Chapter 402: Chapter 251: Diagnosis Confirmed, Too Late! Removal of the Right Eyeball

Wu Ting enjoyed a high degree of freedom at City First Hospital. Forget Dr. Sun; even their own Director Dou might not have been able to get her to come in.

After informing the patient, Dr. Sun left.

At that moment, Shi Lihong was truly filled with regret. Before, Director Dou and Dr. Sun had arranged everything for her; all she had to do was go. Now, because she had refused the examination, if she wanted to get an abdominal CT scan, there was no green channel for her. She could only get in line and wait like everyone else.

More than an hour later, it was finally her turn.

The radiology department performed an abdominal plain and contrast-enhanced CT scan for her.

Sure enough, they discovered a quasi-circular, slightly low-density lesion with ill-defined borders in the S4 segment of her liver, measuring 42mm x 44mm.

After a contrast scan of the lesion, it was finally confirmed that Shi Lihong had a liver abscess!

Following Wu Ting’s previous medical advice, a culture of the pus from the liver abscess was also performed.

With these results in hand, Wu Ting’s diagnosis was essentially confirmed.

Director Dou transferred Shi Lihong directly to the Hepatobiliary Surgery Department, where Cai Qi and Lu Xuejun began the next stage of her treatment.

When the lab results came back, Shi Lihong’s condition was officially diagnosed: Invasive Klebsiella pneumoniae liver abscess syndrome!

The mortality rate for liver abscesses is very high, as much as 30-40%!

Furthermore, the resulting endophthalmitis requires early intervention to have any chance of effectively improving the prognosis. Unfortunately, Shi Lihong had stubbornly wasted the optimal treatment window.

Cai Qi didn’t dare to delay and immediately began treating the patient.

Klebsiella pneumoniae is a highly virulent pathogen. The cause of Shi Lihong’s illness was mainly due to her diabetes combined with intestinal irrigations, which damaged her intestinal mucosa and allowed pathogenic substances from her intestinal contents to enter her circulatory system!

Fever is the most common symptom of invasive liver abscess syndrome, and endophthalmitis is also a classic one!

One could only say that Shi Lihong had brought this upon herself. She already had a top-tier doctor like Wu Ting who had found the cause of her illness. All she needed to do was complete the examinations and treatment to recover, but she delayed for an entire day because of her own stubbornness.

In the end, she missed the best window for treatment.

Under the care of Cai Qi and Lu Xuejun, her liver abscess could be completely cured, but her eye was not so lucky.

After another joint consultation with Director Dou from the Ophthalmology Department, the decision was made to remove her right eyeball!

Dr. Sun from Ophthalmology also updated his online post at the same time.

”A Bizarre Patient’s Treatment Journey: Hematuria after Four Colon Cleanses in a Diabetic, Followed by Persistent Eye Pain. What Was the Truth?”

At the very bottom, he posted the answer:

”Alright, everyone, the final results are in. The diagnosis is confirmed: Invasive Klebsiella pneumoniae (Kp) liver abscess syndrome!”

”The outcome is just rather unfortunate. The deputy director of our Emergency Surgery Department had already deduced this illness. All that was needed were some tests to confirm it, and treatment could have begun. There was a high probability she would have made a full recovery. It’s just a shame the patient insisted on not getting the tests. It wasn’t until a day later, when she started experiencing abdominal discomfort and the swelling and pain in her eyes grew more severe, that she begged for an examination and treatment.”

”Unfortunately, it was all too late. The patient ultimately received symptomatic treatment in our hospital’s Hepatobiliary Surgery Department, and the liver abscess symptoms were completely cured.”

”It’s just a shame that she permanently lost her right eyeball.”

”PS: A few extra tips. First, for Invasive Klebsiella pneumoniae (Kp) liver abscess syndrome, an abdominal B-scan can easily miss the diagnosis. An abdominal CT scan is necessary. The typical lesion for this disease is located in the right lobe of the liver. CT monitoring is more sensitive than ultrasound for liver abscesses! CT has a high specificity for ruling out liver abscesses and is the most sensitive imaging method for detecting them.”

”Second, the golden window for treating Invasive Klebsiella pneumoniae (Kp) liver abscess syndrome is within 48 hours of onset. Without early intervention, it is highly likely to progress from endophthalmitis to blindness, and even to eyeball removal.”

”Third, when a doctor recommends tests to rule out a disease, you must take it seriously. Delaying treatment can have truly tragic consequences. After the patient’s eyeball was removed, her husband directly filed for divorce, saying he couldn’t accept such a monster. What a tragic story!”

After this answer was posted,

Many netizens were in an uproar.

”That’s so tragic. But really, it’s her own fault. She had such an amazing doctor find the cause, and she still refused.”

”Learned something new. This is the first time I’ve ever heard of Invasive Klebsiella pneumoniae (Kp) liver abscess syndrome. I had no idea it was so dangerous.”

”Not gonna lie, guys, I showed this case to my director. Based on the initial examination data and indicators, our director couldn’t make a diagnosis either and would never have thought of this disease. And we’re a top-tier provincial hospital. So, this patient was really lucky to meet such a brilliant doctor who could decisively order an abdominal B-scan and CT scan based on just endophthalmitis. It’s a pity the patient brought about her own doom.”

”Yeah, seriously. It’s only after the diagnosis was confirmed, looking back, that you can see how incredible that deputy chief physician is. Just from a few abnormal liver function indicators, combined with diabetes, hematuria after intestinal irrigation, and fever, she deduced it was Invasive Klebsiella pneumoniae (Kp) liver abscess syndrome. I really want to know which hospital the original poster is from. So much respect!”

”It’s true, even a god-tier doctor can’t save someone who is determined to die. This patient must be kicking herself so hard right now.”
