System Came When the Doctor was Thirty

”Director Lu, what exactly is our trauma center responsible for? I asked other people in the department before, but no one seems to have explained it clearly.”

Du Hua’an’s name is delicate, but his appearance is imposing, and his voice is deep: ”We are all afraid that we’ll end up taking in cases we shouldn’t.”

Lu Cheng didn’t answer immediately but looked at Qu Daowen: ”Professor Qu, Professor Dong, there’s nothing happening here right now, so please go ahead and rest.”

Overall planning, it doesn’t hurt for Qu Daowen and the others to listen, but allowing them to hear the upcoming details would be somewhat laughable.

Qu Daowen and Dong Liumeng were quite sensible and got up to find a place to rest together.

Whether there’s work or not, they earn the same.

After seeing the two professors to the office door, Lu Cheng returned:

”Our trauma center differentiates from the emergency surgery department mainly by the factor of traumatic external force.”

”The simplest way to understand it is wound debridement and suturing; I also want to discuss some plans based on wound debridement and suturing.”

Lu Cheng continued as he walked back, in a steady tone: ”Discussing ideals without considering food is just fooling around.”

”Even though we’re not very old, everyone has their own pressures, be it economic or life pressures, it’s something personal.”

”Coming to work here means you’re hoping to earn some money steadily through this job.”

”In the hospital, performance equals the number of patients, which translates to workload.”

”Our trauma center is newly established, and we don’t have any exclusive surgeries yet, so wound debridement and suturing will be our major source of income.”

”My current arrangement is three shifts, rotating.”

”Dai Linfang and Zhang Zixu in one group; Chen Fang and He Dongsheng in another group; Du Hua’an and Liu Nongqiu in the last group.”

”Among them, Dai Linfang, Chen Fang, and Du Hua’an are the principal staff, while the other three are assistants.”

”Although I don’t take first-line duties, I am on call 24 hours. This means, at any time, under any circumstances, if you encounter cases that you cannot handle, you can call me.”

”I won’t be far from the hospital.”

”Is this duty arrangement clear enough?” Lu Cheng looked at everyone.

Lu Cheng’s explanation was clear, and no one had any objections, but Du Hua’an thought for a moment and asked: ”Director Lu, isn’t this similar to an attending schedule? Can you handle it?”

”In the first few months, even if I can’t handle it, I have to. Professor Qu and the others have irregular on-call schedules, and I definitely need to accompany their learning throughout.”

”Rest assured, my workload won’t be less than yours.”

Lu Cheng pressed his hand down, ending this topic, and then said: ”Next, I need to address some very practical issues.”

”Many of you have just graduated and entered the workforce, so I need to explain some things to you.”

”Aside from your salary, our hospital offers both average performance and departmental performance pay. As the name suggests, average performance is like sharing a big pot of food, meant to supplement departments with particularly low performance.”

”We clinical doctors don’t get much; we’re not administration, so we don’t have administrative subsidies.”

”We must also pay attention to this, so currently, those wound debridement and suturing cases that the emergency surgery department looks down upon, if we have the time, we must do them.”

”A patient getting debridement elsewhere or here is the same. Our ability to handle wounds comprehensively is not worse than other small hospitals.”

”During the initial operation of the trauma center, doing these small diseases for a bit more meal isn’t a problem.”

”Du Hua’an, Liu Nongqiu, this is your best opportunity to improve and hone your basic surgical skills!”

”It’s also your main task during your duty.”

Although Lu Cheng didn’t pursue further study, he trained at Zhongnan Hospital, and trainees like him and professional master’s students aren’t skilled enough to perform serious surgeries.

Being able to let you do an internal fixation removal surgery is the top limit.

After Du Hua’an and others nodded in agreement, Lu Cheng continued: ”At present, the serious surgeries belonging to our trauma center are roughly these few categories.”

”First, splenectomy for spleen rupture.”

”Second, abdominal debridement and bowel repair for bowel rupture. The others, like appendicitis or cholecystitis, belong to emergency surgery.”

”Third, external fixation for open fractures. For open fractures, you can call trauma surgery; they’re definitely willing to take the patients, and the hospital’s current arrangement is parallel admissions between the trauma center and trauma surgery.”

”What I mean is, if we can handle it ourselves, let’s not bother the orthopedic colleagues.”

”Fourth, open vascular injury and hematoma clearance, which are cases that cardiothoracic surgery doesn’t want.”

”Fifth, manual reduction of joint dislocation and simple fractures.”

”Du Hua’an, you’re a specialized master’s in orthopedics. Can you handle manual reduction?”

Du Hua’an shook his head: ”Director Lu, I was previously in joint surgery.”

”Alright, then follow me and we’ll learn slowly.”

”Chen Fang, during your duty, you mainly need to be responsible for splenectomy, and if there’s extra capacity, you can try hematoma clearance surgery and vascular suturing.”

”Dai Linfang is the same, but aside from spleen removal and vascular injury, you can also talk privately with the hand surgery department about tendon open injuries and nerve injuries.”

Dai Linfang understood these issues and wasn’t concerned with these minor ailments: ”What if we encounter a traumatic closed intra-abdominal hematoma?”

”Call me! Call me anytime!” Lu Cheng’s stance was clear.
