Chapter 316: Chapter 224: Flexing at Zijingang Again! He Knows Everything

Shen Siyuan, Huang Dian, and the others were all very happy to see Wu Ting.

Lately, Shen Siyuan’s research project had been progressing rapidly under Ye Xin’s guidance.

Today was a good opportunity to relax, so she accompanied Wu Ting to the cardiac surgery ward for a walk.

Huang Dian began his rounds, leading two junior doctors under him.

Shen Siyuan and Wu Ting followed alongside them.

Relatively speaking, Wu Ting’s face was still rather unfamiliar in the cardiac surgery department. Only the mid- to high-level doctors knew him better.

The junior doctors saw the young Wu Ting with Shen Siyuan and assumed he was just some newcomer, so they didn’t pay him much mind.

Still, with his good looks, he was easy on the eyes.

Just as the group was halfway through their rounds, a patient at the other end of the hall, who had been scheduled for discharge today, was suddenly struck by severe pain.

The patient clutched their chest, appearing agitated and restless, sweating, and experiencing chest tightness...

Seeing the patient’s condition, the family was frantic and at a loss. They were supposed to be discharged today; how could this suddenly happen?

”Doctor, Doctor!”

Without a second thought, Huang Dian and Shen Siyuan rushed over.

”ECG monitor...”

”Blood oxygen saturation...”

”And start monitoring with an electrocardiogram...”

Shen Siyuan tensed up; she knew a bit about this patient.

”Get Director Shao over here immediately! And contact the emergency department for a consultation...”

After Shen Guofeng and Shao Yuqiao arrived, the results of the patient’s various physical examinations came in.

Huang Dian looked at the patient, who was in agony on the bed, and broke out in a sweat himself.

”The patient’s blood pressure is below 80 mmHg... Blood oxygen saturation is dropping... And their temperature is rising...”

As Huang Dian was reporting, the patient began to experience abdominal bloating and pain. Shortly after, they became nauseous and started vomiting...

While examining the patient, Shao Yuqiao began to get a clearer picture.

”Is the ECG out yet?!”

Urged on by Director Shao, the electrocardiogram results were quickly produced and handed to him.

”...Left ventricular outflow tract stenosis, with an internal diameter of less than 20 mm... Systolic anterior motion of the mitral valve (SAM)... A pressure gradient in the left ventricular outflow tract greater than 30 mm Hg... And anterior motion of the anterior mitral valve leaflet’s CD segment during systole, appearing as an abnormal wave on the mitral curve’s CD segment...”

Director Shao’s expression turned grim as he looked at the ECG.

”Professor Shen, take a look... My initial suspicion is a myocardial infarction and heart failure!”

Shen Guofeng took it, and after a quick look, he reached the same conclusion.

At that moment, the patient’s condition deteriorated further, developing into a malignant arrhythmia accompanied by ventricular fibrillation!

”Get the patient to the OR, now! It’s an acute myocardial infarction with ventricular fibrillation. They could go into sudden cardiac arrest at any moment!”

Director Shao’s face paled. He knew the situation was critical. There was no time to determine the cause of the myocardial infarction and ventricular fibrillation; they needed to start cardiopulmonary resuscitation immediately!

Professor Shen and Director Shao began emergency resuscitation on the patient with the medical staff.

Shen Siyuan, Huang Dian, and Wu Ting followed behind.

”What’s this patient’s situation?”

Wu Ting asked, a little confused.

Shen Siyuan, her expression grim, explained anxiously:

”The patient is a 45-year-old female, admitted a month ago for sudden deafness in her right ear. Her initial symptom was tinnitus. An aortic CTA revealed a coarctation of the descending aorta. She has a history of hypertension, early-stage breast cancer, and coldness in both lower limbs. After walking for about forty minutes, she would experience lower limb weakness and intermittent claudication...”

”On physical exam, she had a regular heart rhythm, hyperactive heart sounds, and a distinct systolic blowing murmur was audible in the aortic and mitral valve auscultation areas...”

”An echocardiogram showed: an enlarged left atrium with an anterior-posterior diameter of 42 mm; an enlarged left ventricle with a diastolic anterior-posterior diameter of 57 mm; and symmetrical thickening of the left ventricular wall, with both the interventricular septum and the posterior wall measuring about 14 mm thick...”

”The final diagnosis was a congenital heart defect. She underwent an interventional procedure under local anesthesia to place a stent in her thoracic descending aorta. The surgery was successful...”

”She was supposed to be discharged today, but who knew she’d suddenly have a myocardial infarction... I’m not sure what caused it either.”

Shen Siyuan didn’t understand what was happening. All they could do was let Director Shao and Professor Shen stabilize the patient first before trying to find the underlying cause.

The principle of emergency care is to save the life first, then treat the illness.

While explaining to Wu Ting, Shen Siyuan handed him the patient’s file, which contained her previous records along with today’s physical exam results, ECG, and other data.

Previously, Wu Ting wouldn’t have known anything about the complications of cardiac stent procedures.

However, after his exchange with Lou Musaang, he had obtained her entry: Common Risks of Cardiovascular Stents (Yellow). Looking at the patient’s file, he actually had a few theories.

But with Director Shao and Professor Shen here, he wasn’t in a hurry to voice his ideas.

’I’ll just wait and see how things develop.’

Director Shao and Professor Shen handled the emergency treatment for the myocardial infarction with practiced ease.

After they finished the resuscitation, the patient’s condition was temporarily stabilized. They then left the trauma bay and moved to a consultation room to discuss the case.

In the conference room, Director Shao, Shen Guofeng, and Shen Siyuan began their discussion. The other doctors, like Huang Dian, had no useful input and could only listen quietly.

Director Shao was the first to offer his opinion:

”We can be almost certain that the patient’s myocardial infarction and ventricular fibrillation are related to the previous stent implantation. The question is, we don’t know the exact cause.”
