Chapter 281: Chapter 211: High Expert Fee

When Huang Yuliang saw Sister Hong collapsed on the ground, her chest covered in blood, he was about to rush over and hold her.

Wu Ting immediately stopped him. ”Uncle Huang, don’t move her. She has abdominal trauma, likely a liver injury, and is in hemorrhagic shock. The iron rod has caused a penetrating wound. If you move her improperly, it could cause uncontrollable massive bleeding. I’ll perform initial first aid and diagnosis now. All you can do is wait for the ambulance.”

Wu Ting was, after all, an ER doctor from City First Hospital. Despite his youth and junior status, his firm tone at a time like this convinced Huang Yuliang to nod and wait by the side.

Wu Ting began a preliminary diagnosis of the patient so that when the ambulance arrived, he could quickly brief the doctors from Yu State Hospital.

Just after Wu Ting had preliminarily stabilized the patient’s condition, the ambulance arrived.

Wu Ting happened to know the doctor who came with the ambulance; it was Dr. Du, a resident physician under Director Duan.

Dr. Du was pleasantly surprised to see Wu Ting.

”Dr. Wu, what are you doing here?”

Wu Ting explained briefly, then relayed his assessment:

”Patient is a 43-year-old female. There’s subcutaneous bruising slightly to the left of the xiphoid process, slight swelling in the upper abdomen, and tense abdominal muscles. Percussion reveals dullness in the mid-upper abdomen...”

”Preliminary diagnosis: abdominal trauma, liver injury, hemorrhagic shock... We need to monitor her blood pressure, heart rate, and other vitals. I also need to perform an abdominal paracentesis to determine if there’s other parenchymal organ damage...”

Wu Ting had given quite a few training lectures in the ER of Yu State Hospital and had also led many instructional surgeries, so Dr. Du took his opinion very seriously.

”It’s such a relief you’re here, Dr. Wu.”

Dr. Du cooperated with Wu Ting, safely moving the patient onto the ambulance stretcher while simultaneously preparing the necessary instruments and equipment for him.

Watching Wu Ting take complete control of the situation, with even the doctors and nurses from the state hospital cooperating with him, was a sight to behold.

Huang Yuliang and Wu Mu were both filled with emotion. ’That Wu family boy has really made something of himself.’

Just then, a few other villagers also drove up to the scene.

When they heard about Wu Ting’s performance, they all gave him a thumbs-up. But with Sister Hong’s life still hanging in the balance, everyone could only watch helplessly as the ambulance drove away.

In the ambulance, a nurse had already started checking the patient’s blood pressure and quickly reported:

”Dr. Wu, the patient’s blood pressure is 78 over 60...”

Hearing this, Wu Ting’s expression turned grave. The patient’s condition was not optimistic. She would need a blood transfusion immediately upon arriving at Yu State Hospital.

”I’ll perform the abdominal paracentesis. You draw blood samples for typing and crossmatching, administer oxygen, keep her airway clear, and continue monitoring her vital signs...”

Since the patient was unconscious and in a state of shock, Wu Ting prepared to make a flexible adjustment to the abdominal paracentesis procedure.

”Dr. Du, get me a thin plastic tube as well...”

Dr. Du didn’t quite understand this, and he also noticed that Wu Ting’s puncture site wasn’t in the lower abdomen.

Seeing Dr. Du’s confusion, Wu Ting casually explained:

”For an unconscious patient, the puncture site shouldn’t be limited to the lower abdomen; the optimal location is the area of abdominal dullness... The positive rate of the paracentesis is related to the severity of the liver damage and the amount of bleeding. In a clinical setting, when diagnosing liver trauma, it’s very possible for the paracentesis to come back negative. You can perform repeated or multi-site punctures. If necessary, you can also insert a thin plastic tube into the abdominal cavity through the trocar cannula and periodically flush and aspirate with saline. This increases the positive rate...”

Listening to Wu Ting’s explanation, Dr. Du understood. He felt a sense of admiration; Wu Ting’s considerations were so thorough and meticulous, even under such urgent circumstances.

”I’m impressed.”

Dr. Du immediately assisted Wu Ting.

Soon, the results of the abdominal paracentesis came back. As expected, there was massive liver hemorrhaging, combined with a small amount of bile leakage... The situation was dire.

However, whether it was the hepatic vein or the retrohepatic inferior vena cava that was damaged could only be determined during surgery.

After they arrived at the state hospital, Director Duan also hurried over.

Seeing Wu Ting get out of the ambulance, Director Duan was a bit surprised. ’I didn’t call for any outside help.’

Dr. Du immediately went forward and explained, and only then did Director Duan realize that Wu Ting was considered family to the patient.

”Director Duan, the patient’s condition is critical. I’ve already performed an abdominal paracentesis in the ambulance and confirmed massive liver hemorrhage with bile leakage... She’s in hemorrhagic shock and needs an immediate exploratory laparotomy for hemostasis, blood transfusion, and related surgical procedures...”

Wu Ting very clearly handed over the patient’s case.

After all, this was their territory; Wu Ting couldn’t just step in and take over the rescue. He was familiar with Director Duan, but this was Yu State Hospital, and they had their own rules and regulations.

If Director Duan didn’t extend an invitation, it wouldn’t be appropriate for Wu Ting to interfere.

He and Huang Yuliang were fellow villagers, after all. In this situation, only if the patient was truly at death’s door and would die if he didn’t act would Wu Ting step forward. Clearly, things hadn’t reached that point yet.

So, Wu Ting tactfully stood aside, taking on the role of a family member.

At that moment, the other directors from Yu State Hospital also rushed over. For Yu State Hospital, a case like this was extremely serious, and everyone was giving it their full attention.

One of them was Luo Can, the director of general surgery. The other was even more distinguished: Liu Zhanshuang, the vice president of Yu State Hospital and the director of hepatobiliary surgery.
