System Came When the Doctor was Thirty

Lu Cheng said, ”Of course, I want to rest too, but you don’t know that during night shifts, just after midnight and around six or seven in the morning is when we have the most patients.”

”It’s work, setting up stalls… there might be more accidents...”

Du Yali picked up her phone and said, ”Sister Juan took a lot from the storeroom, it’ll definitely be enough for today.”

”Dr. Lu, you’re so fast at this. You’ve done about nine already, right?”

”Yes, nine.”

”It’s just my bad luck and the patients’ too, I guess,” Lu Cheng smiled.

”Let’s hope no more patients come…” Du Yali didn’t even have the heart to play with her phone anymore and just leaned against the wall, closing her eyes to rest.

However, Lu Cheng also planned to rest for a while. But unexpectedly, before he could put his phone back, a loud rolling sound came from the direction of the emergency department hall.

And someone was shouting loudly, ”Traumatic intra-abdominal hematoma. Push to the resuscitation room!”

”Oxygen, cardiac monitoring, check vital signs, establish an intravenous line!” a doctor in the resuscitation room immediately shouted.

”Call Lu Cheng from surgery.”

Lu Cheng was already up, quickly put on a mask, and rushed towards the resuscitation room.

When Lu Cheng saw the patient, the patient already displayed typical signs of a frog-like abdomen, an indication of significant intra-abdominal fluid accumulation.

The paramedic in green from the ambulance said, ”When we picked the patient up, the abdomen wasn’t this distended. There’s definitely active bleeding.”

”Get a surgical consultation immediately and send to the operating room as soon as possible.”

”Beep beep beep…” the blood pressure monitor sounded a continuous alarm.

”Systolic pressure 70!” said the nurse.

The lower the blood pressure, the shorter the working time of the blood pressure monitor, and the faster the data is gathered.

Lu Cheng had already reached the patient’s side. After checking the patient’s eyes and finding no changes in the pupils, he immediately said, ”Quickly secure them here; we need to open the abdomen for emergency hemostasis.”

”Rapid fluid infusion after establishing the intravenous line!”

”Du Yali, help me call the chief on duty. I need to do an exploratory laparotomy here.” Lu Cheng directly assigned her this task.

”Okay, I’ll call right away.” Du Yali walked to the side.

”Heart rate 144! Blood oxygen 91%!” The nurse reported more alarming data.

”Has the intravenous line been established? Administer norepinephrine intravenously!” said Deputy Director Long Chunbo from internal medicine sternly.

”Still searching for a vein!” a nurse quickly replied.

”No airway obstruction found on site!” The ambulance doctor observed the airway while signing off the handover with Long Chunbo.

”It still needs intubation!”

”Order an emergency ultrasound, get a general surgery consultation, call hematology.” Long Chunbo continued issuing rapid medical orders.

”Take pre-transfusion tests, blood typing, liver and kidney function...”

Standing by, Lu Cheng calmly pulled the patient’s shirt up as far as it would go.

Fortunately, the patient was male, so privacy concerns were less of an issue, saving Lu Cheng at least ten seconds of critical rescue time.

Lu Cheng took iodine solution and began pouring it around the patient’s navel, drawing larger and larger circles. The dark iodine solution flowed over the patient’s abdomen.

”Scalpel.” Lu Cheng turned his head.

”The debridement kit hasn’t been opened yet,” a nurse said.

Lu Cheng had to get the debridement kit himself, violently ripping it open, and then casually wiping the patient’s abdominal wall with gauze from inside, before beginning with sterile blades.

The suction machine uses a negative pressure aspirator, so Lu Cheng said, ”Later insert a negative pressure suction tube.”

”Record the time, 5:28 AM, the patient is in hypovolemic shock due to massive intra-abdominal bleeding, proceeding with emergency hemostatic surgery.” As Lu Cheng spoke, his hands did not stop and directly incised the patient’s abdominal wall.

His swift and decisive action made Long Chunbo frown deeply.

Today was not the first time he worked with Lu Cheng, but before today, Lu Cheng had always followed protocol, even with such patients, waiting for a surgical consult and then transferring to the operating room.

Performing a laparotomy right in the resuscitation room was unheard of.

However, Long Chunbo had obviously heard about Lu Cheng; the hospital had actively sought him out and granted him authorization to perform exploratory laparotomies and emergency hemostasis in the operating room and resuscitation room.

”Dr. Lu, I reported to the chief on duty for authorization. They said,” Du Yali approached.

”Gloves on, retractors!” Lu Cheng interrupted, not letting Du Yali waste any more time!

Having confirmed authorization, the chief won’t refuse, there would certainly be other instructions, but those weren’t important.

The family isn’t present, the police haven’t arrived, and there’s no way to get a signature.

As for documentation such as consent forms and critical condition notices, they’ll have to be completed later.

If resuscitation fails, the family will come to claim a death certificate and the resuscitation process record.

Lu Cheng was quickly exposing the site, soon opening the patient’s abdomen. But without an abdominal retractor in the resuscitation room, Lu Cheng had to brace one end of the retractor with his abdomen for leverage.

With no one on the other end of the retractor, Lu Cheng had only a partial view to operate.

The residual blood in the abdomen was substantial, but the bleeding rate wasn’t torrential; otherwise, considering the patient’s lengthy transfer time, they would have lost the chance for resuscitation long ago.

After a few glances, Lu Cheng roughly determined the bleeding location to be in the upper abdomen, and began exploring patiently!

But soon, Lu Cheng’s expression drastically changed, ”Pancreatic rupture, retroperitoneal hematoma.”

”Quickly establish dual intravenous lines,” Lu Cheng shouted.
