「Xiao City.」

Wu Ting drove to Xiao City. He first went to the apartment that Teacher Shen Siyuan was letting him use rent-free, dropped off his things, and planned to grab lunch at the nearby food street before going to see Teacher Shen.

Just as he reached the second floor of the food court, he heard a sudden outcry. A crowd was gathered around something.

Wu Ting hurried over and saw a man lying on the ground in the center of the circle. The entire upper half of his shirt was soaked in blood, and the situation looked grim. Beside him was a frantic, weeping middle-aged woman.

The woman looked old enough to be the young man’s mother, but she had kept herself in good shape and had a nice figure.

”What happened?”

Wu Ting asked someone on the periphery of the crowd.

”Probably a love triangle. A guy in his forties just stabbed this young man. The attacker already ran off.”

”Someone already called the police and the paramedics.”

”I heard it’s a female professor from Jinggang University who got involved with one of her graduate students. Her husband found out. Today, the professor and the student came out to see a movie and get something to eat. The student posted a picture of them in matching outfits on social media, and the husband saw it...”

Listening to the gossip, Wu Ting got the gist of the story. If it was true, it was all pretty melodramatic.

Just as Wu Ting was about to step forward to check on the victim, the ambulance arrived.

However, this was on the second floor of the food court and indoors; the vehicle couldn’t get in.

A doctor and the driver rushed over, carrying a stretcher.

Wu Ting recognized him at a glance—it was a doctor from the emergency department of Jinggang University Affiliated First Hospital, one of Professor Shen Guofeng’s subordinates.

”Dr. Qian.”

Wu Ting immediately greeted him.

”Director Wu!”

Dr. Qian was surprised to see Wu Ting. He was an attending physician in the ER and was quite familiar with Wu Ting. Having followed Professor Shen Guofeng and witnessed many of Wu Ting’s surgeries, he knew Wu Ting was a formidable emergency physician.

He held Wu Ting in high regard and greatly admired him.

”Director Wu, come take a look with us.”

Dr. Qian was more than happy to have Wu Ting suddenly join them. At a time like this, having another expert on hand would definitely relieve some of the pressure.

”Everyone, please make way! We’re doctors from the emergency department of Jinggang University Affiliated First Hospital, and we need to provide first aid to the patient now!”

At Dr. Qian’s shout, the crowd parted, and the three of them reached the patient with the stretcher.

At that moment, the female professor stood up, her hands covered in blood.

”Doctor, you have to save him! He’s only twenty-five, in the prime of his life! Please, save him!”

The professor sobbed uncontrollably.

Dr. Qian knelt down and began his examination. One look was enough to make his scalp tingle with dread!

This was a classic penetrating injury to a major thoracic blood vessel!

For penetrating injuries to major blood vessels and their associated complications, the most critical steps are early diagnosis, hemorrhage control, repair of the damaged vessel, and treatment of any related visceral ruptures.

Needless to say, the mortality rate for this type of injury was extremely high.

It required the fastest possible diagnosis, rapid preoperative preparation, a swift surgical approach, effective CPR, timely hemorrhage control, and proper suturing and hemostasis techniques.

All of this demanded an incredibly high level of comprehensive skill from the doctor.

If not treated promptly and correctly, the patient could easily go into shock from the massive blood loss.

Given the limitations of the location, all they could do for now was apply initial hemostasis, then get him onto the ambulance for the next stage of treatment. They could only hope he’d make it to the hospital’s trauma room in time.

Dr. Qian didn’t dare be careless. For now, all they could do was apply immediate local pressure to stop the bleeding, pack the wound, and get the patient onto the ambulance right away.

Wu Ting assisted Dr. Qian, and with the driver clearing a path, they managed to carry the patient to the ambulance with great difficulty.

But just as they got him into the vehicle, they noticed the patient was experiencing chest tightness, difficulty breathing, and his face had turned pale. The wound was clearly bleeding profusely!

”This is bad. We’ve already done immediate wound packing and applied a pressure dressing, but there’s still a lot of seepage, and now there’s a fluctuating hematoma!”

As expected of a doctor from the emergency department of Jinggang University Affiliated First Hospital, his level of skill and experience was incomparable to that of the ER doctors at City First Hospital.

Of course, Wu Ting was not to be outdone. Noting the bleeding from the wound and the chest drainage, he performed a quick examination on the patient inside the ambulance.

”Dr. Qian, the patient’s shock is disproportionate to the amount of blood loss. Plus, his blood pressure and oxygen saturation are starting to drop... I suspect cardiac tamponade. There’s pericardial effusion!”

Hearing that the patient’s pericardium had been pierced and that he was developing cardiac tamponade, Dr. Qian’s expression turned grim.

Once pericardial effusion reaches moderate or large volumes, it can lead to acute cardiac tamponade in severe cases, which is a life-threatening condition.

But at this point, there was simply not enough time to get from here to the trauma room.

Moreover, due to the patient’s ruptured blood vessel and massive bleeding, Dr. Qian was still applying manual pressure to control the bleeding point. The situation was dire, and it was uncertain if the patient would even survive the trip to the trauma room.

For cardiac tamponade, the best approach is Pericardiocentesis. It serves as both a diagnostic measure and an emergency treatment to relieve the pressure. However, clinical opportunities to perform it are rare. Most emergency physicians are out of practice, and the procedure takes a long time.
