System Came When the Doctor was Thirty

In the emergency operating room, the shadowless lamp shines on the patient’s oversized lower limb’s posterior side, illuminating the surgical field clearly.

Lu Cheng, holding the needle holder and forceps, stands at the main operating position. After Peng Kun has pulled open the skin and subcutaneous tissue layer, he patiently says, ”The Pulvertaft weaving method, also known as the fish mouth suturing method, is quite an unconventional suturing technique with a very narrow application range.”

”It is specifically aimed at tendon suturing techniques with varying diameters. This patient’s tendon rupture occurs at the gastrocnemius-soleus transition, where these two tendons have different thicknesses.”

”Moreover, tendon rupture rarely happens at this location, so Professor Tong normally doesn’t spend time learning it, and I picked it up...”

Peng Kun, with his head down, carefully examines the rupture Lu Cheng is showcasing with forceps, nodding constantly: ”Professor Tong indeed mentioned that ordinary suturing techniques are not optimal for tendon suturing where tendon thickness varies.”

”Turns out the fish mouth suturing method is the most practical. Brother Lu, you are truly impressive—after learning so many techniques, you still have the spare time to learn such rare skills.”

”I salute you!~”

Although Lu Cheng didn’t boast, this calmness represented his talent.

Everything else had been learned, Tong Yuan’an even became ’withdrawn,’ angrily claiming Lu Cheng didn’t count as human; Lu Cheng, who used his spare time to learn ’narrowly applicable’ unconventional techniques, indeed has the talent that makes one ’unflappable.’

In the short term, Peng Kun doesn’t plan on learning this.

Lu Cheng thought for a moment and continued: ”Young Master Kun, the entry barrier for the Pulvertaft weaving method is relatively low, quite suitable for you to learn, and if you wish to, I can send you the materials.”

”If you encounter it in the future, you’ll be able to apply it right away.”

”Of course, you could also maintain your own learning pace.”

Since Tong Yuan’an does not understand the Pulvertaft weaving method, Lu Cheng can offer some advice to Peng Kun.

Peng Kun has been learning tendon suturing for over a month, most of his time spent honing basic skills, and he urgently wants to perform legitimate surgeries to prove his strength by receiving feedback.

Lu Cheng understands this mindset, and indeed, Peng Kun has been quite disciplined in his studies lately.

Lu Cheng does not equate Xiang Kuihua and Peng Kun.

”Really?” If Lu Cheng says so, Peng Kun becomes interested!

”Yes, indeed, the operating techniques of the Pulvertaft weaving method aren’t overly profound and don’t require mastering suturing skills as meticulous as the Modified Tang Method...”

”If only a teacher had explained it to me before, I would have prioritized learning it.” Lu Cheng nodded.

Lu Cheng teaches Peng Kun partly because Peng Kun’s learning attitude is sufficiently honest and because during the mentoring process, it enhances the other person’s abilities, which, in turn, provides Lu Cheng with a modest amount of skill points.

For example, Du Daihua and Tian Zhuang, whom Lu Cheng mentored, contributed approximately 1.2 skill points to Lu Cheng after two months.

Not much, but effective in the long run!

Lu Cheng believes that in the future, the skill point income generated from mentoring students might be substantial.

Based on this, Lu Cheng isn’t stingy with his techniques. Because Tong Yuan’an and Chen Song shared resources generously with him, without much reservation.

As for past matters, Lu Cheng would settle with the few in orthopedics and Director Peng, not with Peng Kun.

”Thank you, Brother Lu...”

”You see, the core operations can be categorized into several steps: coarse tendon puncture opening, vertical cross puncture, embedding tendon ends, fish mouth trimming and wrapping...”

”First, we perform coarse tendon puncture opening, which involves horizontally piercing the first small hole on the coarse tendon end’s proximal side 2-3cm with a sharp knife, pulling out the fine tendon end through the hole, maintaining appropriate tension, then temporarily suturing two stitches to secure it at risk...”

”The vertical cross puncture involves a position 0.5cm distal from the first hole... Remember these two distances: one is 2-3cm, the other is 0.5cm...”

Lu Cheng’s familiarity with the Pulvertaft weaving method is not particularly high, only at the level of (skilled 5/10), but Lu Cheng didn’t invest too many skill points!

Yet, even with such familiarity, under professional circumstances, the treatment effect on tendon suturing in Area A3 still surpasses other suturing techniques.

...

In one surgery, Lu Cheng only operated for close to thirty minutes and completed it.

”This weaving suturing method, based on traditional suturing techniques, has two advantages: first, significantly enhancing tensile strength: due to multiple cross-stitches forming a woven structure, offering greater tensile strength compared to traditional suturing methods.” Lu Cheng said while pulling on the tendon, ensuring the ruptured tendon was steady.

Then Lu Cheng released the tendon: ”Next is reducing suture area bulging, embedding fine tendons and fish mouth wrapping to avoid local protrusion!~”

”Thus, after surgery, the patient’s sliding resistance and tensile force will be excellent, particularly suitable for suturing between tendons of varying thicknesses.”

”The teacher who created this suturing technique was also a genius, skillfully resolving this challenging issue. Moreover, the entry difficulty isn’t high, though the adaptation range is quite narrow...”

After watching, Peng Kun nodded and smiled, saying: ”It indeed looks like it doesn’t require very meticulous suturing skills, thank you, Brother Lu.”

”Actually, I didn’t expect you would teach me anything.” Peng Kun’s voice was earnest.

Xiang Kuihua in orthopedics has significant opinions about Lu Cheng, constantly trying to ’trap’ Lu Cheng, so Peng Kun never expected Lu Cheng would treat him generously.

”One thing is one thing, Young Master Kun, to put it plainly, in my heart, you and Director Peng are two different people.” Lu Cheng said directly.

In the past, saying this didn’t sting Lu Cheng, but now, having mastered the tendon suturing field and being the first surgeon in the hospital currently capable of doing multiple Spleen Preservation Surgery and Gallbladder Preservation Surgery,

He has already somewhat upheld the hospital’s reputation. Saying this with justification and reasoning, it isn’t idle fury anymore.

The positioning of Peng Kun and Lu Cheng was different, taking a deep breath, Peng Kun said: ”Brother Lu, although it may sound pretentious, there’s one thing I still need to say.”

”Whether I go to graduate school or learn tendon suturing, it is all based on not affecting you.”

”I don’t go for sponsored graduate studies, you, Brother Lu, wouldn’t get this placement either.”

”If I didn’t learn tendon suturing, Director Xiang Dahong from the State People’s Hospital wouldn’t come to our hospital.”

Peng Haibo ’found quite a few avenues’ for himself, but it did not affect Lu Cheng.

And previously in orthopedics, Lu Cheng also never had the opportunity, not just because Peng Haibo feared Lu Cheng would overshadow Peng Kun, but also because a few others controlled ’disease types’!

Now, even Peng Kun couldn’t squeeze in, having to abandon common surgical procedures in orthopedics, ’going off the beaten path.’

Compared to Fracture Open Reduction Internal Fixation Surgery, common surgeries in sports medicine and spine surgery, tendon suturing and hand surgery techniques in the County Hospital could also be considered unconventional technologies.

”That’s why, one thing is one thing.” Lu Cheng laughed and said.

Peng Kun thought for a moment and said: ”Brother Lu, I can affirm and guarantee that orthopedics won’t trouble you.”

Peng Kun could only ensure Peng Haibo wouldn’t, on behalf of orthopedics, pressure Lu Cheng; regarding other people’s personal choices, Peng Haibo wasn’t willing to sell this favor.
