System Came When the Doctor was Thirty

Although Lu Cheng went to the hospital office, discussing with hospital leaders about the management responsibilities for the trauma center’s cases in advance, the trauma center itself is yet to be established.

This month’s system is still the specialty consultation system; whenever there is an emergency case, a specialist consultation must be appropriately requested.

Lu Cheng has surgical privileges, but not the primary right to admit the surgery. After notifying by phone, the other party allowed Lu Cheng to proceed, so it’s considered that the surgery is relatively simple, leaving the primary right empty.

After hanging up the phone, Lu Cheng walked out of the ’rest room’.

Dai Linfang approached with the medical record book: ”What did the cardiothoracic department say?”

”They told us to handle it ourselves,” Lu Cheng replied.

Dai Linfang blinked and laughed: ”Alright... if it’s inconvenient for you, I can go over.”

Vascular Suturing Technique, Dai Linfang has learned it too; although his current skill isn’t as absolutely proficient as Lu Cheng’s, he has a good basic skill set, certainly sufficient to handle this patient.

Quite fittingly, he can apply and try that new suturing technique.

Lu Cheng nodded: ”Then you go, but before you do, make sure to report to Director Tian.”

Dai Linfang is an attending physician with secondary surgical privileges; ordinary vascular suturing skill is, at most, a secondary operation.

”Okay!~” Dai Linfang excitedly ran out of the emergency room, about to commence his first vascular suturing surgery as lead surgeon in the hospital.

Lu Cheng then sat down, his right hand lightly tapping the table, reflecting on these recent events.

Afterward, Lu Cheng stood up again, preparing to follow up on the patient’s CT situation.

Establishing a trauma center isn’t as simple as assuming a leader’s title and charging ahead with a few people; it definitely needs its own category of cases.

Otherwise, within a month, there won’t be much performance incentive, and those following along will eventually leave.

Doctors need medical ethics, need passion, but also need to survive, have their own lives.

Based on this, passion alone isn’t enough. Lu Cheng immediately proposed to the hospital leaders that he needed to draw several simple case types from specialties to boost the trauma center’s workload.

These include, but are not limited to, open fractures, simple closed fracture reductions, and vascular injuries.

...

The trauma center doesn’t have its own ward; postoperative patients still have to be transferred to the specialty wards of emergency surgery.

After Dai Linfang finished the surgery, he personally escorted the patient back to the ward!

Upon learning of this situation, Lu Cheng went immediately to the bedside to perform a physical examination on the patient.

After all, it wasn’t a surgery he led, and without deeper understanding of Dai Linfang’s actual operation skills, Lu Cheng trusted more in what he observed and the results of the physical examination.

”Doctor, is everything alright?” The patient and family looked at Lu Cheng nervously.

”It’s quite good. The pulse of the terminal artery is clear, indicating good vascular suturing,” Lu Cheng assured.

”Next, what you need to pay attention to is avoiding strenuous activity, or else if the arterial suture cuts into the artery wall, subcutaneous hematoma will develop, which will be quite troublesome.” Lu Cheng stood up, used hand sanitizer while instructing.

The patient patted his chest: ”Just fine, Dr. Lu, you startled me coming in suddenly.”

Lu Cheng smiled: ”Rechecking before and after the surgery is for greater caution, not due to any issues. Being attentive is never wrong.”

”Alright, rest well; I’ll be leaving now.”

”If there’s any issue, you can call the on-duty physician; our ward also has one on duty.”

”Avoid food irritants, avoid spicy things.” Lu Cheng advised a bit more.

While speaking, Lu Cheng’s phone rang again; it was Zhou Lü, on duty in the emergency surgery office.

”Teacher Zhou.” Lu Cheng isn’t very familiar with Zhou Lü!

”Lu Cheng, there’s a joint dislocation here. Is this under your charge or the trauma surgery’s?”

”The permissions suddenly changed mid-month; I’m a bit confused.”

The surgery office is responsible for initial assessment and post-assessment notification for specialty consultation; hence they need clarity on which case types belong to which departments.

”Joint dislocation still belongs to trauma surgery. I am currently only responsible for fractures, open fractures, open bleeding, traumatic subcutaneous hematoma.”

”Teacher Zhou, I’ll personally come over later and list a form for you; really sorry for the inconvenience.” Lu Cheng quickly responded.

In the process of preparing for the trauma center, Lu Cheng pulling some case types is indeed beneficial for himself, but it indeed has inconvenienced the brothers on duty in the office.

It’s only today’s implementation that made Lu Cheng realize this matter.

No one can manage everything flawlessly or comprehensively.

Zhou Lü replied: ”It’s just a minor issue.”

...

When Lu Cheng took his compiled report to the surgery office to update Zhou Lü, his face showed bitter frustration.

Zhou Lü pulled Lu Cheng close, whispered: ”Lu Cheng, the on-duty doctor in trauma surgery may be a bit upset; he’s unwilling to come over. We should ask the patient to go to the ward.”

”What to do with this patient? Should we send them directly to trauma surgery or?”

Lu Cheng replied softly: ”Then call the patient to the department. In any case, it’s just a joint dislocation... often patients go to the ward themselves.”

Lu Cheng understood Zhou Lü’s implication; the on-duty doctor in trauma surgery was disgruntled.

Why is it that fractures are handled by Lu Cheng, while joint dislocations are by trauma surgery?

The on-duty doctor in trauma surgery might not want his duties dictated by chaos triggered by Lu Cheng.

If you’re going to do it, do all, or none at all.

Wu Xiang’s arrangements with the hospital leaders might favor some, but have no impact on junior doctors.

Without authorized actions, Lu Cheng cannot proceed, unless the trauma surgery directly refuses the patients.
