System Came When the Doctor was Thirty

After hanging up the phone, Lu Cheng regretted eating slowly and hurriedly shoveled a few more bites into his mouth.

Lu Cheng picked up the mineral water, drank it down, then pushed the door and ran outside.

A patient needing ECMO can’t be kept waiting!

Of course, ECMO is expensive, and poor people definitely can’t afford it.

Currently, only one senior doctor in the ICU had learned this technique after Lu Cheng went for study, so he was responsible for ECMO previously.

...

When Lu Cheng rushed into the operating room, he heard a middle-aged man in his forties curse, ”Damn, it slipped again...”

Lu Cheng had already scrubbed in and disinfected, went straight to the dressing table to grab the sterile surgical gown, and softly called in Huang Haibo’s direction: ”Director Huang, I’m here.”

Huang Haibo was also on the stage: ”Get dressed and get on the stage!~”

Then Huang Haibo said, ”Dr. Lu is our specialist in the emergency department and has also attended advanced classes at Xiangya Second Hospital, so he has some understanding of this.”

”I hope I can assist you, Director Xu.”

Director Xu was a bit agitated at this moment and turned to ask, ”How’s the patient’s blood oxygen levels?”

”Still heart failure!~” The anesthesiologist’s hands were moving frantically, adjusting the infusion pump speed while fine-tuning the oxygen rate.

”You might need to hurry up; the prolonged low oxygen time might lead to brain death.”

”Alright, I’m doing my best!~” Director Xu patiently nodded, ”I’ll try again.”

”What’s the situation?” By now, Lu Cheng had started putting on sterile gloves and stepped closer to Director Huang Haibo.

”The patient initially had low blood pressure and anemia; currently, the surrounding blood pressure is very low, the vessels are occluded, and the vascular condition is poor, making them easily damaged and difficult to catheterize,” Huang Haibo explained.

”Heart failure; Director Xu initially planned to do a femoral vein-femoral artery VA mode, the femoral vein is relatively large, and one side catheter is already placed.”

”But the femoral artery is much more complex to operate on.”

”The carotid artery was also attempted once, but it slipped...”

ECMO is an external circulatory system that temporarily or partially replaces cardiopulmonary function.

It requires connecting the arteries and veins of the body to supply oxygen internally.

This patient’s vascular condition is so poor, combined with heart failure, and choosing to rescue in this way signifies they must be pretty wealthy...

Of course, Lu Cheng isn’t biased against the rich.

With gloves on, he untied the sterile surgical gown straps, handed it to Huang Haibo, and turned around.

”Director Lin from cardiac surgery is also here...” Lu Cheng approached the operating table as Huang Haibo reminded him.

The cardiac surgery department of the State People’s Hospital also covers vascular surgical cases; whether it’s intervention or sheath insertion, the operations are quite proficient!

Huang Haibo was clearly reminding Lu Cheng to be mindful of his actions.

Lu Cheng nodded and approached, proactively reporting, ”Director Xu, I’m here to learn a bit; let me first check the specifics.”

Director Xu had already failed several times. Hearing Lu Cheng’s youthful voice calmed him. ”This patient’s vascular walls are particularly poor, and if the puncture position isn’t precise, it’s easy to pierce the vascular wall.”

”I pierced once, Director Lin from cardiac surgery has done it thrice, all with the same issue.”

”A special balance point and angle are needed, which aren’t easy to find.”

Lu Cheng understood gradually upon hearing this.

Puncture skill, it’s both easy and challenging.

Easy to learn, not hard to become skilled, but when faced with a troublesome vascular condition, you need to find the right angle to stabilize the sheath insertion.

[Puncture Skill (Specialization 20/20), ECMO Technology (Specialization 0/20)]

This reflects Lu Cheng’s current skill list.

Lu Cheng nodded and began scanning the visual field, quickly grasping the previous operational state.

The patient’s skin was loose, indicating they should be quite elderly.

The femoral artery path already had multiple incisions, all under pressure dressing.

”The skin tissue is loose, the body is thin but the vessels aren’t prominent, the vascular condition is indeed poor.”

”Operations are quite tricky,” Lu Cheng replied.

”Do you want to try once?” Director Xu directly asked Lu Cheng.

Although he works in the ICU, he has close contact with the emergency department, having heard Lu Cheng’s name from Huang Haibo many times.

[Current Skill Points Balance: 166.2!]

Soon, 60 skill points were spent.

ECMO Technology and Puncture Skill both became expertise. 60 skill points could take seven to eight days to accumulate but are used in a flash.

Lu Cheng emphasized, ”Director Xu, I’ll only try once!~”

”Alright!~”

”If it doesn’t work, we’ll have to discuss it with the family outside!~” Director Xu didn’t pressure Lu Cheng.

Lu Cheng’s success would prove effective rescue; failure couldn’t demean Lu Cheng’s abilities, could it?

What about the previous attempts by Director Xu and Director Lin from cardiac surgery?

No better than a dog?

Lu Cheng carefully received the puncture instrument from Director Xu, then seriously palpated the patient’s skin over the femoral artery.

After a while, Lu Cheng’s brow furrowed deeply.

This patient is indeed extremely troublesome; even the pulse of the femoral artery is very weak.

To describe her as ’at death’s door’ wouldn’t be wrong.

Having explored both sides, Lu Cheng gave up on the left and right femoral arteries and moved to the patient’s left hand!

Previous failures by Director Xu and others caused a missed opportunity in femoral artery operations.

Pressure dressing at present means even successful puncture won’t be very effective.
