct 0.5 cm from the common bile duct and then transected it with scissors.

Watching Wu Ting operate, Qin Donglai was filled with admiration.

Even surgeons who had been the primary operator for some time would find it difficult to immediately identify the relationship between the three structures: the cystic duct, common hepatic duct, and cystic artery.

But Wu Ting did it in one go. Such confidence, such composure!

After completing this most critical step, it was time to start dissecting the gallbladder from the liver bed.

Seeing Wu Ting’s choice, Qin Donglai was a little surprised.

’He actually chose retrograde dissection! By maintaining the right amount of tension, he’s making the plane between the gallbladder and the gallbladder bed clearer. A clever idea!’

Wu Ting’s subsequent actions were also performed with the air of a seasoned pro. Who could tell he had only recently started performing cholecystectomies?

’In the field of medicine, an ordinary person can’t even begin to imagine what it’s like to face absolute talent.’

The more he collaborated with Wu Ting, the more of a blow it was to his confidence.

’Decades of my own hard-earned skill... is this all it amounts to in front of Wu Ting?’

’Could it be? Can you really achieve a quantum leap in skill just by watching surgical videos?’

Qin Donglai began to ponder whether he should adjust his training methods for junior doctors in the future.

Before long, Wu Ting had excised the gallbladder, placed it in a specimen bag, and opted to extract it through the umbilical incision.

Because the patient’s gallstones were large, Wu Ting first brought the neck of the gallbladder out of the incision, aspirated the bile, and then used forceps to crush the stones before smoothly removing it.

The whole sequence of actions was as smooth as flowing water!

After suturing the incision, it was time to verify the most crucial part of this surgery!

After leaving the operating room, Wu Ting’s team members, Yasui and Cheng Feng, were put to work.

Calling the two over, Wu Ting began to lay out the key indicators for them to monitor.

”Yasui, Cheng Feng, I’m going to have to trouble you with the next part.”

”At 6, 24, and 48 hours post-op, you’ll need to assess the patient’s abdominal distension, the recovery of their gastrointestinal function, their post-op bowel sounds, the time to first flatus, and the time to first bowel movement...”

”Also, the patient’s post-operative nausea... autonomic nervous system function...”

”These indicators are extremely important. You absolutely cannot be careless.”

Cheng Feng and Yasui both thumped their chests in assurance:

”Don’t worry, we’ll keep a close eye on the patient’s post-operative recovery and guarantee the task is completed.”

As a top student, Cheng Feng was perfectly at home with this kind of data collection and statistical analysis.

He quickly created a table of observational indicators and testing methods according to Wu Ting’s design.

He printed it out, and he and Yasui began to take shifts recording the data.

The two were also very curious to see whether the patient from this joint National Medicine and modern medicine surgery performed by Wu Ting and Director Huang would have any significant complications.

”Dr. Wu, Dr. Qin, I’ll be heading off then. I’ll come by tomorrow morning, and we’ll start the next surgeries together.”

With the first case done, the rest would begin during the day tomorrow.

”Alright, thank you for your hard work, Director Huang. We’ll continue tomorrow.”

[DING! Congratulations to the host for completing the debut of the Qi-Regulating Laparoscopic Cholecystectomy in collaboration with Director Huang and Qin Donglai. You have obtained the following entry from Director Huang: Yinchen Lidan Soup (Huang), a traditional Chinese herbal decoction that can promote bile excretion, aid in the recovery of liver function, improve the patient’s clinical symptoms, and enhance treatment efficacy.]

[Compared to Western medicines like ursodeoxycholic acid, Yinchen Lidan Soup is relatively more expensive, limiting its widespread use. However, its principles are highly effective for certain specific patients. It can serve as a perfect adjunctive therapy for treating some severely ill patients.]

Seeing this entry, Wu Ting was a bit surprised. However, he hadn’t yet found a use case for this traditional Chinese decoction, so he didn’t pay it too much mind.

「The next morning, bright and early.」

While Xiao Xixi was compiling statistics on inpatients, she discovered a problem.

Given the current surgical schedule for the emergency surgery department and the subsequent hospitalization periods, the ward beds were extremely tight.

If all the patients that Qin Donglai and Wu Ting had already admitted, plus those who were about to be admitted, underwent surgery, there would be no way to turn over the beds based on standard recovery times.

Following the conventional post-operative observation period, such a large number of cholecystectomy patients would severely affect the turnover for other surgical patients in the coming days.

On top of that, the Wu’s Umbilical Hole Laparoscopic Appendectomy was also being performed on a large scale recently, and Jiang Ming’s side would also be transferring over a considerable number of post-operative patients.

Xiao Xixi prepared to go find Wu Ting and Qin Donglai to discuss putting a cap on the number of surgeries.

The operating rooms could handle the schedule, but the wards couldn’t house the patients. ’What a happy problem to have!’

’We can’t just discharge patients early, can we?’
