System Came When the Doctor was Thirty

Surgery is not child’s play.

In clinical practice, it’s said that treatment is thirty percent and nursing is seventy percent, and it’s not a joke. However, the reality is, there are far more people skilled in the seventy percent of nursing than those adept at the thirty percent of treatment.

After limb preservation surgery for severe injuries, professional nursing staff is needed for care. This kind of limb preservation is closer to limb replantation surgery.

At Xiangzhou People’s Hospital, it is more appropriate to transfer patients from limb preservation surgery to the hand surgery department.

Xiangzhou People’s Hospital does not have a specialized hand surgery sub-specialty department. Hand surgery is set within the upper limb trauma, hand, and microsurgery downstream professional group.

When Dai Linfang sent the patient to the upper limb surgery ward, Director Wu Xiang had already communicated with the on-duty doctor and nurse here.

At the nurse station, the nurse and the on-duty nurse immediately came to receive the bed, guiding the patient and family to the female ward.

The nurse turned and shouted, ”Fu Luoyin, the patient is here. Write the postoperative instructions.”

”Come... this way.” The nurse’s tone when speaking to Dai Linfang and the patient was much gentler than when addressing Fu Luoyin.

Dai Linfang told the anesthesiologist, ”This patient, when changing beds, does not need pressure on the foot, the calf area can bear force.”

”It’s relatively simple; I’ll go and communicate with the on-duty doctor about the instructions.”

At Xiangzhou People’s Hospital, escorting patients back to the ward is actually the task of the operating room orderly and the anesthesiologist. Dai Linfang accompanied them mainly to explain to Fu Luoyin the necessary instructions conveyed by Lu Cheng.

The anesthesiologist nodded, ”You go ahead, this patient is slender and indeed doesn’t need your help.”

If it were a two-hundred-pound heavy person, Dai Linfang certainly would have been indispensable.

After stepping sideways out of the crowd, Dai Linfang swaggered along the corridor of the department, dressed in scrubs, drawing no particular attention from anyone.

No one thought the limb preservation surgery he escorted was very difficult, or realized how challenging it was for this patient to avoid amputation.

This didn’t affect Dai Linfang’s cheerful mood and relaxed pace at the moment, soon arriving at the doctor’s office door.

The dual-opening bright yellow wooden door and the large, solid elliptical table inside stood prominently.

Compared to the narrow emergency office and computer setup, the upper limb surgery department’s computer stations resemble those in a company’s ’cubicles,’ single workstations without crowding for space.

Four ”elegant seats” on each side lining the walls.

Fu Luoyin was the on-duty doctor today. Of course, ever since receiving Director Wu Xiang’s call, he’s been making phone calls constantly, even now.

”Bro, are you on duty today?” Seeing the person was his age, Dai Linfang asked.

Fu Luoyin, holding his phone sideways and covering the microphone with his hand, looked up pleadingly, ”Shh, wait, I’ll finish soon.”

Then immediately, Fu Luoyin replied, ”Yes, Director Xiang, this is the current situation. Director Wu called to say that after coordinating with Director Liu, Director Liu agreed.”

”The patient is placed in our group and has already arrived.”

The Director Xiang being referred to by Fu Luoyin is Associate Director Xiang Dahong of Xiangzhou People’s Hospital. As of April 2024, he had returned from rural assignments.

Having become Director of Medicine, while the hand surgery department in the hospital was not yet independent, he naturally had already individually led a treatment group.

Xiang Dahong said, ”The patient has been sent, then handle the consultation well. Remember all instructions, and try to consult trauma surgery Director Wu as much as possible.”

”This patient was sent by him, must not have any issues, otherwise, with his temper, if you don’t ask, he can rake you over the coals till you doubt your life.”

Fu Luoyin hurriedly replied, ”Director Xiang, I will definitely report everything meticulously and timely to Director Wu.”

After saying this, Fu Luoyin hung up the phone. Short and stocky, once considered for trauma surgery but overlooked, hence allocated to hand surgery, he had gained much social knowledge: ”Sorry, brother, I was reporting to Director Xiang just now.”

”During our junior shifts, any patient in the group needs detailed reporting.”

”No worries, I’ll get right to the point. Note this down: after surgery, care is crucial. For instance, every four hours, check blood circulation and keep the local area warm.”

”Especially the postoperative anticoagulation, that needs to be applied appropriately. For the first five days after surgery, coagulation function must be checked every day, and every three days, a thromboelastogram should be done.” Dai Linfang’s memory was good, and his tone was formal.

Fu Luoyin’s eyes flickered lightly, ”Thromboelastogram is a bit expensive, isn’t it?”

In general, after surgical patients, coagulation function tests are conducted but not this one, mainly due to high expenses, hard to cover under DRG/DIP.

”This patient’s diagnosis does not fall under a single category. Cost issues, I’ve already discussed with the patient; they have agreed.” Dai Linfang, once a junior doctor, explained accordingly.

Fu Luoyin’s expression eased slightly, ”Then it’s fine.”

”For infection control, ensure sufficiency, our suggestion is to start with Piperacillin-Tazobactam. I recommend you immediately consult the pharmacy department for guidance on application.”

”After severe injuries, the biggest fear is infection; for other aspects, provide vasodilators, swelling reducers, pain relief, all medications must be precisely administered.” Dai Linfang did not specifically name less crucial details.

”Okay!~” Fu Luoyin nodded in agreement.

Upon hearing, Dai Linfang queried, ”Can you remember without noting it down?”

Fu Luoyin looked up, his face slightly stiff, ”Uh-huh?”

Thus, Dai Linfang repeated, and Fu Luoyin detailedly noted it down in writing before putting down the pen, drafting instructions, and saying harmoniously, ”Brother, your Director Wu has some real ideas.”
