System Came When the Doctor was Thirty

Under the shadowless lamp, the hands left no blur, the surgical field was clear, and the blood flow to the spleen had already been blocked.

Lu Cheng stood at the chief surgeon position, after completing the preparations for the official excision of the lesion, he looked up for permission: ”Teacher Chen, I’m starting.”

Chen Song nodded: ”I told you to handle it throughout, you do it then, you’re the chief surgeon, and it’s legitimate! Stop wasting words.”

Chen Song just wanted to continue observing Lu Cheng’s operational details, the preliminary work of incision, hemostasis, and other fine operations were also done by Lu Cheng, but these are foundational tasks.

Exposing the incision is a procedural process, it has been refined to a precise level by countless predecessors, truly there’s nothing particularly remarkable about it!

This is a scheduled internal splenic fibroma surgery, not due to trauma related to emergency, hence, at the moment, the spleen mostly appears round, except for some cord-like protrusions at the upper-right corner.

Lu Cheng had carefully observed the growth direction of the tumor on the spleen and assessed its size. After cautiously cutting open the capsule, the red pulp was exposed.

The tumor was located at the junction of the white and red pulp, pushing the outer red pulp outward!

”Careful with the splenic sinus.” Chen Song wasn’t completely at ease, adding a remark.

Lu Cheng didn’t reply, focusing on delicately proceeding deeper with the precise knife in his hand, every point of entry and depth of cut was just right.

The ability to control subtle variances so precisely is difficult to capture directly with the eye but rather emerges through the details of the separation surface post-incision.

Chefs can use knives, ordinary people can use knives too.

A surgical expert’s use of a knife differs in its precision, the higher the mastery, the more absolute the control over one’s hand during the excision.

Those of high realm, hand and knife become one, those of higher realms, mind, hand, and knife become united.

Lu Cheng’s skills haven’t reached such an extraordinary level, but he could achieve complete unity of hand and knife, enough to change Chen Song’s perception of him.

Fibromas are benign tumors, they have their own complete capsule, structure, and pedicle.

To remove it entirely is not just cutting where it is, but to follow the splenic trabeculae, expose it as much as possible, and excise it entirely with the smallest incision.

Or, use part of a Splenectomy technique from Spleen Preservation Surgery, excising together with the splenic parenchyma where the fibroma is located!

Chen Song watched for about two minutes, then slowly nodded: ”Not bad, no exaggeration.”

At this moment, Zhang Tiesheng, although the second assistant, at a realm not yet there, couldn’t discern the details, only responsible for shouting ”666”: ”Very nice!”

Chen Song asked: ”What’s nice about it?”

”Operation.” The clueless Zhang Tiesheng gave the answer to gain some points, his eyes kindly flattering with friendship.

”Forget it, this question is too difficult for you.” Chen Song didn’t pursue a specific answer from Zhang Tiesheng.

Asking a primary student to give the demonstration for an ellipse function is overly challenging for them.

”There are many in the red and white pulp.” Chen Song paused in his explanation to Lu Cheng.

Lu Cheng had already dissected the proximal end of the red pulp parenchyma, found the proximal end of the red pulp’s nutritional vessel, single-handedly sucked out the residual blood in the cavity as swiftly as a bachelor finding a widow, clamping the principal branch of the vessel.

Then organized the branches...

Spleen Preservation Surgery is not simply three words, nor is it about fighting or killing, nor social affairs, it’s meticulous operation, for the organ in part, it’s life re-blooming...

The essence of medicine’s disenchantment lies in the difficulty and not knowing, knowing makes it not difficult.

Operational skill and details, reaching that level, seeing a mountain is a mountain, not reaching that level, to an outsider, it appears unbelievable.

…

After the surgery, Lu Cheng did it thoroughly smooth, Chen Song watched thoroughly smooth, only Zhang Tiesheng felt Lu Cheng and Chen Song were somewhat odd.

Especially Chen Song seemed like he had trained in Rongcheng, his gaze towards Lu Cheng carried some ’playfulness’, turning around Lu Cheng repeatedly: ”Tsk tsk, tsk tsk, tsk tsk tsk...”

Lu Cheng cautiously replied: ”Teacher Chen, actually, I suddenly feel now, regarding Spleen Preservation Surgery, adhering to basic principles and performing each operation step just right, there’s no need to follow other teachers’ ’standardized procedures’!”

”Of course, I’m not sure if my understanding is correct!~”

Chen Song stood firmly at Lu Cheng’s right, his right hand cupped his chin: ”Hey, Lu Cheng, let me give you an analogy, guess why when middle school students solve a linear equation, they need to list detailed steps.”

”But not in high school?”

”And in undergraduate university, even more process can be omitted? Why?”

Lu Cheng vaguely guessed, replied: ”Teacher Chen, you want to say that the so-called standard surgical technique is actually prepared for the beginner doctors?”

”That’s right!~”

”We discussed before, a standard surgery can be seen as a human-made ’procedure’, for example in Splenectomy, first find the splenic artery, clamp the blood vessels going in and out of the spleen.”

”The essence is, prepare everything preliminarily, ensuring post-Splenectomy, no blood vessel leaks.”

”Beginner doctors studying this will generally not go wrong by following this principle.”

”But some professors, some chief physicians have their own experience, operations in fine detail, they don’t need to follow recommended surgical techniques.”

”This is the essence of surgery, also the reason why surgery, medicine, and regulatory agencies grant senior clinical physicians proactive permissions.”
