System Came When the Doctor was Thirty

Lin Qianlong was just testing Xiang Kuihua’s reaction and did not ask Xiang Kuihua to bring the equipment back directly.

Lu Cheng: ”Director Hou, how are the patient’s vital signs? They should be okay, right?”

Hou Jiagui replied: ”There’s not much of a problem. The difficulty with limb amputation patients is limb preservation, not stabilizing vital signs.”

”If the bleeding is stopped before entering the operating room, it’s generally hard for them to go into shock.”

”Well, let’s leave it like this for now...” After finishing, Lu Cheng began to transport the patient.

Circulating nurses and Hou Jiagui from the anesthesiology department first transported the patient to the recovery room.

After taking off the sterile surgical gown and gloves, Lu Cheng glanced at the surgery form filled out by the circulating nurse.

After hesitating for a moment, Lu Cheng replied: ”Hey, this form isn’t filled out well. Let’s revise it, and adjust the surgical billing later too.”

”Don’t write limb replantation, just write open fracture internal fixation plus muscle suturing, nerve, vascular suturing.”

There was more than one circulating nurse present, and one of them stiffened a bit upon hearing Lu Cheng’s words: ”Dr. Lu, I’ve already completed the form.”

Lu Cheng chuckled: ”Just write a new one; your surgical technique is incorrect.”

”We planned for limb replantation, but in the end, it wasn’t limb replantation.”

Xiang Kuihua glanced at Lu Cheng, thinking Lu Cheng was truly cautious to the point of obsession, leaving no blind spots for anyone.

”Alright.” The circulating nurse conceded reluctantly.

Her surgical technique was incorrect, and the lead surgeon could refuse to recognize the surgery check form.

The actual surgical technique spoke to the absolute authority of the lead surgeon, although she didn’t understand why Lu Cheng wanted to do this, even if she was unwilling, Lu Cheng could apply for another circulating nurse to do it.

Moreover, for something this trivial, it was clearly unreasonable to create discord with Lu Cheng just as a single circulating nurse.

...

Upon exiting the operating room, Lin Qianlong glanced at Lu Cheng and casually asked: ”Xiao Lu, the charge for limb replantation is slightly higher; what were you just now?”

”Director Lin, our hospital hasn’t approved registration for microsurgery and hand surgery specializations. Doing this is against regulations,” Lu Cheng replied.

Lin Qianlong paused: ”Then Professor Tong Yuan’an?”

”Professor Tong has surgical privileges independently; he’s here for outreach support, which is considered multi-center practice.”

”Strictly speaking, Director Lin, calling Director Hou over aligns with the rules. The anesthesiologist for limb replantation surgery must have at least an associate chief physician title.”

”Having a regular attending sign off is a lapse...”

Lin Qianlong nodded: ”Oh, that’s how it is. No wonder the people in the operating room would say Professor Tong has a lot of requirements during surgeries.”

”So writing the surgical technique like this is fine?” Lin Qianlong asked further.

Could it be that no one thought you deliberately broke down the surgery?

”Director Lin, we can’t just stand by and watch!”

Lu Cheng said: ”If we couldn’t do limb replantation, we’d work to preserve the limb; we just have to follow the rules...”

Lu Cheng immediately went out to communicate with the family members.

The patient’s father had already arrived. Upon seeing Lu Cheng, both rushed over at once, eagerly looking at Lu Cheng and Lin Qianlong:

”Director Lin, how was the surgery? Was it successful?”

Lin Qianlong, being a seasoned associate chief physician and one of the surgical team leaders, Lu Cheng was his assistant: ”The hand should be preserved, but for postoperative function, don’t have too high expectations.”

”You sent him in too late, surgery may also result in necrosis.”

”Moreover, the bones, blood vessels, and nerves were all severed; even if reconnected, they certainly won’t match the original.”

”If it doesn’t necrotize, its function will be much worse compared to the other hand and peers.”

The middle-aged man’s face, seemingly shrewd and calculating, shook his head helplessly: ”It’s come to this, and there’s no other way.”

Lin Qianlong said: ”Don’t think too much now; it’s good that the hand can be saved for now. It’s better to have it than not.”

”You can take a rest for now. He’ll come out of the recovery room soon...”

”This is Dr. Lu Cheng from our department. He will be your son’s primary care doctor. If you have any questions, feel free to communicate with him.”

”Dr. Lu will also thoroughly explain postoperative precautions to you.”

”Limb replantation isn’t like planting vegetables; it won’t just grow fruit. It requires careful nurturing.”

Lu Cheng returned to the emergency department doctor’s office with the patient and family, explaining postoperative precautions while prescribing medical orders.

”There will be two oral medications later; one is mecobalamin for neural nourishment, and the other is sodium aescinate for reducing swelling. Remember to take them timely.”

”The nurse will bring them to you and will explain the usage again.”

”Additionally, over the next few days, make sure to monitor for any signs of the hand turning blue or black; after surgery, thrombosis can easily occur, hindering blood flow, which is the risk inherent in this surgery...”

”Also, when you have time later, pay the surgical fees.”

The man nodded: ”Dr. Lu, it’s not a problem. As long as my son’s hand is saved, we’ll pay the fees in full.”

Though his words had a stipulation, Lu Cheng could only say so much.

”There’s nothing else special, just wait for him to come out, and I’ll see him again before leaving.” Lu Cheng stated.

”Dr. Lu, are you leaving soon?”

”Aren’t you going to always watch over my son?” the woman instantly panicked.
