Chapter 193: Chapter 175: Coming to the Rescue, You’re That Big Shot

”Convert to an open abdominal procedure!”

Liu Xingye made a snap decision. At the same time, he had Wang Nanjin contact Director Shen Xiaofei to come provide backup. The patient’s condition was unusual; it was better to be safe.

Wu Ting’s expression was grave as he stood to the side. This was also the first time he had ever encountered a situation like this.

Removing gallstones, unlike treating simple cholecystitis, was far more complex. The patient also had a variant of Mirizzi syndrome, which meant their only option was an open procedure to perform an intraoperative exploration.

Liu Xingye calmly began the open procedure on the patient.

Once inside, he performed a routine exploration. What he saw made his scalp tingle.

Just as he’d suspected, there was a 7x5 cm mass in the patient’s gallbladder region.

Liu Xingye took a deep breath and began to explore it. It was a cystic structure filled with stones and surrounded by adhesions, with the relationship on the left side being particularly unclear.

This was also the first time Liu Xingye had encountered such a case.

”The patient’s triangle of Calot has disappeared. The common bile duct isn’t visible, and the stomach wall is adhered right to it...”

Liu Xingye took another deep breath and began the routine dissection of the triangle of Calot. However, with the structure gone, the dissection was incredibly difficult.

Under normal circumstances, this fused cystic mass should have been the gallbladder, so Liu Xingye began to dissect it.

But the surrounding adhesions were severe and transected the area. Liu Xingye, ultimately lacking experience and unable to guess the true cause, forced the dissection.

With just one cut of the scalpel, Liu Xingye was suddenly horrified. ’This is bad,’ he thought, but it was already too late!

Bile immediately began to leak out. A fistula, about 2 cm wide, had appeared, and there was no way to close it on the spot...

Only after making that cut did Liu Xingye realize what the problem was.

”Wu Ting, it’s a disaster. I’ve caused a major complication.”

”This patient has a rare fusion of the common hepatic duct, common bile duct, and gallbladder. Because the three have fused together, the normal anatomy of the triangle of Calot is gone.”

”A moment ago, I mistook this mass for the gallbladder and started dissecting it. In reality, I sliced right through the common hepatic and common bile ducts!”

”Now the common hepatic and common bile ducts are ruptured and need to be repaired, but this fistula is too large!”

Liu Xingye’s mind went completely blank.

”Has Director Shen arrived yet? Go get him, quickly! This is a matter of life and death. I’ll try to control the bleeding and do what I can to repair it in the meantime!”

Although Liu Xingye had caused a disaster, he was still responsible. He immediately prepared to remedy the situation.

The video he’d watched that morning on ”Emergency Treatment for Traumatic Liver Rupture” flashed through his mind. It included liver repair and a jejunal anastomosis for the common hepatic and common bile ducts.

While this patient’s condition was even rarer, the level of danger wasn’t as high as in that other surgery. ’If I can just imitate that Technician, I should be able to hold out until Director Shen Xiaofei arrives.’

Liu Xingye tried to psyche himself up, but his heart was filled with regret. ’If only I had watched it a few more times instead of just glancing at it, marveling at how awesome it was, and then saving it for later.’

’If only that Technician were in this operating room right now.’

Just as Liu Xingye was trying to imitate that Technician, Wu Ting, who had been standing to the side, sprang into action.

”Dr. Liu, it’s okay. This patient’s condition isn’t that serious. Let me...”

”First, we’ll irrigate the patient’s liver to wash away the bile. Next, we’ll resect the gallbladder and then repair the fistula. For that, we’ll use a Roux-en-Y choledochojejunostomy...”

”Because the patient’s common hepatic duct, common bile duct, and gallbladder are all fused, we’ll need an external repair material to compensate for the missing sections of the ducts...”

Under the influence of his Grandmaster Level Common Bile Duct Repair technique, a flash of inspiration gave Wu Ting the perfect repair plan.

”Dr. Liu, assist me. Prepare a pedicled gallbladder flap. I’ll use it for the repair.”

At Wu Ting’s calm analysis, it all suddenly clicked for Liu Xingye.

”Yes, yes, exactly! Good, good!”

Liu Xingye voluntarily gave the chief surgeon’s position to Wu Ting.

And Wu Ting began his work.

Liu Xingye silently cheered him on, watching with rapt attention from the side.

Wu Ting first dissected the adhesions on the right side of the cystic mass, separating it from the hepatic region, then made a longitudinal incision into the cystic wall...

The mucous membrane was coarse and filled with stones—four in total. The largest was 4x3 cm, another was 3x2 cm, and the last two were 2x2 cm. After they were removed, clear bile could be seen flowing out from two small openings at the top of the cystic cavity...

At that moment, Director Shen Xiaofei and Wang Nanjin rushed back into the operating room.

Seeing that Wu Ting and Liu Xingye had switched places at the operating table, with the latter not daring to even breathe, Wang Nanjin was rather surprised.

However, just as he was about to call out, Shen Xiaofei stopped him.

Wang Nanjin could only let Director Shen go ahead while he followed behind to watch.

Then, the two of them saw the situation in the surgical field.

It was the first time Wang Nanjin had seen anything like it. ’Holy shit,’ he exclaimed inwardly. ’The common hepatic duct, common bile duct, and gallbladder are all fused... After being cut open, this is a huge problem! How can you possibly repair the bile and hepatic ducts?’

Wang Nanjin was completely at a loss for how to handle such a situation.

He could only look to Director Shen Xiaofei. However, a trace of amazement and admiration appeared in the director’s eyes. He simply let Wu Ting continue operating, showing no intention of stepping in.

At the operating table, Wu Ting was completely focused and didn’t notice Director Shen and Wang Nanjin watching from the sidelines.
