System Came When the Doctor was Thirty

”If he could help a bit, applying for funds might not always face obstacles.”

The ease of gaining resources and the underlying needs of individuals are different.

”Da-da-da!~” Just as Lu Cheng was about to respond, there was a knock on the door from outside.

”Alright, Brother Zhang, if there’s nothing else, let’s hang up for now. We’ll discuss this later; I’m on duty as well,” Lu Cheng said.

”Alright, I just wanted to inform you as soon as possible, so you’d be aware,” Zhang Tiesheng hung up voluntarily.

Lu Cheng opened the door and saw that the one knocking was the on-duty nurse, Qin Xiaoli: ”Xiaoli, is there a new patient?”

Qin Xiaoli replied, ”It’s not a new patient. The operating room called to say that the first patient just sent in has died despite resuscitation efforts.”

”The family hasn’t arrived at the hospital yet. The operating room wants our emergency room and department to prepare accordingly.”

”The medical department and the general on-duty staff will be coming over later.”

Lu Cheng’s eyelids fluttered as he replied, ”Okay, I’ll head out right away.”

”I just had Tan Yueyue send his medical records into the operating room. Call Tan Yueyue and remind her to bring out the medical record books as well.”

In an emergency, you definitely can’t wait for a signature before performing resuscitation. Without family present, the hospital must authorize emergency procedures without a signature.

But even this kind of resuscitation carries risks. If the patient can’t be saved, we might need to explain things to the family with the help of traffic and police departments.

This is human nature; you can’t just let someone die in the hospital without an explanation.

Qin Xiaoli replied, ”Sister Lu has already called Yueyue. I’m just here to inform you that Director Chen from Internal Medicine is already reviewing the medical records.”

”Then I’ll go find Director Chen.” Lu Cheng immediately headed out, moving towards the workstation computers in the direction of the resuscitation room.

Chen Changlin was the deputy senior physician in the emergency internal medicine department and the supervising doctor on duty in the resuscitation room today.

When Lu Cheng arrived, Chen Changlin was already reviewing the medical records he had previously written and the signed documents. Lu Cheng called out, ”Director Chen...”

Upon hearing this, Chen Changlin nodded with a smile, ”Xiao Lu, your work is meticulous. You’ve updated the medical record up to the last measurement of vital signs before the patient entered the operating room.”

”From the situation at the time, the patient had the emergency surgical indication necessary to enter the operating room for treatment.”

Lu Cheng replied, ”The nursing teachers recorded everything in detail. I just faithfully documented what they noted.”

”Also, I reviewed the internal medicine teachers’ records and learned a bit.”

Chen Changlin said, ”It’s good to be eager to learn!”

”Anyway, the procedures we performed in the emergency resuscitation room were standard. What changes might occur in the operating room is a matter for the anesthesiology and surgical teams, which is beyond our reach.”

”We just need to do our reporting work well.”

”Okay, Director Chen.” Lu Cheng nodded.

As he responded, Lu Cheng’s phone rang again; it was the delivery Liang Jianhong ordered. After answering, Lu Cheng instructed to leave it on the emergency department’s food station.

Chen Changlin said, ”You should quickly call Director Xiao. For such matters, it’s better if he, as the senior second-line shift, addresses it.”

Today was Lu Cheng’s evening shift in the resuscitation room and Xiao Zhaoxi’s second-line shift.

Lu Cheng said, ”I sent Director Xiao a message when I arrived, and he’s already replied.”

”That’s fine then,” after saying this, Chen Changlin called out, ”Zhou Xinghong, please print our operation records and medical records...”

...

Xiao Zhaoxi arrived almost simultaneously with the general on-duty and medical department teams.

However, they didn’t question Lu Cheng and others about why the patient died. If a patient dies in the resuscitation room, Lu Cheng and Chen Changlin need to explain, but if in the operating room, the final attending doctor is responsible for explaining the cause of death.

After recording the data on the emergency department side, the general on-duty and medical department immediately headed to the operating room.

Furthermore, the general on-duty immediately contacted the 120 referral center to find out where this patient was transported from, as the traffic police might need to testify that the patient was seriously injured at the accident scene...

After taking the medical record documents prepared by Lu Cheng and Chen Changlin, Xiao Zhaoxi glanced at Lu Cheng casually and asked in a low voice, ”This patient’s cause of death was shock caused by active bleeding.”

”If it were you on the operating table, could he have been saved?”

The resuscitation room isn’t an operating room and can’t achieve complete hemostasis.

So, the resuscitation room can only perform emergency hemostasis, major artery hemostasis due to bleeding.

Lu Cheng shook his head, ”I don’t know, Director Xiao.”

”There’s no hypothetical situation for things that haven’t happened.”

Xiao Zhaoxi said, ”But do you know that for patients like this, being transported from the scene to the hospital is palliative care, and the resuscitation in the resuscitation room is just formality?”

”In the past, they basically never had the chance to enter the operating room.”

Lu Cheng replied, ”Director Xiao, when I was at Long County People’s Hospital, such cases were transferred to higher-level hospitals. So, I’m not very familiar.”

Xiao Zhaoxi shook his head and sighed, ”Lu Cheng, there’s no denying your personal abilities are indeed strong.”

”But clinical is clinical, and rules are rules.”

”You can break the norm personally, but the norm won’t break its baseline for you, so we must endure when necessary.”

”Our hospital’s emergency physicians aren’t responsible for many emergency surgeries, as there are specialist doctors after all.”

If it were just once or twice, Xiao Zhaoxi felt it was common; after all, it used to be this way.

But this was the third time under Lu Cheng’s care where he nearly brought someone back from Yama Hall, only for them to slip back.”

Xiao Zhaoxi wasn’t heartless; naturally, he felt a bit emotional as well.

Xiao Zhaoxi believes that if Lu Cheng could follow the patients to the operating room, while not all three could be saved, it’s conservatively estimated that at least one more could be saved.

Lu Cheng said, ”Director Xiao, no matter how strong individual abilities are, they can’t stretch the average level.”

”No matter how good the techniques at State People’s Hospital are, they can’t significantly affect the average level across the state; on the contrary, the average level is determined by the majority’s average level.”

”So, I’ll just do my duty.”

...

About two hours later, the deceased patient’s family finally arrived at the hospital. Upon hearing the bad news, they instinctively questioned whether the treatment was untimely in the emergency department.

After learning that their family members had entered the operating room but still hadn’t been saved, they began to suspect whether the chief surgeon’s skills were lacking.

Their questions were a bit irritable, which caused a deputy director from the trauma surgery department to get upset: ”Not all diseases can be treated, and not every patient sent to the hospital can walk out alive.”

”Why are you blaming us instead of the person who caused the accident?”

”What? Was I not on duty today, went out driving, and hit someone? Was it me who hit them?” Nie Ping from the trauma surgery department laughed mockingly.

The patient’s family was provoked and angry: ”What kind of attitude is this? Not being able to save the person proves your skills are lacking.”

Nie Ping said, ”My skills are lacking? Then get someone skilled over. My skills are lacking!”

”If you’re skilled, why didn’t you come?”

The safety office personnel quickly pulled Nie Ping away, pushing him to one side, and looked to the general on-duty person: ”Old Tao, you should explain things to the patient’s family, and I’ll take Director Nie to calm down.”

”What kind of attitude is this?” The group of patients surrounded Old Tao from the safety office.

Tao Long smiled and said, ”Director Nie is our deputy chief physician in trauma surgery; his technical level is undoubtedly superb!”

”The injuries were too severe; you can’t expect every injury to be salvaged! Otherwise, why are there still car accident deaths worldwide?”

”There was a lot of blood loss on-site, during transfer, and in the resuscitation room. By the time they reached the operating room, the chances were already slim...”

Tao Long understood the safety office’s approach; the trauma department in orthopedics has the highest rate of medical disputes, but the lowest rate of physical contact disputes.

The main reason is that people in the trauma department have explosive temperaments and are physically robust, which could lead to fights easily!

Nie Ping, able to become a deputy director in trauma orthopedic surgery, surely has had his share of fights.

”We didn’t even see the person, and now they’re gone!~ How do we accept this?” the family continued to argue.

Fortunately, the hospital had already informed the traffic police and the public security departments; a traffic police officer from the scene also came to help explain, and the patients and their families were taken to the safety office’s office.

The noise gradually diminished...

None of this concerned Lu Cheng, who merely sat in the resuscitation room, listening to the internal medicine group’s resuscitation efforts, quietly turning pages amidst the chaos.

Now, waiting for emergencies and reading books are his duties.
