Chapter 163: Chapter 151: NanJin’s ERCP Training Camp

Having obtained the purple-tier Endoscopic Retrograde Cholangiopancreatography (ERCP) skill from Dean Liu Rong, Wu Ting was naturally not the least bit intimidated by Wang Nanjin.

Wu Ting was also looking forward to seeing just how skilled this elite young physician from the Hepatobiliary Surgery Department of Jinggang University Affiliated First Hospital truly was.

ERCP is an examination and treatment method developed by combining endoscopic technology with the local anatomy of the biliary tract.

The principle behind the technique is simple, but mastering it requires a profound clinical foundation. Once mastered, it can serve as a fundamental technique, combined with others, for a wide range of applications, such as diagnosing and treating conditions like biliary stones, biliary tumors, and biliary stenosis.

It is also a milestone technique in the field of laparoscopy.

That afternoon, Wang Nanjin’s team continued to lecture and practice the technique with the doctors from City First Hospital’s Emergency Department.

Having demonstrated a live procedure for the Emergency Department doctors in the operating room that morning, Wang Nanjin could tell from their reactions that they still lacked a deep understanding and systematic knowledge of ERCP.

He had the next week to work with them, and Wang Nanjin planned to leave a deep impression on the doctors at City First Hospital, as well as get the expert exchange week off to a good start.

To ensure the training program ran more smoothly, Wang Nanjin intentionally added Dr. Yan Xi to his teaching group.

Wang Nanjin and Yan Xi had quite a few similarities and were fairly familiar with each other from their time at Haijin Port. Soon, they were both scheduled to speak at the graduate school’s new student orientation as representatives of outstanding young doctors and outstanding graduates.

Wang Nanjin planned to include his experience from this expert exchange week at City First Hospital to enrich his resume. A member of his team was even responsible for photographing the entire class.

Once the trainee doctors from City First Hospital’s Emergency Department were seated.

Wang Nanjin, with a debonair flair, began to impart his knowledge.

”Dr. Yan, let’s begin.”

Yan Xi was junior to Wang Nanjin in both academic and professional standing, so during this expert exchange week, she was naturally expected to cooperate and follow his arrangements.

The title of the course was displayed on the slide:

”Nanjin’s ERCP Simulator Training Camp Lecture: From Novice to Competent to Master in One Week!”

The lecture’s cover page was very simple: a single line for the title, below which was a professional portrait of Wang Nanjin. The bottom right corner was filled with Dr. Wang’s brilliant resume.

Wu Ting, along with the other Emergency Department doctors, quietly watched Dr. Wang’s lecture.

’I have to admit, this Wang Nanjin really knows how to package himself. There’s some substance to this course,’ he thought.

Given Wu Ting’s skill level, he naturally didn’t need this kind of training course, but he wanted to understand the thought processes and technical proficiency of a young doctor like Dr. Wang.

After using the cover to highlight himself, Wang Nanjin moved on to the first section: the foreword.

”ERCP is an advanced endoscopic procedure. Among gastrointestinal endoscopies, it is the most technically demanding and carries the highest risk. ERCP requires specialized skills and knowledge, and it has a higher risk profile than other endoscopic operations. Studies show ERCP has a complication rate of 6.85% and a mortality rate of 0.33%...”

Listening to Wang Nanjin’s introduction, the doctors present felt a sense of gravity and anticipation.

Wu Ting was a little impressed. ’This Wang Nanjin is quite good at structuring his course and stirring up emotions.’

After introducing the difficulty and risks of the surgical technique, Wang Nanjin casually shared some of his own insights:

”Therefore, an excellent ERCP operator must receive adequate training to ensure the safety of the procedure. Otherwise, it will severely affect the success rate and the incidence of complications.”

”Statistical analysis of extensive data shows that ERCP has a long learning curve and a high potential for complications, such as a 50% increased risk of post-ERCP pancreatitis. Furthermore, most doctors don’t have enough patients to practice on. Therefore, traditional observational learning is a very inefficient and passive method.”

”For this reason, my team and I have developed a brand-new assisted training method for ERCP. It doesn’t require extensive live practice; the teaching can be done on a simulator. Moreover, our team—which I call the teaching group—is constantly learning new techniques and theories and exploring new methods. We also hope that through this exchange week, we can jointly explore new possibilities for ERCP.”

Wang Nanjin spoke with sincerity, combining his personal insights with his glorious resume and the endorsement of the Jinggang University platform. It was indeed very persuasive and captivating.

Even a senior doctor like Qin Donglai was completely drawn in by Wang Nanjin’s course, beginning to learn with earnest and humble attention.

As for people like Yasui and Luo Ming, they already worshipped a genius doctor like Wang Nanjin, who seemed to have a natural halo.

They felt incredibly fortunate to be able to learn from Dr. Wang’s original ERCP training camp, as if they’d found a priceless treasure!

In the classroom, only Wu Ting listened in a state of scrutiny and evaluation. He didn’t care about all this superficial fluff; he just wanted to know Wang Nanjin’s true skill.

Wang Nanjin moved to his next slide:

”So, how do we implement this new teaching model? We need to abandon the traditional apprenticeship model of learning through clinical trial and error, which increases patient discomfort, risk of complications, and prolongs procedure time.”

”The simulator we use is an internationally cutting-edge Erlangen Endo-Trainer, a biological simulation model. It uses a realistic approach, employing real endoscopes and accessories with pre-programmed and recorded images to perform ERCP operations. The results are exceptionally effective.”

”During the ERCP simulation, we require everyone to complete six tasks in order:”

”The first task is to establish the correct endoscope position in front of the papilla to obtain the proper cannulation axis...”

”The second task is, during cannulation of the common bile duct, to record the number of pancreatic duct cannulations. The same applies to pancreatic duct cannulation and unintentional common bile duct cannulation...”

”Next, participants are required to place a plastic stent in the pancreatic duct and a plastic stent in the bile duct...”

”Finally, everyone must be able to retrieve a stone from the common bile duct...”

Hearing Dr. Wang’s explanation, everyone was eager to give it a try.

To motivate the crowd and stir their competitive spirit, Dr. Wang also included his own records for the six tasks.

”Since this training course was developed, no one has even come close to Dr. Wang’s six records.”

The teaching assistant at his side announced proudly.

”For the next week, we will all be training on these six techniques. Only those who pass the assessment for these six techniques will be allowed to go to the operating table and perform a live procedure under Dr. Wang’s supervision!”

”Of course, we also welcome our colleagues from City First Hospital to challenge our Dr. Wang!”

The assistant declared this breezily. After all, in their estimation, it would be a mark of genius if these doctors from City First Hospital could simply complete the six tasks within a week.

Approaching, let alone surpassing, Dr. Wang’s records was considered impossible.

”Naturally, during this week, in addition to simulator training under our teaching group’s guidance, Dr. Wang will still be performing a real ERCP surgical technique every day. You are all free to observe and learn as you choose.”

”But the same rule applies: only when you can accomplish these six tasks will you be able to truly follow Dr. Wang for clinical practice.”

Dr. Wang’s arrangement was made with the risks of ERCP in mind. If they couldn’t even complete these six simulator tasks, allowing them to operate on patients would be highly irresponsible.

Qin Donglai and the others were completely won over by Dr. Wang’s course. They were all preparing to practice hard in the coming days to meet the graduation standard as soon as possible.

Being able to truly master ERCP would be a milestone event for City First Hospital’s research center for laparoscopic diagnosis and treatment techniques.

Only Wu Ting remained uninterested. After all, practicing on a simulator like this was completely meaningless for him.

’I’ll just wait until I have time to personally observe Dr. Wang’s surgery.’

Wu Ting turned to Qin Donglai beside him and said:

”Director Qin, I have a few things to take care of over the next few days, so I won’t be participating in the ERCP simulator training course. When the time comes, I’ll go observe Dr. Wang’s surgeries.”

Wu Ting had no interest in challenging Dr. Wang’s records; there was no sense of accomplishment in it for him. Nor did he want to waste time in a simulation class like this.

’Still, I could try these six tasks when no one’s around and see where my limits are.’

’After all, if I go up and perform now and accidentally fail to hold back my true strength, how could Dr. Wang’s course possibly continue?’

With his purple-tier skill, Wu Ting’s level would absolutely crush Wang Nanjin’s.

Wu Ting prepared to slip away using the ”I have to pee” excuse.

”Are there any doctors from City First Hospital who are ready to give it a try? We’ll start with the first task!”

Wang Nanjin wanted to find someone for a live demonstration, using their failures or mistakes as a negative example to make a more lasting impression.

Wang Nanjin’s teaching team was also looking forward to Dr. Wang’s lecture. They wanted to open the eyes of the City First Hospital doctors and make them directly perceive the gap between themselves and Dr. Wang.

After all, Dr. Wang’s established records were just a bunch of data. Only through a real comparison would they understand the difference.

Just then, Wu Ting stood up to make his escape.

”You, Doctor. Would you like to come up? Are you interested?”

Wang Nanjin, of course, recognized Wu Ting. In previous conversations with Dr. Yan Xi, he had heard her repeatedly praise this doctor from City First Hospital. Their department head, Shen Xiaofei, also frequently mentioned Wu Ting and even wanted to take him on as a graduate student.

For an ordinary young doctor, Wu Ting’s current achievements were astounding enough. But to someone like Wang Nanjin, they were nothing special.

Therefore, Wang Nanjin had intentionally chosen Wu Ting.

The doctors from City First Hospital also looked at Wu Ting, wondering what he would do.

After all, in the hearts of many junior doctors, Wu Ting was the last bastion of pride and glory for their research center.

Dr. Yan Xi was also a bit curious. She wanted to know how quickly Wu Ting would progress on the ERCP simulator.

”Dr. Wang, I’m sorry, maybe next time. I need to use the... facilities.”

Wu Ting pretended to blush. It was their first meeting, after all; it would be rude to just crush the guy right off the bat.

Besides, while Dr. Wang’s training camp was useless to him, it was very helpful for the other staff at City First Hospital.

’If I perform too well, and Dr. Wang feels so inadequate that he just packs up and leaves, wouldn’t that be a disaster?’

So, Wu Ting used a not-so-dignified excuse.

Wang Nanjin was a little disappointed but couldn’t do anything about it—the man had already played the bathroom card. It just meant that Wu Ting didn’t make a very good first impression on him.

The members of the teaching team also found it lame. ’Did this Wu Ting just chicken out? Still, you can run, but you can’t hide. You can’t use the bathroom excuse every day unless you just stop coming to class altogether!’ they thought. But since they were on his home turf, they couldn’t very well taunt him.

Dr. Yan Xi, on the other hand, had full confidence in Wu Ting. It was just unfortunate timing.

The people from City First Hospital also felt it was a bit of a shame.

”Dr. Wang, I’d like to give it a try.”

Cheng Feng stood up directly, saving Wu Ting from his ”predicament.”

Wu Ting secretly gave Cheng Feng a thumbs-up. Thanks to Cheng Feng, Dr. Wang’s awkward situation was resolved, and the training camp wouldn’t crash and burn before it even got off the ground!

Just as Cheng Feng was preparing to take Wu Ting’s place and be the first to try, Xiao Xixi rushed in from outside the classroom.

”Director Qin, Dr. Wu! In the ER, Dr. Song has a patient with a complicated situation. It’s suspected to be cystic hepatic echinococcosis, which is very rare in our region! But Dr. Song’s diagnostic ability for this disease is limited.”

”So, please, Director Qin, Dr. Wu, come for a consultation.”

”Cystic hepatic echinococcosis?”

Hearing the name, Qin Donglai’s expression turned grave. Even if this condition was diagnosed, not just anyone could perform the surgery for it.
