System Came When the Doctor was Thirty

”The reason there are so many tendon suturing techniques is that too many orthopedic surgeons have failed using simple interrupted sutures on tendons, which has forced orthopedic surgeons to innovate suturing techniques.”

”The suturing of the spleen, gallbladder, and liver you just mentioned, however, works quite well with simple interrupted sutures, so there’s no need to discuss advanced suturing techniques.”

Chen Song was pleased that Lu Cheng shifted the topic to professional content.

Otherwise, Chen Song wouldn’t know what to discuss with Lu Cheng—talk about Lu Cheng’s ’betrayal’?

No, no, no, such thoughts are too harsh, Lu Cheng had never been labeled as Chen Song’s prohibition.

Lu Cheng said, ”Teacher Chen, take a look at this article, it’s a paper published by a urology and hand surgery team, discussing some microsurgical suturing techniques.”

”It allows for separation and replantation of the corpus cavernosum...”

Chen Song picked up Lu Cheng’s phone, glanced at the abstract of the paper, and understood what it was about.

Turning his head to look at Lu Cheng: ”So what? What do you want to express?”

Lu Cheng explained to Chen Song in detail: ”Teacher Chen, when I went to Union Hospital to study, a teacher there mentioned that the essence of suturing skills is to reconstruct the order of life through suturing means.”

”Suturing is the means, reconstructing the order of life is the essence.”

”For instance, the essence of nerve suturing, vascular suturing, tendon suturing, is all about connecting the ends of tissues with suture techniques, allowing scar tissue to regenerate.”

”Many other surgeries are similar.”

”The concepts in spleen preservation surgery are actually no different.” Lu Cheng said to Chen Song.

Lu Cheng’s words included both a summary and specific references, making them easy to understand, prompting Chen Song to ask, ”Are you implying that suturing techniques in spleen preservation can be used to further expand its indications?”

Lu Cheng replied, ”Teacher Chen, it’s not about expanding the indications of spleen preservation surgery, but rather expanding the scope of suturing skill application within it.”

”Currently, spleen preservation surgery still focuses on partial spleen resection. If suturing techniques can be modified to allow cases requiring partial resection to be treated with sutures, it would lead to a good prognosis.”

Lu Cheng didn’t dare to consider changing surgical indications and contraindications, just thinking about whether the suturing techniques from handsurgery could be integrated into spleen preservation surgery to expand the application of suturing skills.

This is a fairly grounded speculation.

Lu Cheng’s ideas are easy to understand, but very challenging. Chen Song furrowed his brows tightly: ”Spleen tissue is completely different from tendon tissue, these substantial organs are much more fragile!”

”Plus, the internal blood supply system of the organ is intricate, making processing extremely difficult.”

”If not handled well, it could lead to local necrosis, and the risk the patient faces is much higher than the failure after tendon suturing.”

Lu Cheng said: ”But, Teacher Chen, the standard rescue technique for spleen rupture is spleen removal surgery. Comparing the risks between spleen preservation surgery and spleen removal surgery, expanding the application of suturing skills actually keeps the risk manageable.”

Lu Cheng’s statement is logical and well-founded.

How high is the risk for the first surgeon to perform spleen preservation surgery?

Chen Song glanced at Lu Cheng: ”You haven’t even started your own research projects, now you’re thinking about other topics?”

”Eating from one bowl while eyeing the pot? Is that the kind of person you are?”

Lu Cheng’s tone was calm: ”Teacher Chen, the focus of the tendon suturing technique project is on whether it’s done or not, rather than the theoretical inadequacy of the technique.”

”This project currently just lacks sufficient data support, but the maturity of the techniques is adequate.”

”As for the spleen suturing technique, it’s still in its nascent stage, requiring early preparation.” Lu Cheng said.

”So you’re planning to use this to impress me?” Chen Song raised his proud chin, becoming momentarily feline-like.

Lu Cheng calmly shook his head and said, ”Teacher Chen, by saying this, aren’t you bringing the topic back?”

”I just feel that spleen preservation surgery has already utilized the Hemostasis Technique extensively, but hasn’t integrated all the suturing skills.”

”Thus, I wonder if there’s a loophole to exploit. These are the surgeries I’m capable of, and the ones I ponder day in and day out.”

Chen Song’s brows began to tighten; sometimes they stood upright, sometimes they relaxed.

Who knows how long it took before Chen Song looked at Lu Cheng, his tone somber: ”You too are a restless character, the type to make waves in still water.”

”Quite the pursuit of intellectual understanding you’re speaking of.”

…

Half a minute later, Chen Song noticed Lu Cheng was unfazed and posed a soul-searching question. ”Do you know specifically what you’re talking about?”

Outside, there’s a gentle breeze. The breeze sways the leaves, creating a shadowy dance, playing tricks on two pairs of eyes, bit by bit, layer by layer.

Lu Cheng nodded and then shook his head: ”Teacher Chen, to be honest, I don’t fully understand, which is why I want to ask you about this.”

Lu Cheng hadn’t done much research, and his reading volume of academic papers was not substantial.

Though he recently started reading, his scope remains very limited.”

Chen Song pursed his lips, then held back, but in the end couldn’t resist: ”A newborn calf is not afraid of tigers. Do you think what you, Lu Cheng, can think of, others can’t?”

”The current implementation of the spleen preservation surgical technique indeed hasn’t maximized the advantages of suturing, but is it that we don’t understand this?”
