System Came When the Doctor was Thirty

Zhang Tiesheng felt a slight sense of crisis and touched his own chest with his right hand. ”What’s this?”

”Xiao Lu, it’s you who has spoiled the atmosphere in our department. We were never this competitive before.”

Although Zhang Tiesheng said that, he hasn’t been slacking off in practice recently.

We could even say that Zhang Tiesheng’s suturing skills are now second only to Lu Cheng’s in the entire department.

Previously, people like Tian Zhuang, Lin Qianlong, and Zeng Huanqi, who had better basic suturing skills than him, have all been quietly surpassed by Zhang Tiesheng.

The foundation is there; if Zhang Tiesheng were ten years younger, Lu Cheng even suspects that Zhang Tiesheng could perform all the upper limb tendon sutures as the main surgeon by now.

Of course, Zhang Tiesheng is not in a hurry to start performing tendon surgeries in the department.

According to Zhang Tiesheng, as long as Lu Cheng is around and serves as a role model, Zhang Tiesheng won’t thoughtlessly steal Lu Cheng’s specialized surgeries.

Tian Zhuang doesn’t have any surgeries of his own, performing tendon sutures, so Zhang Tiesheng wouldn’t snatch them away either.

After all, during Zhang Tiesheng’s shifts, he can still conduct appendectomies and cholecystectomies.

Han Xiaoping also nodded, ”Brother Lu, you’ve messed up the atmosphere, especially for Brother Wu and Tian Zhuang. I heard from him that sometimes he only sleeps four or five hours a day.”

”Brother Lu, can Tian Zhuang basically handle all the upper limb tendons now?”

Lu Cheng nodded, ”Except for woven sutures, Tian Zhuang does quite well with other tendon sutures.”

”But he still needs some time to perfect his flexor tendon sutures in Zone 2.”

”For flexor tendon ruptures in Zone 2, strict standards must be followed before attempting anything.”

”Orthopedic doctor Peng Kun failed in this type of surgery.”

The emphasis from Lu Cheng was directed at Zhang Tiesheng.

Letting Zhang Tiesheng continue to refine his basic suturing skills might take longer, but it will make him more adept in future clinical operations.

Actually, Zhang Tiesheng is quite a tough guy; in less than a month, his suturing skills have improved rapidly.

This proves that although Zhang Tiesheng acts carefree in the department, he puts in a lot of effort in private and probably invests most of his time in technical practice.

Just as Lu Cheng was about to respond, the harsh sound of a rolling cart came from outside.

”Doctor, doctor!~”

”Help!”

”Doctor,” a middle-aged woman’s voice called out in the hall outside.

Lu Cheng and the others immediately stood up and quickly ran towards the consultation room’s exit.

At a glance, Lu Cheng and the others saw the man on the cart, his abdomen covered in blood, his face pale, being guided by the emergency room nurses and other family members into the emergency room…

”Dr. Lu and the others are here,” a nurse informed the middle-aged woman after spotting Lu Cheng and his team.

”What happened here?” Lu Cheng asked, while signaling Han Xiaoping to help position the patient’s cart.

”My husband was walking home at night and fell. Something pierced him, and I couldn’t lift him up alone.”

”I called for help, and they managed to drag him up, but it’s a belly full of blood…”

”Is there anything below?” Lu Cheng queried.

”Someone had chopped wood, and the sharply cut stump just pierced into his abdomen. My husband turned over by himself, or we wouldn’t have dared to move him.”

”Doctor, please save him…” the middle-aged woman pleaded.

After she finished, another middle-aged person helping with the bed said, ”Doctor, how’s my brother?”

Zhang Tiesheng immediately interrupted, ”Don’t speak; we’re assessing…”

”Blood pressure, 75/50, oxygen saturation 89, body temperature 37.9!” The nurse quickly reported the basic vitals.

”There’s no time for a CT scan; call the blood bank,” Zhang Tiesheng looked toward Lu Cheng.

Lu Cheng then addressed the middle-aged woman, ”Your husband’s condition is very critical and he might not make it.”

”Now you have two choices: first, transfer him to a higher-level hospital, or second, take him to the operating room and see if we can save him.”

The middle-aged woman immediately panicked, trembled, and froze.

”What’s your decision?” Lu Cheng raised his voice.

The middle-aged woman remained silent, leading another middle-aged man, possibly the patient’s brother, to ask, ”What’s the chance of saving him?”

”The situation is urgent and hope is slim. However, given his condition, if you transfer to a higher-level hospital, there’s a good chance he wouldn’t make it there.”

After answering, Lu Cheng shouted to the nurse, ”Prepare for intubation! Call the operating room, surgery is imminent.”

The patient’s oxygen saturation was very low, and simple oxygen inhalation was inadequate now.

Endotracheal intubation along with timely abdominal bleeding control were necessary to possibly stabilize his vital signs.

”Doctor, what’s actually wrong with my brother?” the middle-aged man asked.

”It’s trauma, bleeding, shock; it’s quite apparent, but precisely, we can’t evaluate without X-rays,” Lu Cheng replied and continued instructing another nurse. ”Prepare gauze and cotton pads! Temporary packing for hemostasis first.”

Han Xiaoping quickly assessed the patient’s state, ”The patient is comatose, in shock. Lips are pale, and the blood color in the fingertips and thumb is very poor.”

”Pupils are slightly dilated, with prompt and symmetrical light reflex coordination.”

Zhang Tiesheng, after briefly listening to the chest, stated, ”No dullness in the lungs.”

”You need to decide quickly,” Lu Cheng urged the relatives again.

”This situation can’t be handled in the emergency room.”

”Gauze and cotton pads are here.”

”Intubation is ready.” Two nurses arrived at the bedside almost simultaneously.

Zhang Tiesheng, accompanied by Han Xiaoping, proceeded with intubation and instructed, ”Prepare epinephrine, norepinephrine drip, open bilateral venous access…”

”Doctor... doctor…” At that moment, a call from outside was heard.

”Dr. Zhang is in the emergency room, please wait a moment,” the guiding nurse said.

”I’m in such pain,” cried an external patient, ”Where’s the doctor?”

Zhang Tiesheng and Lu Cheng ignored it and continued to face the patient’s relatives, ”Speak up? Every minute wasted reduces his chance of survival.”

The chaotic surroundings added to the confusion of the middle-aged woman, who finally snapped back to reality with a rush:

”Doctor, you have to save my husband...”

”Surgery is the only hope; without it, he will definitely die.”

”There’s also a high risk with transferring; what’s your choice?” Lu Cheng stared sharply, prompting a direct response.

”Surgery, we choose surgery, you must save my husband, doctor, please.” The middle-aged woman hurriedly agreed.

”Han Xiaoping, take them for pre-surgery counseling and consent signing, call Director Lin for a general surgery consultation.”

”We’ll take the patient to the operating room first!~”

”This patient has an active bleeding abdominal injury: he’s already in shock!~” Lu Cheng provided a presumptive diagnosis.
